Cellular S-100 protein immunostaining in human dysfunctional temporomandibular joint discs.

S-100 protein was detected immunohistochemically in diseased human temporomandibular joint discs with different degrees of pathology, and the findings compared with those of normal discs. In normal discs, large nerve trunks in the posterior ligament were strongly stained by anti-S-100 antiserum; the very few chondrocyte-like cells sometimes showed faint staining, while no staining was observed in any fibrochondrocyte-like or fibroblast-like cell. In dysfunctional discs, S-100 protein immunostaining seemed to correlate with structural pathological findings. The discs showing an abnormal collagen arrangement or fragmentation of collagen fibres presented overall the same immunolabelling pattern as normal discs. In discs with fibrocartilaginous metaplasia and dystrophic cartilage formation, fibrochondrocyte cells showed a very strong immunoreaction for S-100 protein and fibroblast-like cells in some instances were also positive. These findings suggest that S-100 upregulation in disc cells can be considered an attempt at tissue repair by chondroid metaplasia following an injury in that it enables fibroblast-like cells and fibrochondrocytes to acquire a chondrogenic phenotype.

Immunolocalization of CD44 adhesion molecules in human periradicular lesions.

OBJECTIVE: The hyaluronate receptor CD44 is a cell surface protein that is involved in several functions. To elucidate whether CD44 plays a role in periapical lesions, an immunohistochemical technique was used to study its distribution. STUDY DESIGN: Twenty periapical lesions-16 periapical granulomas and 4 radicular cysts-constituted the sample. Formalin-fixed/paraffin-embedded tissue sections were studied by means of immunohistochemistry for the presence of the standard CD44H form and its V3 splicing variant. RESULTS: Immunohistochemical staining for CD44H and CD44V3 was observed on epithelial, endothelial, and connective tissue cells. The cells of the fibrous lining around each granuloma were positive, showing an immune reactive pattern directly correlated with the dimension of the lesion. Epithelial rests of Malassez were strongly positive; the reaction product was also evident in the epithelial lining of the cysts. Blood vessels, mainly observed around the lesion, were immunoreactive for CD44. CONCLUSIONS: Our findings demonstrate that CD44H and its V3 variant are expressed in at least 3 different tissue types of periapical lesions. These glycoproteins may be involved in different steps of periapical lesion pathogenesis and evolution.

CD44 standard form (CD44H) expression and distribution in dysfunctional human temporomandibular joint discs.

The expression pattern of the cell adhesion molecule CD44 standard form (CD44H) in dysfunctional human temporomandibular joint (TMJ) discs was studied immunohistochemically and compared with normal disc pattern in order to evaluate the expression of this adhesion molecule and correlate it to histopathological changes. Immunohistochemistry with anti-CD44H antibodies was performed on paraffin sections of pathological and normal discs. In normal TMJ discs, a moderate immunolabelling with anti-CD44H antibodies was detectable in fibroblastlike cells, in the few fibrochondrocytes and in chondrocytelike cells. In dysfunctional discs, the staining pattern and intensity varied according to the histopathological findings of the specimens. The TMJ discs showing abnormal collagen arrangement or fragmentation of collagen fibres presented overall the same immunolabelling pattern of normal discs. In the discs showing areas of fibrocartilaginous metaplasia, CD44H expression was upregulated in fibrochondrocytes and fibroblastlike cells, especially around the chondroid tissue. Overall, these results suggest that CD44H mediates the binding of some ECM proteins in TMJ disc cells. The up-regulation of CD44H observed in some dysfunctional TMJ discs seems to indicate a prevention of apoptosis in fibroblastlike cells and an important role in phenotypical change of fibrochondrocytes into chondroblastlike cells, enabling the aggregation of chondroid tissue pericellular matrix components.

Alteration in alpha 2 integrin immunocytochemical expression on cultured human gingival fibroblasts following nicotine exposure.

BACKGROUND: Clinical epidemiologic studies carried out in smokers versus non smokers support the concept that tobacco-related factor may affect the periodontal tissues and wound healing. METHODS: In this study, the role of nicotine on the integrin alpha 2 chain immunocytochemical expression, in human gingival fibroblasts (HGF) was investigated in vitro. The kinetic induction of this type of integrin on HGF in response to increasing percentage of nicotine was been characterized. A human gingival fibroblast strain, derived from a healthy individual with non-inflamed gingiva, was used in this study. The cells were then grown on acetylated microscope slides and fixed with cold ethanol, samples were then incubated for 16 to 19 hrs at 4 degrees C to anti-human alpha 2 integrin chain monoclonal antibody. RESULTS: In control cultures and in HGF treated with 0.025 microM nicotine the initial higher expression of alpha 2 chain decreased (grade 1 in both culture) while in HGF treated with 0.075 microM increased (grade 3). After 48 hours HGF exposed to 0.075 microM nicotine, increased further their expression of alpha 2 chain. CONCLUSIONS: The results obtained demonstrate a dose-time dependent nicotine influence on immunocytochemical expression of alpha 2 integrin chain. These data suggest that nicotine may promote a collagene breakdown via an increase of alpha 2 integrin chain expression.

