Management of the oral and maxillofacial surgery patient on anabolic steroids.

Recent Food and Drug Administration estimates indicate that the anabolic steroid market has reached the $100 million level. Although the news media report steroid-related deaths, there is little in the medical literature communicating the clinical implications concerning these drugs. Electrolyte disturbances, coagulopathies, hypertension, and dysrhythmias are a few of the potential side effects. The pharmacology and pathophysiology of anabolic steroids are presented with a discussion of the anesthetic and surgical concerns for the patient undergoing oral and maxillofacial surgery.

Measurement standards for the prevertebral region in the lateral soft-tissue radiograph of the neck.

One hundred thirty-one lateral soft-tissue radiographs of the neck, taken over a 6-month period, were reviewed. Eighty-six radiographs were normal. The mean prevertebral soft-tissue thickness in the posterior-anterior dimension for each normal radiograph was measured. The mean thickness in the retropharyngeal region ranged from 6.2 mm in the infant to 3.7 mm in the adult, while the mean thickness in the retrotracheal region ranged from 9.2 mm in the preschool group to 12.1 mm in the adult.

Cervical spine fractures and maxillofacial trauma.

Cervical spine fractures are a serious complication of maxillofacial trauma because of the high potential for mortality and neurologic morbidity. This study reviewed 563 patients with facial fractures treated by the Division of Oral and Maxillofacial Surgery at a level I trauma center and identified 11 concomitant cervical spine fractures (2.0%). These patients were almost exclusively male (91%), white (100%), between 20 and 35 years of age (64%), involved in a motor vehicle accident (91%), with a concomitant mandibular fracture (91%). The examination, diagnosis, and management of patients with cervical spine injuries are discussed.

Diagnosis and treatment of the retropharyngeal abscess in adults.

While the retropharyngeal space abscess is an extremely rare entity that may arise from odontogenic infections, its potential complications may be fatal. When infection of the retropharyngeal space occurs, urgent surgical and antibiotic therapy is required. A review of the anatomy, symptoms, treatment, diagnostic methods and complications is provided. As well, the successful treatment of a multispace odontogenic infection is discussed as a reminder that the retropharyngeal abscess does exist.

An epidemiologic survey of facial fractures and concomitant injuries.

A 5-year review of facial fractures and concomitant injuries at a level 1 trauma center is presented. The anatomic location of the facial fractures, age and sex of the patients, cause of injury, and associated systems injuries are presented. The majority of facial fractures were found in males; the most prevalent age range was 16 to 30 years. Mandible fractures outranked zygomatic and maxillary fractures (6:2:1). Assaults and motor vehicle accidents were the most frequent causes of facial fractures, and lacerations followed by neurologic and orthopedic injury were the most frequently encountered concomitant injuries. Motor vehicle accidents were the most frequent cause of associated injury.

Reviewing the unicystic ameloblastoma: report of two cases.

Classification of ameloblastoma into solid, multicystic, unicystic, and peripheral types based on clinical appearance and effects has gained recent recognition. The unicystic ameloblastoma is a less encountered variant of the ameloblastoma. It appears more frequently in the second or third decade with no sexual or racial predilection. It is almost exclusively encountered asymptomatically in the posterior mandible. The following report of two cases discusses the pathophysiology, demographics, histology, and prognosis of the unicystic ameloblastoma.

Xeroradiography in the diagnosis of nonradiopaque sialoliths.

Sialolithiasis may be the most common disease of the salivary glands in patients more than 20 years of age. Approximately 19% of sialoliths occur in the parotid gland. Of these, only up to 71% can be successfully diagnosed radiographically. The remaining 29% provide a diagnostic dilemma. Xeroradiography, because of its tissue-edge enhancement, can distinguish tissue margins of different density more dramatically than can conventional imaging and is an invaluable aid in the diagnosis of nonradiopaque sialoliths.

The infected prosthetic total temporomandibular joint replacement: report of two cases.

Two cases of infected total temporomandibular joint replacements are reported. The WBC count, erythrocyte sedimentation rate, and temperature were within normal limits, but erythema, edema, and pain provided the initial clue to the infection. Ultrasonography ultimately revealed a fluid collection and allowed aspiration of the fluid-filled cavity, providing a diagnosis and directing antibiotic therapy.

Serotyping and bacteriophage typing of group B streptococci.

Three hundred and eighty-two human streptococcal strains of serogroup B, collected from different sources and from different parts of Norway have been serotyped and phagetyped . The results of serotyping show a predominance of serotype Ia; 150 strains belonged to this type. Only 32 strains belonged to serotype III, a serotype well-known as the dominating cause of neonatal septicemia and meningitis (11, 23). The other serotypes were evenly represented. The phagetyping was performed by the procedure and with phages described by Stringer (19). The strains were lysed by phages at a frequency of 70%. A total of 145 strains (38%) was lysed by one or two phages, while 124 strains (32%) were typable by different phage patterns. When combined with serotyping, phagetyping of group B streptococci is an important tool in characterizing these microbes for epidemiological and other purposes.
