Sustained-release theophylline in chronic bronchitis.

Twenty-one patients with chronic bronchitis entered a double-blind, cross-over study in which they received sustained-release theophylline ('Nuelin'SA) 350 mg daily for 4 days followed by 700 mg daily for 4 days and matching placebo tablets for 8 days with one week separation. Seventeen patients completed the study. Three patients receiving higher doses for weight than the mean for the group were withdrawn because of side-effects. Mild side-effects only were reported in six other patients. Theophylline given twice daily produced a steady-state mean serum concentration of 13.9 micrograms/ml, 13 patients having concentrations inthe range of 10-20 micrograms/ml. There was no demonstrable improvement of symptoms but pulmonary function measurements in the clinic at the end of active treatment showed a statistically significant improvement in PEFR, 1-second forced expiratory volume and forced vital capacity.

Labelling of membrane glycoproteins on rat spermatozoa collected from different regions of the epididymis.

The major glycoprotein on the plasma membrane of testicular spermatozoa labelled with the galactose oxidase/NaB3H4 technique has mol.wt. 110 000. As spermatozoa pass through the epididymis, labelling of this glycoprotein disappears and is replaced by labelling of a 32 000-mol.wt. protein. The latter protein is a major component of epididymal secretions. The evidence suggests that it is inserted into or absorbed on to the plasma membrane, and since its appearance on spermatozoa correlates with the acquisition of fertilizing capacity it should serve as a good marker for assessing maturation in vitro.

Characterization of hormonally regulated secretory proteins from the caput epididymidis of the rabbit.

1. The incorporation of [35S]methionine into protein was investigated in tissue minces from different regions of the rabbit epididymis incubated in vitro. Rates of synthesis were in the order: epididymal regions 2-5 greater than region 7 greater than region 6 greater than region 1 greater than region 8 greater than ductus deferens greater than ductuli efferentes. 2. Separation of labelled proteins on polyacrylamide gels containing sodium dodecyl sulphate followed by fluorography revealed that one protein (mol.wt. approx. 90 000) was characteristic of region 1, four proteins (one of mol.wt. 54 000 and three of mol.wt. 20 000) were synthesized principally in regions 2-5, and one protein (mol.wt. 22 500) was produced mainly in regions 6, 7 and 8. 3. Castration for 14 days decreased incorporation of [35S]methionine into total protein to less than 10% of that in controls in all regions of the epididymis. However, testosterone treatment for a further period of 14 days restored protein synthesis to normal values in regions 6, 7 and 8, but not in region 1 or regions 2-5. In regions 2-5 the synthesis of three proteins of mol.wt. 20 000 declined after castration, but was not stimulated by exogenous testosterone. Since the 20 000-mol.wt. proteins were major tissue proteins, accounting for 16-25% of the total synthesized, they were used as markers for investigating hormone action in the epididymis. 4. Castration followed immediately by testosterone treatment or ligation of the ductuli efferentes resulted in a decrease in their synthesis, suggesting that they are partially dependent on factors in testicular fluid. Purification and characterization showed them to be acidic glycoproteins with a number of biochemical and immunological properties in common. 5. It is suggested that there is a synergistic action between blood androgens and factors in testicular fluid in regulating protein synthesis in the proximal regions of the rabbit epididymis.

Single 600-milligram oral dose of doxycycline in the treatment of gonorrhoea.

One-hundred and fifty patients suffering from uncomplicated gonococcal infections were treated with a single oral dose of 600 mg doxycycline. Of the 117 patients who returned for follow-up, 14 were classified as dropouts; 92 of the remaining 103 patients were judged to be cured by the results of clinical examination and negative culture.

Subglottic stenosis in newborn intensive care unit graduates.

Five infants with postintubation subglottic stenosis were identified among 64 intubated patients in a follow-up clinic. The presence of laryngeal lesions was not suspected until three weeks to three months after discharge from their initial hospitalization. The onset of stridor, often precipitated by respiratory illness, led to the diagnosis, which in one case was delayed for as long as three months. No correlation was found between the presence of the severity of laryngeal pathology and the gestational age of the infants or the duration of intubation in this study. In spite of the precautions taken for the intubation of neonates requiring assisted ventilation, subglottic stenosis may develop in these infants and should always be considered in newborn intensive care unit graduates when they manifest refractory or recurrent respiratory obstruction in the first year of life. Routine follow-up of all intubated neonates to detect subglottic stenosis is recommended.

The influence of electrical activity on the development of newborn innervated rat muscles.

Mammalian slow and fast muscles are incompletely differentiated at birth, and their specific characteristics emerge gradually during the early postnatal period. The pattern of activity received by young muscles may be instrumental in bringing about their differentiation. Hind limb muscles of rats were stimulated via the sciatic nerve in order to test the influence of different patterns of activity on their development. Slow frequency stimulation (10 Hz) prevented the development of fast contraction times in "fast" muscles. The slow soleus became slightly faster when stimulated a 25 Hz. Stimulation of muscles of rats between 4 and 12 days at 40 Hz apparently resulted in some denervation in both fast and slow muscles, although stimulation at this frequency in older rats resulted in shortening of slow muscle contractions.
