Total colectomy with rectal mucosectomy and ileoanal anastomosis for chronic ulcerative colitis in children and young adults.

Total colectomy with rectal mucosectomy and ileoanal anastomosis has been utilized as a sphincter-saving operation in young people with chronic ulcerative colitis. From 1977 to 1979, our section of pediatric surgery performed this procedure on 12 children and young adults with chronic ulcerative colitis, with encouraging results. All patients are alive and well, and all have had excellent rectal continence. Follow-up ranges from 7 to 27 months. Eight patients describe an excellent result, two have had a fair result, and two have required a temporary ileostomy. Numerous loose stools have been observed early, but stools become formed by diet and medical management, and early return to school and work has been possible. The number of stools continues to decrease for at least 1 year.

Effect of proximal gastric vagotomy and truncal vagotomy and pyloroplasty on gastric functions and growth in puppies after massive small bowel resection.

The effects of truncal vagotomy and pyloroplasty and proximal gastric vagotomy on gastric acid hypersecretion, hypergastrinemia, and growth after massive bowel resection were studied in beagle puppies. In puppies with 80% enterectomy, neither type of vagotomy alters significantly the postprandial hypersecretion of acid from the Heidenhain pouch or the concentration of serum gastrin. Proximal gastric vagotomy tended to decrease the hypersecretion more than did truncal vagotomy. In beagle puppies undergoing 70% small bowel resection, growth was significantly decreased but survival was not impaired. Neither proximal gastric vagotomy nor truncal vagotomy and pyloroplasty reversed completely the impaired growth produced by massive resection. Proximal gastric vagotomy caused a small improvements in growth, while truncal vagotomy and pyloroplasty resulted in a slight decrease in growth. It remains possible that proximal gastric vagotomy could be of value in the management of growing infants with hypersecretion of acid due to short bowel syndrome.

Heterotopic splenic autotransplantation in prevention of overwhelming postsplenectomy infection.

An experimental study was undertaken to evaluate the protective effect of heterotopic splenic autotransplantation in weanling rats. Rats were divided into three experimental groups: splenectomy, control, and splenic autotransplantation. Rats were challenged with i.v. type I pneumococcus. Bacterial bloodstream clearance and survival were determined. Splenic bacterial uptake was measured by determining the isotopic activity of technetium-99m-labeled pneumococci. Autoradiographs and material stained with hematoxylin and eosin and Gram strains were examined for histologic features. All autografts survived and were histologically compatible with normal splenic tissue. Bloodstream clearance of pneumococci was significantly greater in rats with splenic autotransplantation. Splenic autografts had 10 to 30 times greater uptake of pneumocci than did the liver. Rats with autotransplantation had a prolonged survival time. Heterotopic splenic autotransplantation may prove to be an important adjunctive surgical measure in the treatment of children undergoing splenectomy.

Relative merits of partial splenectomy, splenic reimplantation, and immunization in preventing postsplenectomy infection.

Partial splenectomy, splenic autotransplantation, and immunization with pneumococcal vaccine have been reported to protect patients against overwhelming postsplenectomy infection, and this study was undertaken to evaluate these therapeutic alternatives. For this purpose 136 rats were divided into experimental groups: 34 controls, 34 splenectomy, 34 partial splenectomy, and 34 splenic autotransplantation animals. Five weeks after operation, two-thirds of the animals were immunized with killed pneumococci. The effects of operation and immunization were studied by challenging the animals intravenously with pneumococci. Pneumococcal antibody titers were determined, and phagocytic uptake of pneumococci by the spleen and liver was measured. Immunization impressively increased the survival rate in all groups. At low-challenge doses autotransplantation prolonged survival. At higher-challenge doses only partial splenectomy increased survival. Partial splenectomy and control animals had higher antibody titers than did splenectomy and autotransplantation rats. Animals with the highest antibody titers had the greatest splenic and hepatic phagocytic uptake of pneumococci. Partial splenectomy was more efficient in removing pneumococci than was autotransplantation. Thus immunization is one of the most important factors contributing to survival after splenectomy. Partial splenectomy is preferable to splenic autotransplantation because it is associated with higher antibody titers after immunization, better pneumococcal splenic uptake, and improved survival rates.

Surgery for thyroglossal duct and branchial cleft anomalies.

A retrospective review was carried out of 338 patients with thyroglossal duct remnants and 274 patients with branchial cleft anomalies who had undergone surgery at the Mayo Clinic from 1954 to 1975. Attention was directed to the preoperative history of these lesions, their clinical presentation, the nature of surgery performed, the complications of surgery, and the incidence of recurrence. The recurrence rate after the Sistrunk procedure for thyroglossal duct remnants was 2.6 per cent, whereas the recurrence rate after surgery for branchial cleft anomalies was 5.8 per cent. The findings suggested that these lesions are a significant source of morbidity and that surgery should be performed before infection occurs and should be radical in the first instance if the recurrence rate is to be minimized.

Bilateral massive nephroblastomatosis in infancy.

This report describes an infant with bilateral diffuse nephroblastomatosis, illustrating the typical clinical and pathologic characteristics that allow it to be differentiated from true bilateral Wilms' tumor.

