A case-control study of uterine endometrial cancer in Japanese and Finn.

The difference between the endometrial cancer incidence in Japanese and Finnish women (lower and higher incidence, respectively), was evaluated on the basis of data from cases of endometrial cancer, cervical cancer and benign gynecological disease in both countries. The comparison took into account the various personal and clinical characteristics of these cases. In endometrial cancer, Japanese and Finnish women had similar characteristics except for the age at first delivery, the age at last delivery and obesity. However, obesity in postmenopausal women in the two countries was similar. Common factors in the two countries included few pregnancies and deliveries, nullipara and single women. In cervical cancer, no difference between the characteristics of Japanese and Finnish women was found except that Japanese women had a higher frequency of pregnancy. In benign diseases, characteristics were similar to those of endometrial cancer in Finnish women, but this was not the case in Japanese women. These facts may indicate the number of Finnish women with endometrial cancer risk factors is greater than the number of Japanese women with these risk factors. This was thought to account for the difference in the incidence of endometrial cancer.

Vaginal hysterectomy without ligation of the ligaments of the cervix uteri.

An operative procedure for vaginal hysterectomy is reported. The procedure is performed without ligation of the paracervical ligaments to simplify and avoid ureteral injury. However, a portion of the cardinal ligament is ligated before peritoneal closure. In a study from 1955 to 1987, of 9,230 patients requiring vaginal hysterectomy, blood loss was less than 300 milliliters with this procedure in 83 per cent of the patients. Operative complications, such as bladder injury, occurred in 66 patients or 0.7 per cent. Ureteral injury occurred in only three patients or 0.03 per cent. From 1972 to 1987, only five of 2,460 patients (0.02 per cent) required postoperative hemostasis. These data indicate that vaginal hysterectomy without ligation of the paracervical ligaments is a safe and convenient operative method with fewer complications.

ras oncogene expression and prognosis of invasive squamous cell carcinomas of the uterine cervix.

By using anti-ras p21 mouse monoclonal antibody rp35, the authors studied the reactivity of 170 squamous cell carcinomas of the uterine cervix of different histologic types. The overexpression of p21 was noted in 57.1% of keratinizing type (K type) and 54.2% of large cell nonkeratinizing type (LNK type), but only 38.7% of small cell type (S type). Upon statistical analysis of the correlations between p21 overexpression and patient prognosis with the Kaplan-Meier method and generalized Wilcoxon text, the p21-positive K and LNK types showed poorer prognosis, whereas the positive S type showed better prognosis in comparison with the negative cases. These findings suggest that the expression of ras p21 is one of the parameters related with prognosis of cervical cancers, but the mode of the correlation is dependent on their histologic types.

[The production and characterization of monoclonal antibody, 1C5, reactive with cervical adenocarcinoma of the uterus].

A new monoclonal antibody, 1C5, was produced by fusion of spleen cells obtained from mice immunized with CAC-1, a human cell line of cervical adenocarcinoma of the uterus, and NS-1 myeloma cell. The objectives of this study were to obtain moAb that can be used for routine histology and cytology, and to examine the histogenesis of cervical adenocarcinoma. 1. 1C5 reacted with 88% of cervical adenocarcinoma of the uterus, but did not react with cervical squamous cell carcinoma of the uterus and other squamous cell carcinoma. However, 1C5 reacted with some adenocarcinomas, such as endometrial carcinoma of the uterus and ovarial carcinoma. 2. The staining pattern by 1C5 was different, in cervical adenocarcinoma from that in endometrial carcinoma of the uterus, and also different in the endocervical type from that in the endometrioid type of cervical adenocarcinoma. Therefore, 1C5 is useful in distinguishing between two types of adenocarcinoma of the uterus. 3. 1C5 did not react with normal squamous cells or normal columnar cells of the uterine cervix, or with normal endometrial cells of the uterus. However, the columnar cells in a limited area of the squamocolumnar junction were strongly stained with 1C5. 4. 1C5 reacted with ethanol-fixed, and routine formalin-fixed and paraffin-embedded tissue. Thus, 1C5 may be used for clinical diagnosis. 5. 1C5 was found to be IgG1. 6. The molecular weight of the 1C5-defined antigen was 26,000 daltons, and the epitope of the 1C5-defined antigen was carbohydrate moiety. 7. Immunohistological identification of T and B cells in normal and malignant tissues of the uterine endometrium.

