Prognostic significance of chromosome analysis in de novo acute myeloid leukemia (AML).

Between 1981 and 1986 cytogenetic studies of bone marrow and/or blood cells in 139 patients with de novo acute myeloid leukemia (AML) were performed. The overall incidence of chromosomal aberrations was 53%, and this was not significantly influenced by sex, age nor the FAB classification. The aberrations most often found were: complex anomalies (n = 13), t(8; 21) (n = 10), trisomy 8 (n = 9), monosomy 7 (n = 6), monosomy 5, 5q-, trisomy 11, 12p- (n = 4) and trisomy 6, 11q-, inv (n = 3). The prognostic significance of chromosomal findings was evaluated in 112 patients treated by combination chemotherapy. The chromosomal status NN, AN, AA did neither significantly influence complete remission rate (NN: 68%, AN: 71%, AA: 60%) nor mean survival (NN: 24, AN: 26.6, AA: 35.6 months). On the other hand, certain types of chromosomal anomalies were of prognostic value. CR was obtained in all 10 patients with t(8; 21) but only in 2 out of 9 patients with complex aberrations. Median duration of CR in patients with t(8; 21) was significantly longer than in patients with a normal karyotype (30 vs 7 months).

[Haptoglobin determination in the serum of patients following intraoperative autotransfusion using the Haemonetics Cell Saver III. Studies on the loading of patients with free hemoglobin in retransfused erythrocyte concentrate].

In addition to hemodilution, mechanical intraoperative autotransfusion (IAT) is the most important method of preventing or minimizing the transfusion of homologous blood in operations with major blood loss. Most of the problems associated with IAT could be solved by the use of cell separators, but the separated red blood cells still contain an average of 200 mg/dl free hemoglobin. By the determination of haptoglobin levels before and after IAT, we studied the effects of free hemoglobin on the patient. Seventy-seven patients with hip-joint replacement were studied. In one group, n = 47, both intraoperative blood loss and drainage blood (for 6 h post-operatively) were collected and transfused back to the patient after cell separation with the Haemonetics Cell-Saver III as a red-cell concentrate. A second group, n = 34, received only homologous blood. Serum haptoglobin was determined after anesthesia induction and after the last transfusion on the day of operation. There were no significant differences in preoperative haptoglobin levels between both groups. In the IAT group, haptoglobin was significantly lower then in the control group after transfusion (t-test, P = 0.05). In both groups 14% of the patients' haptoglobin levels were pathologic preoperatively. Post-transfusion 60% of the IAT group showed minimum levels while in another 14% no haptoglobin could be measured. In these 14%, free hemoglobin was circulating in the patients' blood because the transport capacity was exhausted. In the control group only 26.5% of the haptoglobin levels were below normal and in no case was transport capacity exhausted (Table 3). The correlation between volume of retransfused autologous blood and decrease in haptoglobin level was small (r = 0.15). In a few cases with low volumes of retransfused blood the haptoglobin decrease may have been greater, so that free hemoglobin may have been present.

[Tympanoplasty with incus interposition].

We report on a 5-year experience with 44 patients (1980-1985) with incus interposition using a modelled or sculptured incus, either autograft or homograft, to correct ossicular discontinuity when a functional malleus and stapes are present. After a period of 21 months, 75% of our patients achieved an air-bone gap closure between 1 and 10 dB. The postoperative hearing results of patients with trauma and middle ear atelectases were better than the hearing results of patients with cholesteatomas. These long-term results are dependent upon intact malleus attic ligaments and an intact tensor tympani tendon. It is very important that the modelled incus is repositioned under the malleus handle neck.

Computer quantification of delta activity in sleep EEG.

The current project assessed the accuracy of a period-analytic computer program in the quantification of delta EEG activity during sleep for the purpose of the assignment of sleep stage. Ninety-four 25-sec epochs of data from two subjects were analyzed both using the computer and by two visual scorers in order to generate percent measures of the amount of delta in the EEG. This information was then used to assign stage score. A comparison of the inter-scorer and scorer/computer agreements showed good correspondence between them, with the human/computer contrasts being equivalent to the human/human contrasts. Data are presented for both the percent of delta in the records and for the agreement of stage score assignments. It was concluded that the computer program provided an acceptable alternative to human visual scoring. Suggestions as to how to implement digital computers in sleep research were presented.

[Primary reconstruction of injuries of the capsular ligament of the knee joint].

Whether operative or conservative treatment is indicated for acute knee ligament injuries depends on the lesions of the cruciate ligaments: complex instability with rupture of one or both cruciate ligaments and injuries to the lateral or medial ligamentous structures should be treated by operation. Surgical treatment of an isolated rupture of the anterior cruciate ligament is recommended only for the young active patient. Surgery is performed by way of a single anterolateral incision with standard medial and if necessary, lateral arthrotomies. Ruptures of the cruciate ligaments are reconstructed with absorbable sutures, which are passed through bone channels. Augmentation with an absorbable allograft is used in most reconstructions of the cruciate ligaments. A knee brace with limited range of motion is used for postoperative rehabilitation. Our long-term results after operative reconstruction of acute instabilities of the knee joint show that ligamentous stability was achieved in most cases, but the functional results were impaired by pain and limited range of motion.

[Primary biliary liver cirrhosis and granulomatous pulmonary infiltrates].

Primary biliary cirrhosis was diagnosed in a 48-year-old woman in the light of clinical and laboratory data, antimitochondrial antibodies and histologic features in liver biopsy. Four years later the patient developed pulmonary infiltration of changing localization. Histologic investigation of transbronchial biopsy revealed granulomas in the bronchial mucosa and peribronchial tissue. There were no further signs of sarcoidosis. The question is discussed whether granulomatous pulmonary infiltrations are an extrahepatic manifestation of primary biliary cirrhosis.

Dream self-reflectiveness as a learned cognitive skill.

This research was directed toward the contradiction sustained by cognitive dream psychology, which on the one hand regards dreaming as higher symbolic activity and, on the other, sees its organizational and functional characteristics as derivative and/or inferior to those of waking consciousness. Study 1 evaluates the degree of self-reflective meta-cognition in dreams from different sleep stages. Subjects were 24 college students selected such that half were self-reported high-frequency dream recallers and half were low-frequency recallers. Both groups were composed equally of men and women. Greater self-reflectiveness (SR) was found in REM dreams as compared with those from stages 2 and 4, which did not differ. High-frequency recallers showed more dream SR than did low-frequency recallers. Study 2 assessed the extent to which self-reflective and lucid dreaming can be learned as a cognitive skill by varying levels of intention and attention paid to dreaming. After 3 weeks of home dream collection, results showed that four experimental groups had greater dream SR than did a baseline group. The most effective treatment was the mnemonic, wherein attention patterning schemas learned in waking resulted in more self-reflective and lucid dreaming than did either baseline or attention-control conditions. These results provide evidence that dreaming is not single-minded but variable along a self-reflective process continuum, and suggest functional and organizational levels that are consistent with the conception of dreaming as higher order cognitive activity.