Anticardiolipin antibodies in pre-eclampsia and intrauterine growth retardation.

OBJECTIVE: To establish an association of anticardiolipin antibody (ACA) levels and pre-eclampsia or intrauterine growth retardation (IUGR). METHODS: Twenty-eight patients with pre-eclampsia, 28 with IUGR and 28 normotensive control group were matched for maternal age, race, weight, cigarette smoking, and parity. All had plasma anticardiolipin antibodies (GPL and MPL) detected by the modified enzyme-linked immuno-absorbent assay (ELISA) technique. RESULTS: No statistical significant difference in ACA values, both GPL and MPL, was found among the three groups studied Furthermore, none reached a value of ACA that could be considered clinically relevant (> 15). CONCLUSION: No association was found in anticardiolipin antibody levels between pre-eclamptic and IUGR versus the control group.

[Respiratory medicine in the aged. Therapeutic problemsd].

Chronic obstructive pulmonary disease (COPD) is very common in the elderly, and its treatment needs special concern. Data from two clinical studies (in-patients of an Internal Medicine Department and Nursing-Home, respectively) have been evaluated in order to compare therapeutic approaches to COPD in three separate settings: home, hospital and Nursing-Home. Less than 50% of the overall patients received medical treatment according to the guidelines emerged in the literature. Although cardiovascular problems were highly common in out-patients, their bronchodilator therapy included systemic beta 2-agonists. Theophylline was the main drug prescribed to in-patients discharged from hospital, although inhaling beta 2-agonists were also recommended in few cases. Topical steroids have been widely used. Nursing-Home patients received mostly systemic steroids and few of them had theophylline and inhaling steroids. Anti-cholinergic drugs have never been employed in the overall population of elderly patients. Data concerning medical treatment of infectious respiratory diseases of Nursing-Home patients (both with and without COPD) have also been evaluated. Antimicrobial therapy has been performed according to the special needs of the elderly. During COPD reacutizations, beta 2-agonists and, again, theophylline consumption increased.

[The cervical third of deciduous teeth. An ultrastructural study of the heard tissues by SEM].

As information on amelocemental junction of deciduous teeth is limited, this topographical area was investigated by scanning electron microscopy (SEM) to verify differences from that of permanent teeth. Twenty-six carious and non-carious human maxillary and mandibular primary teeth were placed in a fixative immediately after extraction. Pulpal tissue was removed from the pulpal chambers and root. The primary teeth blocked onto stubs and all specimens were platinum coated and examined by SEM. In these specimens an overlapping of cementum onto to enamel and an edge to edge relationship was dominant. No gaps between enamel and cementum were observed. The amelocemental junction of deciduous teeth seem to differ to that described for permanent teeth.

["Carious" and "noncarious" lesions of the hard dental tissues. Ultrastructural (SEM) and microanalytical (EDS) analyses of teeth from the 3rd century B.C].

The study was carried out on 80 teeth (70 of permanent dentition and 10 of primary one) of mandibular and maxillary bones. The teeth owned to subjects lived in the III c.b.C. in Sicily, and they were found in Naxos necropolis. The teeth were examined as far as carious and non carious lesions, then they were prepared for instrumental analyses by Scanning Electron Microscopy (SEM) and Energy dispersed X ray spectrometry (EDS). To an objective examination of the sample no caries were detected in the teeth sample apart one tooth, on the other hand there was an extensive occlusal abrasion, from a macroscopic point of view there were no difference as far teeth morphology. SEM examination pointed out all around dentinal tubules a circle of sclerotic dentin as reaction to occlusal wear. The amelocemental junction showed from a ultramicroscopic point of view an overlap of cementum onto enamel. EDS analysis pointed out a higher P concentration on the crown versus the root in all the teeth of the sample.

[Surgical treatment of congenital cysts of the liver].

We report 9 patients who were surgically treated for symptomatic congenital liver cysts. Median age at presentation was 51 years and all the cases were females. Symptoms and clinical findings included a painful mass in the upper right quadrant of the abdomen. Ultrasonography was used most frequently and was an accurate technique for diagnosing these lesions. The cysts were often located in the right hepatic lobe. Surgical technique included total excision (n = 6) and partial excision via fenestration (n = 3). Based in our limited experience we believe that fenestration constitutes the least aggressive approach with a high rate of symptom relief and should be considered as an alternative in the treatment of these lesions.

[Histological study of synchondroses and of the nasal capsule in the human fetal cranium].

The aim of this paper is to describe the development and growth of the middle-line cranial base synchondrosis and nasal septal cartilage of human fetuses. The sample consisted of 25 human fetuses aged from 9 weeks to 6 months. The samples were decalcified and processed in paraffin. Sections were cut at 10 mu and stained with Mallory staining. Histomorphological observation pointed out the presence and development of cranial base synchondrosis and nasal septal cartilage. As far as synchondrosis cell zone layers in endochondral growth sites are described moreover a caudorostral gradient in growth rate was observed. Functional connexions between the cartilaginous nasal capsule and the developing maxillae and premaxillae have been examined. The main connection was found to consist of the anterior septopremaxillary ligament running from the anterior border of the nasal septum posteroinferiorly to blend with premaxillary periosteum and interpremaxillary suture.

