The Wisconsin epidemiologic study of diabetic retinopathy. IV. Diabetic macular edema.

The prevalence of macular edema and its relationship to a number of risk factors were examined in a population-based study in southern Wisconsin. Macular edema was determined from its presence on stereoscopic fundus photographs or from past history as recorded and documented in clinic records and photographs. For participants whose age at diagnosis of diabetes was less than 30 years and who were taking insulin (n = 919), prevalence rates of macular edema varied from 0% in those who had diabetes less than 5 years to 29% in those whose duration of diabetes was 20 or more years. In these persons, macular edema was associated with longer duration of diabetes, presence of proteinuria, diuretic use, male gender and higher glycosylated hemoglobin. For those whose age at diagnosis was 30 years or older (n = 1121), prevalence rates of macular edema varied from 3% in those who had diabetes less than 5 years to 28% in those whose duration of diabetes was 20 or more years. In these persons, presence of macular edema was associated with longer duration of diabetes, higher systolic blood pressure, insulin use, higher glycosylated hemoglobin, and presence of proteinuria.

Intraocular pressure in diabetic persons.

Intraocular pressure measurements were taken in 2366 diabetic persons and 381 nondiabetic persons who lived in southern Wisconsin. Diabetic persons tended to have higher mean intraocular pressure than the nondiabetic persons. Higher blood pressure, earlier time of day of IOP measurement, absence of cataract and, in some comparisons, female gender, were significantly associated with higher intraocular pressure. In this study rates of a positive history of glaucoma were higher in diabetic persons than in nondiabetic persons and the population participating in the Health Interview Survey. These findings suggest that ophthalmologists must be aware of the increased risk of glaucoma when evaluating diabetic patients.

Prevalence of diabetes mellitus in southern Wisconsin.

The prevalence of known cases of diabetes was ascertained from patient records in an 11-county area in Wisconsin in July 1979 to June 1980. The prevalence in the noninstitutionalized population was found to be 1%. Prevalence rates rose from 0.5 cases per 1000 population in the first decade of life to 44.7 cases per 1000 population in the eighth decade. The highest rates were found in the nursing home population. Males had higher age-adjusted rates than females. No difference in prevalence was found between persons living in metropolitan or nonmetropolitan counties. Determination of prevalence by these methods is a rapid and inexpensive means of ascertainment of known cases of diabetes.

Gravidity and diabetic retinopathy.

The relationship between gravidity and severity of diabetic retinopathy was investigated in a population-based sample of diabetic patients. All of these women had had diabetes diagnosed before 30 years of age and were taking insulin. All received their primary medical care in an 11-county region of southwestern Wisconsin (Health Service Area 1). Each participant had an ophthalmologic examination which included stereoscopic fundus photography of seven standard fields in each eye. Photos were graded according to a standard protocol. Of the 397 women, 197 had never been pregnant, 88 had been pregnant once, 56 had been pregnant twice, and 56 had been pregnant three or more times after diabetes was diagnosed. The number of pregnancies was positively associated with the severity of retinopathy. However, after controlling for duration of diabetes, this relationship was no longer apparent. Using an ordinal regression model, duration of diabetes, diastolic blood pressure and glycosylated hemoglobin were significant predictors of the severity of diabetic retinopathy.

Extracellular lumazine from aggregating Dictyostelium discoideum cells. Influence of pH on its fluorescence.

Lumazine has been demonstrated to be one of the two main compounds responsible for the extracellular fluorescence linked with the aggregation ability of Dictyostelium discoideum, strain Ax-2. The other compound, also having lumazine properties, is, however, different from the 7-hydroxylumazine proposed previously. The influence of pH on the fluorescence of lumazine was studied. The possible use of extracellular pteridines as pH markers was stressed and a method for the determination of the amount of "lumazine equivalents" in the extracellular medium of aggregating D. discoideum cells is elaborated.

Blood pressure in a population of diabetic persons diagnosed after 30 years of age.

Blood pressure levels were investigated in a population of diabetic persons who were diagnosed at 30 years of age or older and who received their primary medical care in an 11-county area in Southern Wisconsin. Prevalence of hypertension in these 5,104 persons was compared with prevalence figures from the Health and Nutrition Examination Survey (HANES) population. Diabetic women had a higher prevalence of hypertension until 65 years of age than did women of similar ages in the HANES. The diabetic males had a prevalence of hypertension similar to that of male HANES participants. Mean systolic blood pressures tended to be slightly higher in persons with "poorest" level of glucose control compared to those whose blood glucose was "best" controlled. Persons for whom diuretic agents had been prescribed were as likely to have blood glucose values in the "poorest" levels of control as persons not using diuretics.

