Psychiatric symptoms in disliked medical patients.

Twenty-two patients seen in a clinic for systemic lupus erythematosus were tested for organicity, depression, anxiety, and hostility. Four of the clinic's physicians ranked these patients from most liked to least liked. In three of the four physicians, dislike was significantly correlated with the patient's degree of organicity. Ten of the patients were ranked among the three most disliked patients by one or more of the ranking physicians. This group of most disliked patients contained all patients with signs of organic brain damage and all suicidal patients. Dislike of a patient by the physician may be a clue to serious psychiatric impairment.

Psychotherapeutic strategies in the treatment of psychophysiologic disorders.

The outlined treatment strategies are based on research findings. The techniques include repeated physical examinations to reassure patients with hypochondriacal concern and continuous empathy with the partient's distress; explanatory therapy which includes emphasis on the innocuousness of the phenomena, accurate information about the psychophysiologic processes involved, making the patient aware of his selective perception; deliberate suggestion when this is in keeping with accurate predictions and appropriate treatment of coexisting psychiatric disorders.

Changes in self-rating of symptoms: a comparison of questionnaire graphic scales with test cards.

Forty-five in-patients, with primary diagnoses of neurosis or personality disorder, completed the test cards and booklet versions of the Symptom Rating Test--Day (SRT). In order to facilitate retroactive interference the Manifest Anxiety Scale (MAS) and the Maudsley Personality Inventory (MPI) were administered between the two forms of the SRT. On the next day the patients were given the SRT (Week). The initial SRT, MAS and MPI testing was repeated one week later. On the assumption that positional set is an important consideration predictions were made as to the expected differences between the test cards and booklet modes of SRT administration. The results add support to the practical use of the SRT in its more recent standardized format.

The effects of one-day treatment of anxiety with high doses of halazepam.

The main purposes of this study were to examine the effects of one-day treatment of anxiety and to test whether significant relief of anxiety can be achieved in a short time period by frequency individual adjustment of doses. A double-blind crossover study of halazepam and placebo was carried out with 22 patients, 20 completing the study. Each treatment lasted for one day. Oral medication was administered starting with a dose of 40 mg and the dose adjusted every 2 hours. The total daily dose did not exceed 600 mg. Several rating and self-rating scales were used. All observer rating scales and several self-rating scales discriminated at a significant level. The design appeart to be suitable for the rapid screening for anti-anxiety properties of new drugs.

[Water and mineral balance in refeeding after total fasting].

The water balance and the electrolyte balance were measured in 6 patients after 14 days of total starvation. After fasting a 600 calorie formula diet was given for the whole period. The potassium balance was +32 mval/day and that of sodium +120 mval/day. The total loss of potassium in 14 days was 488 mval and that of sodium only 126 mval. Sodium loss is replaced within one day of refeeding. No replacement of the potassium loss was noticed during the 4 day treatment.

[Potassium substitution in fasting].

Twenty patients, divided in two groups to ten were undergoing a fasting period for 14 days. One group received 80 mEq potassium per day. Despite of identical loss of body-weight, the application of potassium showed the following advantages: 1) The body is able to keep acid-base-balance. 2) The important loss of potassium, induced by starvation can be reduced to a minimum.

[Maltose as a substrate for inducing osmotic diuresis (author's transl)].

10 healthy men received an i.v. infusion of maltose at a dosage of 1.5 g/kg/h over a period of 1 h. No clinical side effects were observed. Total protein and albumin showed a significant drop. The glucose level remained constant. The concentration of maltose rose up to 495 +/- 77 mg/100 ml. The osmo-diuretic action of the solution was proven by a renal excretion of 289 +/- 57 ml water/h, 14.6 g maltose/h and 3.9 g glucose/h. The total carbohydrate elimination was 61.8 +/- 9.1 g/8 h, (44.5 +/- 13.5 maltose and 15.7 [+8.1--21,2] glucose). The specific action of maltose is an induction of glucosuria without hyperglycemia.