We examined the state of T and B cells in tissues from 61 cases of endometrial cancer by immunohistochemistry with monoclonal antibodies. In cancer tissues, T cells frequently appear in clusters at the tip of the infiltrating cancer. As to age, both T and B cells were rare in 9 out of 11 cases involving endometrial cancer in patients in their 40s. In the normal endometrium, infiltration of numerous T cells was seen in only 2 out of 27 cases (7.4%). However, it was seen in cancer tissues in 28 out of 61 cases (45.9%). Thus the infiltration of numerous T cells into cancer tissues was confirmed. Within the same uterus, T cells seldom infiltrated into the area of the benign endometrium while T cells were seen in large numbers in the cancer foci. This indicates that T cells infiltrate after having recognized cancer cells. Compared to stages I and II, T cell infiltration tended to decrease in stages III and IV. While no fixed relation was found between the degree of histologic differentiation and T cell infiltration, fewer T cells were observed in the cases where cancer penetrated to the depth of cancer invasion and where it occupied a large area. Thus, it was assumed that in the host-immune defence mechanism in particular, T cell infiltration played a key role in tumor immunology of patients with endometrial cancer.

New monoclonal antibody, 1C5, reactive with human cervical adenocarcinoma of the uterus, with immunodiagnostic potential.

A murine monoclonal antibody, 1C5, was produced by fusion of spleen cells obtained from mice immunized with CAC-1, a human cell line of adenocarcinoma derived from uterine cervix, and NS/1 myeloma cells. 1C5 can be used for the staining of routine formalin-fixed and paraffin-embedded tissue sections. 1C5-defined antigen was found to have a molecular weight of 26,000. The 1C5-defined antigen was resistant to neuraminidase and trypsin treatment, but sensitive to periodate treatment, indicating that an epitope of the 1C5-defined antigen is a carbohydrate moiety. Immunohistochemical study using immunoperoxidase staining demonstrated that 1C5 reacted with 87% of adenocarcinomas of the uterine cervix, 39% of endometrial carcinomas of the uterus, 100% of ovarian mucinous cystadenocarcinomas, 43% of ovarian serous cystadenocarcinomas, 45% of adenocarcinomas of the colon, and 40% of gastric adenocarcinomas, thus showing the broad reactivity to adenocarcinoma cells of various origins. However, 1C5 did not show any reactivity to ectocervix epithelium, cervical intraepithelial neoplasia, or squamous cell carcinoma of the uterine cervix. In addition, adenocarcinoma of the uterine cervix exhibited strong cytoplasmic reactivity with 1C5, whereas endometrial carcinoma of the uterus showed the luminal reactivity. 1C5 also reacts with 95% ethanol-fixed malignant cells in cervical smears.

Identification of transferrin receptor in cervical and endometrial tissues.

The location of transferrin receptor (Tf.R) which is abundantly contained in proliferating cells and positional relationships of cells containing much Tf.R was observed on cervical and endometrial tissue sections by immunohistochemistry. In normal squamous epithelium, the parabasal cell layer was positive and the basal, intermediate, and superficial cell layers negative. This shows that the cells of the parabasal cell layer are proliferating most actively among those layers. Squamous carcinoma contained many positive cancer cells. But negative as well as positive cells were present, indicating that cancer cells proliferate under various conditions. Cancer cells infiltrating the stroma were made clear by Tf.R staining, and it became easier to identify them when only a few were present. Endometrial glandular cells in the basal layer stained only on the basal side of a cell regardless of the menstrual cycle, whereas in the functional layer glandular lumen side also stained positive in the secretory phase. Based on the present data, an immunohistochemical study of Tf.R seems useful to confirm the localization of proliferating cells and cancer cells and determine the conditions for cancer cell proliferation on the tissue sections.

The cytological features and DNA content of cervical adenocarcinoma.

The relationship among cytological features, DNA content, and degree of histological differentiation of cervical adenocarcinoma was investigated in an attempt to discover a more accurate means of screening for this cancer. In highly differentiated adenocarcinoma (so-called adenoma malignum), the nuclei were only somewhat more irregular in size and shape than those of normal columnar epithelial cells. The cells were arranged in slightly multilayered clusters. The cells of well-differentiated adenocarcinoma were usually columnar in shape, and they exfoliated side by side in clusters. In moderately differentiated adenocarcinoma, solitary cells with markedly atypical nuclei were combined with multilayered cell clusters. The cells from poorly differentiated adenocarcinoma were roundish, occurred as solitary cells or irregularly overlapping cell clusters, and showed markedly atypical nuclei. As the degree of histological differentiation decreased, as determined by measurement of the DNA content of the cells, the DNA distribution covered a wider range in terms of ploidy, and the number of cells exceeding tetraploid DNA content increased.

Yolk sac tumors of the ovary and the human yolk sac.