Demonstration of organ specific antibodies against heart mitochondria (anti-M7) in sera from patients with some forms of heart diseases.

Using submitochondrial particles (SMP) from beef heart, pig kidney and rat liver in the ELISA, we detected partial organ specific anti-mitochondrial antibodies (AMA) against heart and kidney SMP in sera from patients with different forms of cardiomyopathies. Serum samples from 50 of 159 patients with congestive or hypertrophic cardiomyopathy (31%) and from two of 15 patients with acute myocarditis (13%) were AMA positive. These AMA could be clearly differentiated from other known AMA (anti-M1-M6) and were therefore named anti-M7. Thirteen of the 52 sera (25%) reacted only with heart SMP (type a) and 39 showed a cross-reaction with kidney, lung and pancreas mitochondria (type b). However, using liver SMP, no positive reaction was found. The anti-M7 type a and b activity was abolished completely by absorption with heart SMP. The anti-M7 antibodies were directed against an antigen which co-purified with the inner mitochondrial membrane and had a molecular weight of 67,000-72,000. They seem to be confined to some forms of cardiomyopathies and myocarditis of unknown aetiology and were not detected in sera from patients with other diseases.

The politics of ideology vs. the reality of politics: the case of Britain's National Health Service in the 1980s.

In 1979 the British people elected a government that explicitly repudiated the basis of post-war political consensus; Americans did likewise in the following year. New policies, it was expected, would be shaped by a new ideology. Britain's National Health Service offers an opportunity to examine the nature of this relationship. Political, professional, and corporate ideologies about resource allocations are inevitably constrained by the prevailing public philosophy.

Trichinosis in southern Lebanon.

An outbreak of trichinosis in two villages in southern Lebanon affected over 100 patients, aged 3 to 70 years, who celebrated Christmas and New Year's of 1981 by feasting on rare pork. Six were hospitalized in Israel. The diagnosis was made on clinical and histological grounds. The clinical picture included the classical features, but was unusual in that rarely reported pedal or pretibial edema, pruritus and vertigo, were prominent. There was one case with suspected myocarditis. Two women in the first trimester of pregnancy had miscarriages. All patients recovered. With the unstable political situation in Lebanon in recent years, veterinary and sanitary supervision has deteriorated, and most pigpens are infected with trichinosis.

Visual impairment in diabetes.

Visual acuity was measured in a population-based study of diabetic retinopathy in southern Wisconsin. Persons diagnosed prior to 30 years of age and taking insulin (younger onset, n = 996) and those diagnosed at 30 years of age or older (older onset, n = 1370) were examined. Best corrected visual acuity was determined using the Early Treatment of Diabetic Retinopathy Study protocol. In the younger onset group, 1.4% had moderate visual impairment (best corrected visual acuity in the better eye of 20/80 to 20/160) and 3.6% were legally blind (visual acuity in the better eye of 20/200 or worse). Visual impairment in this group was associated with older age at examination, longer duration of diabetes, presence of proliferative retinopathy, and presence of senile cataracts. In the older onset group, 3.0% had moderate visual impairment and 1.6% were legally blind. Visual impairment in this group was associated with older age at examination, longer duration of diabetes, presence of senile cataract, presence of macular edema, and proliferative diabetic retinopathy. When assigning causes of impaired vision, diabetic retinopathy was responsible in part for 86% of eyes with visual acuity of 20/200 or worse in younger onset persons and for 33% in older onset persons.

Use of ATPase-associated antigen (M2) for detection of antimitochondrial antibodies in primary biliary cirrhosis by fluorometric immunoassay.

An indirect binding assay, the fluorometric immunoassay (FIAX), was established for the detection of anti-M2 antibodies which are specific markers for primary biliary cirrhosis (PBC). Submitochondrial particles (SMP) from beef heart and rat liver and the ATPase-associated antigen (M2) were used. The antigens were fixed to a cellulose acetate surface, SMP at a concentration of 2 mg/ml, ATPase at a concentration of 0.2 mg/ml. Sera were used at 1:60 and 1:120 and bound antimitochondrial antibodies (AMA) were demonstrated by fluorescent isothiocyanate labelled monospecific anti-human IgG, IgM and IgA antibodies. The fluorescent signals were proportional to the AMA titre in the serum samples and were measured in a fluorometer (FIAX 100). Of 94 patients with PBC, 92 had AMA against SMP from beef heart compared with 76 in the complement fixation test (CFT) and 84 in the immunofluorescence test (IFL). Ninety reacted with the ATPase-associated M2 antigen. Sera from patients known to have AMA of different specificities (anti-M1, anti-M3, anti-M5, anti-M6) reacted with SMP from beef heart and/or rat liver but not with M2.