[Incidence of dyslipidemia and diseases associated with arteriosclerosis in a hospital population].

The authors present the results of a research on 3,888 patients (1,768 males and 2,120 females) admitted to the 1st Internal Medicine Department of Leghorn Hospital during two years (from July 1993 to June 1995). The research is intended to value the hyperlipidaemia's incidence in hospitalized patients, and the association between hyperlipidaemias and some diseases generally considered as consequences or favouring conditions of atherosclerosis. The results of the study show the not secondary role of hyperlipidaemias in hospitalization phenomen and their frequent association with important cardiovascular and metabolic diseases.

[Reversible broncho-obstruction (asthma and COPD) in the elderly patient. Epidemiologic, diagnostic, and therapeutic features].

In the field of respiratory diseases during aging, obstructive diseases hold a prominent position because they are frequent and are potentially "tractable" and thus subjected to careful therapeutic intervention. Bronchial asthma and chronic obstructive pulmonary disease are most important clinical patterns. Bronchial asthma (in chronic and in uncommon, newly diagnosed, forms) in the elderly are more frequent than commonly known. Wider and deeper studies of chronic obstructive pulmonary disease, besides, have demonstrated that a component of reversibility is present even in the older patients, in a ratio of 20-40%. The reversible bronchial obstruction appears therefore, in the older patients, as a qualitatively and quantitatively important event, in the clinical and therapeutical field. The recent production of international Guidelines (ATS/ERS, 1995) gives the possibility--for this type of diseases--of utilizing more accurate diagnostic criteria and new therapeutic approaches.

Anatomo-clinical considerations on the ovarian fibroma.

The Authors analysed from an anatomo-clinical point of view, 17 cases of ovarian fibroma, observed during the period between 1968 and 1992. The incidence of ovarian fibroma in this study was 2.52% and it affected women in different age groups, though with greater frequency those above 50 years. Even the symptomatology was variable and was characterized by pain in the hypogastric quadrant (47.1%); alteration of the menstrual cycle (17.6%); metrorrhagic episodes (17.6%); sterility (5.9%). Due to the fact that the ovarian fibroma can appear at any age, this suggests different surgical approaches according to age, general conditions of the patient and fertility.

[Respiratory medicine in geriatrics. A cultural niche of relevant theoretical and practical importance].

Respiratory medicine in the elderly represents a topic of increasing interest in the developed countries, because of its peculiar clinical and therapeutical implications. In this respect, a deeper knowledge of the biology and physiology of aging lung is desirable: namely, the age-related immunological and structural changes involving the respiratory system do influence clinical presentation and response to the treatment in older people. A better understanding of the pharmacology in the elderly which is recognized to be different for several aspects from that of the general population, is mandatory in order to better approach these practical problems. This short review emphasizes the need of the "longitudinal", rather than "transversal", epidemiological surveys which are uniquely able to give useful information on the natural history, even in older populations. In addition, clinical and therapeutical peculiarities of some respiratory illnesses such as infectious lung diseases, including pulmonary tuberculosis, COLD and bronchial asthma, sleep-related respiratory disorders will be underlined. Finally, a few problems concerning the management of the geriatric subject with chronic respiratory disease will be analyzed, focusing mainly on the difficulties inherent in the inhalant therapy and functional evaluation of this kind of patients.

Comparison of hemp and cotton fiber implants in muscle rat tissue. Study of the inflammatory response.

Hemp fiber is obtained from the plant Musa textilis. The cost of preparation of its raw fibers is low. The purpose of this paper was to compare the inflammatory response in the rat muscle tissue originated by both hemp and cotton fibers. Both types of fibers, were implanted in gluteal muscles of Sprague Dawley rats. The rats were sacrificed at 15, 30 and 60 postoperative days. Muscle tissue sections were stained with hematoxilyneosin. The inflammatory response was measured by subtracting the suture surface area from the total granulomatous area. At 15 days, the inflammatory response was more conspicuous for hemp than for cotton fiber (P < 0.05). At 30 and 60 days, responses were similar (P > 0.05). We cannot conclude that the hemp fiber is superior to cotton, nevertheless, they behave the same. Therefore, hemp constitutes an alternative as suture material.

[Computerized parametrization in orthognathodontics].

The relationship between unilateral posterior cross-bite and area and volumetric proportions of the palatal vault observed in the frontal plane were studied in 6 cross-bite cases before and after the correction of the malocclusion by removable appliances. Comparison of area and volume were carried out between the hemipalatal vault with the teeth in cross-bite and the hemipalatal vault with the teeth in good occlusion. The results demonstrated discrepancies of measurements between the two hemipalatal vaults before treatment. After the correction of cross-bite few or no asymmetries were recorded.
