Intussusception of the vermiform appendix: a preoperative diagnosis in an adolescent girl.

Intussusception of the vermiform appendix (IVA) in a 14-year-old girl is reported. The diagnosis was made preoperatively, which is rare: only five other cases have been reported. IVA can present with variable symptoms. This patient presented with episodes of recurrent severe paroxysmal pain and vomiting, each episode being separated by several uneventful weeks. The diagnosis of IVA could be made at the fourth attack of colic by the radiological visualization of a "spiral shell" filling defect at the bottom of the cecum that was reducible by intravenous injection of a spasmolytic. Laparotomy with appendectomy rescued the patient from these distressing episodes. Although IVA is an uncommon condition, one should be aware of its existence and include it in the differential diagnosis of acute abdominal syndromes. When confronted with a clinical picture of intussusception in older children or adolescents, one should especially remember this entity as a differential diagnosis with other causes of intussusception, especially tumors.

The sonographic aspects of haemobilia. Clinical and experimental study.

The sonographic aspect of the gallbladder in seven cases of haemobilia is analysed. The changes include a diffuse echogenic gallbladder content during the initial stage. Later, irregular shaped inhomogeneous non shadowing masses are seen in the dependent parts of the lumen. This variable aspect was studied in an experimental model by percutaneous injection of blood in the gallbladder of Guinea pigs. The hyperechoic gallbladder content, seen early after injection is a transitory phenomenon which seems related to red cell aggregation before coagulation occurs. During the later stage the intraluminal masses were shown by histology to represent bloodclots. In a patient the observation of such a rapid evolution from diffuse hyper-reflectivity to less reflective masses is strongly suggestive of haemobilia.

Sonographic diagnosis of intraluminal bile duct neoplasm: a report of 3 cases.

Intraluminal growing tumors of the bile duct are uncommon causes of jaundice. The sonographic appearance of 2 hilar cholangiocarcinomas or Klatskin tumors and a benign extrahepatic biliary cystadenoma is described. Compared to contrast studies of the bile ducts, sonography better defined the intraductal character of the neoplasms. However, the ultrasound appearance did not allow differentiation between the adenocarcinomas and the benign cystadenoma.

Erythrocyte swelling after rapid dilution of cryoprotectants and its prevention.

These in vitro studies on canine red blood cells confirm that cell swelling occurs after rapid dilution of Me2SO and glycerol. Cells loaded with a penetrating cryoprotectant in a medium with low Na+, high K+ composition present significantly less swelling after rapid dilution of the cryoprotectant than cells exposed to an electrolyte medium characterized by high Na+, low K+ composition. The osmotic cell stress during rapid dilution of Me2SO can be completely counteracted by the simultaneous use of the nonpenetrating sorbitol during exposure and loading. However, the addition of sorbitol is of no important benefit when glycerol is used as the intracellular cryoprotectant. This is probably due to the slower elution of glycerol. Thus utilizing a perfusion solution containing sorbitol during loading and dilution of Me2SO reduces the osmotic injury and may greatly improve the survival prospects of cryopreserved organs by avoiding "out-flow" block.

Natural history of recurrent Crohn's disease at the ileocolonic anastomosis after curative surgery.

An endoscopical and histological study was carried out in 114 patients, treated by 'curative' resection of the terminal ileum and part of the colon for Crohn's disease, in order to study the natural history of recurrent Crohn's disease. The recurrence rate of Crohn's disease in patients examined within one year of the operation was 72%. This figure did not differ significantly from that in patients examined one to three years or three to 10 years after surgery (79% and 77% respectively). Recurrence was located in the neoterminal ileum and at the anastomosis in 88% of the patients. Early endoscopic signs of recurrence were small aphthous ulcers in the neoterminal ileum. Ileal biopsies at this stage showed an important inflammatory cell infiltrate of the lamina propria with numerous eosinophils and fusion and blunting of the villi. More advanced lesions observed in patients examined one to three years after surgery, consisted of larger, often serpiginous ulcerations and nodular thickening of folds. In patients examined three to 10 years after the operation, the anastomosis was frequently stenosed and rigid, with large ulcers extending from the stenosis into the colon. Mucosal granulomas may be found in normal appearing mucosa as well as in the obviously inflamed mucosa surrounding the ulcers. These studies suggest that recurrence of Crohn's disease almost always develops in the first year after the operation. Significant endoscopic lesions may be present without clinical symptoms, particularly in the earlier stages of the disease.