Pancreaticocolonic fistula: a complication of pancreatic pseudocysts in childhood.

Two patients had an unusual complication of pancreatitis: pancreaticocolonic fistula, frequently associated with life-threatening gastrointestinal hemorrhage and sepsis. To avoid these complications, early diagnosis is important, but it may be difficult. Treatment consists of external drainage of the pancreatic pseudocyst or abscess and colonic diversion.

Hemodynamics of congenital diaphragmatic hernia in lambs.

Despite surgical treatment, congenital foramen of Bochdalek diaphragmatic hernia continues to carry a high mortality rate due to associated respiratory insufficiency. We studied the pathologic and hemodynamic changes that result from this condition in lambs. Surgical creation of diaphragmatic hernias in fetal lambs was performed in utero with subsequent delivery by cesarean section near term. Cardiac catheterization was performed on these newborn lambs immediately after delivery, before and after surgical repair of the defect. Data from five of these lambs were compared to data obtained from seven normal lambs. Pulmonary hypertension was found only in lambs with hernias. Pulmonary artery pressures and the mean ratio of pulmonary to systemic resistance were higher in experimental lambs. Oxygen saturation and average cardiac index were higher in normals. Lambs in the experimental group, but not in the control group, had large left-to-right shunts at the level of the ductus arteriosus in the early neonatal period. Arterial oxygen saturations were consistently low with no differences in pre- and post-ductal samples, which suggests right-to-left shunting at the atrial level or physiologic shunting across the unexpanded lungs. No further expansion of the hypoplastic lungs occurred following decompression by surgical hernia repair, but transient hemodynamic improvement was noted in some cases. Surgical ligation of the ductus arteriosus did not significantly alter the clinical condition of the lambs. Lungs in lambs with hernias were grossly abnormal, the left appearing more hypoplastic than the right.

Hypertrophic osteoarthropathy in association with pulmonary metastasis from osteogenic sacroma.

Hypertrophic osteoarthropathy in children is rare, especially when it is associated with lung metastsis. The patient reported herein had metastic osteogenic sarcoma and failed to respond to chemotherapy or thoractomy. There are several theories as to the pathogenesis of osteoarthropathy, but none is totally convincing. Vagotomy or transection of intercostal nerves has been advocated to relieve the symptoms of osteoarthropathy.

Resection of pulmonary metastatic osteogenic sarcoma in children.

Over a three year period, 28 children or young adults up to 20 years of age underwent 60 thoracotomies for pulmonary metastatic osteogenic sarcoma. These patients also received adjuvant chemotherapy. Sixteen of the 28 patients are currently alive and 13 of these are free of disease. The patients have been followed for 6 to 48 months after the initial thoracotomy; median survival is 25 months. Actuarial survival curves produced a 4 year survival of 57%. Although a tumor-free interval of more than 1 year greatly enhanced survival, three patients survived with tumor-free interval of 6 months or less. Patients with disease confined to one lobe at the initial thoracotomy did very well, and yet survivors were seen when disease involved more than one lobe or was bilateral. An aggressive surgical approach toward the pulmonary metastases thus appears to be justified.

Use of tolazoline in newborn infants with diaphragmatic hernia and severe cardiopulmonary disease. A preliminary report.

Two newborn infants with congenital diaphragmatic hernia, one of whom died, had significant improvement in arterial oxygen tension (Pao2) after intravenous administration of tolazoline (Priscoline) (1 to 2 mg. per kilogram). In both infants, systemic hypotension developed within minutes of administration of the drug and required pharmacologic and hemodynamic intervention. The response to tolazoline was more dramatic in the infant who survived, and his oxygen requirements were significantly reduced after the use of this drug. The infant who died also had a significant response to tolazoline. Tolazoline appears to be an important pharmacologic agent for use in the postoperative care of infants with diaphragmatic hernia and associated hypoxemia and acidosis.

Increased meal-stimulated gastric acid secretion and serum gastrin after extensive small bowel resection in puppies.

In six beagle puppies provided with vagally denervated pouches of the oxyntic gland area, the effects of massive resection of the small intestine on acid secretion and serum gastrin induced by feeding were studied. The enterectomies produced significant postprandial hypersecretion of acid and hypergastrinemia. A linear correlation existed between the increased acid output and serum gastrin level.

Simultaneous adrenal and cervical pheochromocytomas in childhood.

Pheochromocytoma is an uncommon tumor in childhood. The simultaneous occurrence of adrenal and cervical pheochromocytomas is a rare phenomenon; to our knowledge, this combination has been reported in the literature only once. Cervical pheochromocytomas are more accurately termed "aorticosympathetic paragangliomas." In children, 50% of pheochromocytomas are bilateral, multiple, or extra-adrenal. An increased familial incidence in the form of simple mendelian dominance is also noted in the pediatric age group. Bolus nephrotomography is extremely effective in identifying adrenal pheochromocytomas in children. To prepare the patient for surgery, and alpha-adrenergic blocking agent is administered from 7 to 10 days before operation and a beta-blocking agent is administered 3 days before. A transabdominal approach is essential because of the frequent extra-adrenal sites and multicentricity of the tumor in children.