In the present study a comparison was made between human yolk sacs and yolk sac tumors. Tubules surrounded by several to as many as 10 endodermal cells and intracellular tubules in one endodermal cell were frequently observed. The tubules were seen abundantly in the yolk sac of a 4-week pregnancy, and they resembled the reticular pattern of the yolk sac tumor. It was also observed that the papillary endoderm, which contained blood cells in the center and protruded into the endodermal tubules, resembled the Schiller-Duval body of yolk sac tumor. Ultrastructurally the tumor cells were similar to the yolk sac endoderm. alpha-Fetoprotein was positive in the yolk sac endoderm until approximately the seventh week of pregnancy. Yolk sac tumor was also alpha-fetoprotein-positive. In other words, our study of human yolk sacs of 4- to 11-week pregnancies presumes that the yolk sac tumor resembles the endoderm of 4- to 7-week pregnancies.

Serum lipids in Finnish and Japanese postmenopausal women.

Serum lipids were measured in 30 Finnish and Japanese postmenopausal women. Total cholesterol, HDL cholesterol, LDL cholesterol, apo B and the HDL cholesterol/apo A1 ratio were higher in Finnish than in Japanese women. The LDL cholesterol/apo B and apo A1/apo B ratios were lower in Finns than in Japanese. In serum phospholipids the percentage of arachidonic acid was higher and the percentage of the n - 3 fatty acids, eicosapentaenoic acid and docosahexaenoic acid, and the eicosapentaenoic acid/arachidonic acid ratio were lower in Finnish than in Japanese women. No significant correlations were found between serum oestrone levels and lipid parameters.

Serum selenium, vitamin A, vitamin E and cholesterol concentrations in Finnish and Japanese postmenopausal women.

Serum concentrations of vitamins A and E, selenium, and cholesterol were measured in 30 Finnish and 30 Japanese postmenopausal women. While both groups had identical serum vitamin A levels, the other serum factors differed significantly between the groups, the Finns having higher serum vitamin E and cholesterol and lower serum selenium levels. Adjusted to serum cholesterol, which correlated significantly with vitamin E in both groups, the vitamin E levels were, however, similar in the groups. The differences in the serum levels of these nutritional factors may be associated with the differences in the risk of coronary heart disease and cancer between Finnish and Japanese populations.

The role of adenosinergic, GABAergic and benzodiazepine systems in hyperemotionality and ulcer formation in stressed rats.

The effects of benzodiazepines, GABA and adenosine on distress-induced hyperemotionality and gastric lesion formation were investigated in rats. Hyperemotionality such as struggling, vocalization and defecation evoked immediately after immobilization stress were attenuated by diazepam, adenosine or adenosine plus diazepam. Conversely, pretreatment with these drugs produced rapid and potent exacerbation of gastric lesions observed after 12 h of stress. The potent adenosine A1-receptor agonist N6-cyclohexyl adenosine (CHA) markedly inhibited the distress-evoked hyperemotional behaviors and potentiated the ulceration. gamma-Aminobutyric acid (GABA), muscimol, a GABA receptor agonist, and aminooxyacetic acid (AOAA), a GABA deaminase inhibitor, attenuated both stress-induced hyperemotionality and ulceration. The inhibitory effects of diazepam and GABA on hyperemotionality were reversed, respectively, by Ro15-1788, a benzodiazepine receptor antagonist, and bicuculline, a GABA receptor antagonist. The stimulatory effect of CHA on stress ulceration was potentiated by bicuculline but was not affected by Ro15-1788 or by picrotoxin, a chloride channel inhibitor. These results suggest that the mechanism involved in gastric lesion formation induced by immobilization stress may be different from that in hyperemotional behavior, and that the activation of GABAergic neurons may act as a central modulating factor in the hyperemotionality and ulceration induced by immobilization stress.

[Reorganization of human ovarian adenocarcinoma cells and endometrial adenocarcinoma cells cultured in double-layered floating collagen gels].

Collagen gels have been used as the substratum for epithelial cell culture to maintain their differentiated states. In this report, we proposed a double-layered floating collagen gel method for the culture of human carcinoma cells to show their differentiated structures which closely resembled those in the tumors in vivo. On the 3rd week of the culture, OMC-1 cells (human ovarian mucinous adenocarcinoma) showed organized ductular structures. PAS-positive substances were seen along the surface of the ductules and also in cytoplasms. Electron microscopically, numerous microvilli were observed at the apical surfaces of the cells, with formation of rootlets. Ishikawa line cells (human well-differentiated endometrial adenocarcinoma) formed tubular structures with back-to-back arrangements. Electron microscopically, numerous microvilli, well-developed junctional complexes, and well-developed mitochondria and Golgi apparatus were observed, similarly to those in the cells grown in vivo. SNG-M cells (human moderately-differentiated endometrial adenocarcinoma) were arranged in sheets with occasional formation of tubular structures. Mitochondria, Golgi apparatus and desmosomes were poorly developed in general; however, the cells forming tubular structures showed signs of cellular differentiation. The floating and non-floating methods were compared with the result that the former method was seen to be far superior to the latter for expressing cellular differentiation.