The effects of different concentrations of glycerol and dimethylsulfoxide on the metabolic activities of kidney slices.

Kidney slices either were exposed to the cryoprotectants for 1 hr at room temperature and subsequently washed and incubated in fresh KR buffer containing only the radioactive metabolic tracers, or were immediately incubated for 2 hr at 37 degrees C in KR buffer containing the cryoprotectant and the tracers. Exposure to glycerol by incubation of kidney slices in Krebs-Ringer bicarbonate buffer containing varying concentrations of glycerol from 0 to 70% (v/v) resulted in a pronounced inhibitory effect on the protein synthesizing activity, while thymidine incorporation into DNA and the alpha-aminoisobutyric acid uptake through the cell membranes were less affected. Exposure of the tissue to buffer containing dimethylsulfoxide (Me2SO) in concentrations of 10 to 20% (v/v) resulted in a stimulatory effect on metabolism. At higher concentrations, Me2SO was toxic resulting in damaging effects on protein and DNA synthesis as well as on membrane integrity. The stimulatory effects of exposure to low concentrations of Me2SO on protein and DNA synthesis in kidney slices were concluded to be the result of an increased transport of precursors through the cell membranes.

Case report: fourteen-year follow-up of an apudoma of the bile ducts at the hilum of the liver.

A patient presented with a sclerosing tumor at the bifurcation of the main hepatic ducts and was followed for 14 yr after the initial symptoms. Palliative surgery was performed in 1968, followed by a remission of 10 yr. A reintervention in 1979 showed an impressive fibrotic mass that could be extirpated. The patient is again in complete remission for almost 3 yr. A tumor biopsy specimen, obtained at the second intervention, showed an apudoma-type tumor. This was confirmed by a positive Grimelius stain and by immunohistochemical investigation.

Hemorrhagic pleural effusion and mediastinal mass: presenting symptoms in a child with pseudocyst of the pancreas.

A girl was referred to our hospital because of a right-sided recurrent hemorrhagic pleural effusion associated with a mediastinal mass. It was later found that the findings were due to a pancreatic pseudocyst with internal fistulization through a necrotic opening in the left hemi-diaphragm, and with mediastinal and right-sided pleural extension. The object of this report is to remind clinicians that a pleural effusion and a mediastinal mass may be the presentation of a pseudocyst of the pancreas, and to point out the important of endoscopic retrograde cholangiopancreatography--even for children.

Pre-operative and post-operative evaluation of the rectal bladder.

Five Heitz-Boyer-Hovelacque operations in children were reviewed in which particular attention was paid to pre-operative examinations, including electromanometry (EMM) and post-operative evaluation of continence, functional result and renal function. In our limited experience the rectal bladder operation is a good procedure in selected cases; it produces satisfactory continence and maintains renal function.

Gallbladder wall sonolucency in acute cholecystitis.

The usual ultrasound findings in acute cholecystitis include diffuse hyperreflective wall thickening, hazy wall delineation and gallbladder distension. In 11 patients with acute cholecystitis, an additional sign was seen consisting of a hyporeflective or sonolucent layer, continuous or interrupted, within the hyperreflective, thickened gallbladder wall. Anatomic-pathologic correlation, comparative CT scans and clinical follow-up studies showed subserosal edema and necrosis to be the most probable cause of this finding.

Gallbladder wall thickening: a new sign of gallbladder disease visualized by gray scale cholecystosonography.

The use of gray scale equipment in a prospective study of radiologically non-visualizing gallbladders, permitted detection of a new echographic aspect of gallbladder disease. Along with even, discrete bile thickening and the presence of sand-sized calculi, thickening of the gallbladder wall incholecystitis is another ultrasonographic sign of gallbladder disease that can be visualized successfully by gray scale ultrasound.
