Fifty-one newly hospitalized depressed patients participated in a double-blind comparison of two dosage levels of imipramine hydrochloride (150 mg vs 300 mg daily). Although some patients were suffering from neurotic depressions, they, together with the endogenous depressives, were a severely depressed group who required hospitalization. Improvement occurred with both dosage regimens, although a greater and more consistent improvement was noted in the 300-mg group than in the 150-mg group. There were a few differences between the response of the endogenous and that of the neurotic depressives, as assessed by the physician and self=rating scales. However, endogenous depressives who received 150 mg were overrepresented in the treatment failure group. A comparison of the response of deluded and nondeluded depressives indicated that the deluded patients responded less well than the nondeluded depressives, although half of the delusional group did respond to the treatment.
Thirty-two anxious nonpsychotic inpatients participated in a four-day, double-blind repeat crossover study of a new drug, GPA 2640, and placebo. Each treatment period lasted for one day, and the study lasted for four days. Twenty patients completed the study, and 28 completed two or three days. The doses were individually adjusted and mostly ranged between 1100 and 1300 mg daily. Several valid observer-rating and self-rating scales were used. There was a slight trend for GPA 2640 to yield somewhat lower anxiety scores on drug days than on placebo days, but none of the differences between rating and self-rating scores reached a significant level. A few patients reported a marked improvement in mood after two to four days, but the significance of this finding cannot be evaluated from this design. Two patients developed temporary paranoid delusions which may have been drug related. Two patients had raised SGOT levels after the study. There were no significant differences in vital signs between drug days and placebo days. The results signify that the drug has no immediate antianxiety effects (such as have been demonstrated with benzodiazepines in two previous studies utilizing the same design). This does not preclude the possibility of delayed anxiolytic effects or of other delayed psychotropic effects such as can be observed with antipsychotic drugs or antidepressants.
In 30 obese subjects three different methods for weight, reduction were applicated over a period of 14 days. One group (n=10) was treated with total starvation, the other group (n=10) with total starvation and 80 mval potassium in addition and the third group (n=10) with a 700 cal. diet. In total starvation the balance of sodium, potassium and phosphate amounted to -9 mval/d, -34,9/d and -8,8 mval/d respectively. Whereas calcium showed a positive balance of 4,4 mval/d. During addition of 80 mval potassium sodium excretion increased whereas potassium excretion was diminished resulting in a potassium balance less negative. During treatment with the 700 calorie diet the highest negative sodium and a high positive potassium balance were observed,
Thiothixene was used in a four-week double-blind placebo-controlled study of 42 geriatric patients with chronic organic brain syndrome (psychotic or nonpsychotic). The results, according to several rating measures, showed no significant difference between placebo and thiothixene. Side effects were mild and few. These data support the safety of thiothixene therapy for geriatric patients; however, there is no conclusive evidence of its efficacy in the treatment of chronic organic brain syndrome.
Case report on course and radiological symptomatology of malignant thymoma. The unusual course of the young patient and the extensive haematogenic and lymphogenic metastases are stressed. The current opinion on a low tendency of malignant thymoma to metastasize is discussed. Therapeutic regimen and radiological criteria of thymoma are described.

Published controlled studies of various psychotherapeutic techniques in psychosomatic disorders with adequate designs have formed the basis for the following conclusions: Psychotherapeutic techniques are effective in some patients with psychosomatic disorders. Some psychosomatic disorders, for example, bronchial asthma, peptic ulcer, and migraine headaches are perhaps more amenable to psychotherapy than others, for example, hypertension and ulcerative colitis. There is evidence to suggest that there are differences between the effectiveness of various psychotherapeutic techniques; a few patients differ perhaps from the rest in that they are helped most by the technique that is less effective for the majority.
Six anxious schizophrenic patients who were maintained with phenothiazines participated in a double-blind intensive design study of chlordiazepoxide and placebo for 12 weeks or longer. There were substantial differences between patients in their responses to chlordiazepoxide: two patients experienced significant and conspicuous relief of distress and reduction of typical schizophrenic symptoms. In another patient the differences, although statistically significant, were clinically less striking. In the three remaining patients no differences were observed between responses to the two treatments except that one patient was more depressed with chlordiazepoxide than with placebo. The findings suggest that there are at least two kinds of anxiety in schizophrenia. A few anxious schizophrenic patients apparently benefit more from a combination of a phenothiazine with chlordiazepoxide than from a phenothiazine alone.
In 20 patients, undergoing a fasting period of 14 days, electrolyt balances were studied. Ten patients received 80 mval potassium per day. Sodium, potassium, calcium and phosphate in blood did not change; bicarbonate and chloride levels diminished. Balance studies showed a significant loss of potassium in both groups. Product of sodium and potassium was constant; calcium balance was positive, whereas that of phosphate was negative. During addition of 80 mval potassium per day significant change of sodium balance occurred.

Psychiatrists have recently been criticized from within and outside the profession for rendering professional opinion in instances where their qualifications are inadequate. In forensic psychiatry, the problem of predicting dangerousness is a notable example. The forensic psychiatrist may also be called to testify as to the present or future competency of the aphasic patient. It therefore behooves the medical expert to be fully aware of the new developments within the field of aphasiology so that in cases involving aphasia the best combined expert testimony can be rendered the court.
