Clinical and hormonal studies in precocious puberty.

Data from 31 children, 23 females (74.2%) and eight males (25.8%) with precocious puberty were analysed. They included 19 cases with central precocious puberty, 5 with premature thelarche, 5 with premature adrenarche and 1 each with adrenal adenoma and virilizing hepatoblastoma. Their clinical and hormonal profile and the treatment outcome are briefly discussed.

Augmentation of prolactin response to TRH after cyproheptadine.

Serum PRL, TSH, T3 and T4 were estimated in the basal state and following iv bolus injection of 100 micrograms synthetic thyrotropin releasing hormone before and 7 days after cyproheptadine treatment in 5 healthy adult males. Only the PRL response to TRH was augmented after cyproheptadine. This is perhaps related to the antidopaminergic effect of cyproheptadine.

Hypophosphatemic rickets and osteomalacia in polyostotic fibrous dysplasia.

A 17 year-old girl with polyostotic fibrous dysplasia and hypophosphatemia had inappropriately low tubular reabsorption of phosphate. She had radiological evidence of rickets and osteomalacia. The patient showed clinical improvement after treatment with phosphate supplementation, active vitamin D (calcitriol) and alendronate. It is postulated that either a phosphaturic substance elaborated from the dysplastic bone or target-organ (kidney) unresponsiveness may interfere with phosphate reabsorption in the renal tubule.

Auditory investigations in hypothyroidism.

Twenty patients with thyroprivic hypothyroidism with a mean age of 34.8 years (range: 15-50 years) were investigated. Peripheral/central functions of the auditory system were assessed in the hypothyroid state and they were re-evaluated in the euthyroid state after treatment with levothyroxine. The mean observation period for re-evaluation was 3.7 months (range 3-7 months). Sixteen patients with hypothyroidism (80%) demonstrated hearing loss when compared with randomly selected age- and sex-matched normal subjects. Twelve of the hypothyroid patients had sensorineural hearing loss and 4 had mixed loss. Special hearing tests revealed a cochlear type of hearing loss. The tympanogram curve was of Type B in one ear of each of the 4 patients. Brainstem electric response audiometry showed prolonged, absolute latency of wave V and interpeak latencies I-III and I-V. The amplitudes of waves I, II and V were reduced. Following treatment with levothyroxine a statistically significant improvement in hearing thresholds was observed by pure-tone audiometry. Tympanogram curves returned to normal in 3 out of 4 patients. Brainstem electric response parameters did not show significant reversibility to normalcy following treatment. The results of auditory investigations suggest a causal relationship between hypothyroidism and hearing loss. The site of lesion in the auditory system is probably at several levels, viz. in the middle ear, and at cochlear and retrocochlear sites.

Parathyroid carcinoma: difficult management options.

Parathyroid carcinoma is a rare cause of primary hyperparathyroidism and these tumours are usually hyperfunctional as opposed to other malignant endocrine tumors. Surgery is the only effective treatment while nonsurgical modalities yield poor results. We report a patient, who presented with palpable mass in the neck and severe hypercalcemia. He underwent debulking surgery and received allendronate, calcitonin, dacarbazine followed by in- situ alcohol instillation with some success.

Circulating testosterone in pure motor stroke.

Serum LH, FSH and testosterone were quantitated in 9 patients with pure motor stroke within 24-48 h of its reported onset. High circulating LH with normal or low testosterone was noted in 8 of them. In response to an intravenous bolus of GnRH, the LH responses were exaggerated in all, but the FSH responses in 7 of them were comparable to those in eugonadal age matched controls. The rise in testosterone following 2000U hCG daily for 3 consecutive days was insignificant in the patients group compared to the controls. The data suggest normally operative pituitary testicular feed-back but decreased Leydig cell response in pure motor stroke.
