Cryptorchidism and testicular neoplasia.

A retrospective review of 20 cases of testicular neoplasia in cryptorchid patients is presented. These cases serve to emphasize the need for more effective diagnosis and management of the undescended testis. The various clinical presentations, diagnostic modalities, prognostic indicators, and types of management are reviewed. Due to its inaccessibility and array of clinical presentations, the intraabdominal testies tumor continues to be a diagnostic challenge. Strict attention to diagnostic cues and adherence to the management practices presented here would hopefully result in an overall decrease in testis tumors in the post-pubertal male.

Angiosarcoma with pulmonary siderosis and persistent reticulocytosis. Steroid responsiveness suggests an immune basis.

A 35-year old man with cough, hemoptysis, and dyspnea was found to have diffuse pulmonary infiltrates and iron-laden macrophages in the sputum. Pulmonary siderosis was confirmed by transbronchial biopsy. An associated hypochromic anemia required frequent transfusion. Though marrow iron stores were absent, reticulocytosis was maintained. Corticosteroid therapy resulted in cessation of hemoptysis, clearing of the pulmonary infiltration, and a substantial reduction in transfusion requirement. Splenectomy was of no benefit. The patient developed cerebral symptoms with seizures, and rapid deterioration led to cerebral symptoms with seizures, and rapid deterioration led to death. Disseminated hemangiosarcoma was found at autopsy. Steroid responsiveness of the associated pulmonary siderosis suggests that it had an immune basis.

Automated high-speed analysis of gastrointestinal myoelectric activity.

The value of myoelectric recording in the gastrointestinal tract has been limited by the difficulty of analyzing the prolonged recordings which are required to demonstrate integrated physiological activity. This problem has been solved by the use of a tape recording system with accelerated replay, originally developed for cardiac monitoring. Rapid replay not only reduces the time required for data retrieval but also increases the frequency of the recorded signals to allow electronic separation of fast and slow wave activity for on-line conversion into digital pulses suitable for computer analysis. The technique is illustrated by the analysis of an imperfect prolonged multichannel recording of cannine gastrointestinal myoelectric activity, demonstrating some of the possibilities for data analysis and error detection inherent in the system.

Spenic artery aneurysm presenting as left renal mass.

A case of splenic artery aneurysm presenting as a left upper quadrant mass represents a diagnostic challenge due to its paucity of clinical signs and symptoms. The condition must be distinguished from urologic pathology. Celiac angiography is necessary for diagnostic confirmation. Surgical resection is advised for most patients.

Anemia in urban underprivileged children. Iron, folate, and vitamin B12 nutrition.

The prevalence and type of nutritional anemia was investigated in 344 children aged 1 to 16 years of mixed race and living in a poor urban setting. Iron deficiency anemia was common in 1-year-old children (23%) as was biochemical evidence of iron deficiency (53%). Anemia rates were minimal in older children and the prevalence of iron deficiency decreased with age. Folate deficiency did not appear to contribute to the etiology of anemia, and nutritional vitamin B12 deficiency was not present. No-relationship could be found between a number of familial variables and hematological nutritional status. It is suggested that to identify families whose children are at risk for nutritional anemia new approaches will be needed to define their characteristics.

Relationship between age and tricyclic antidepressant plasma levels.

Older depressed patients treated with imipramine or amitriptyline developed higher steady-state plasma levels of imipramine, desipramine, and amitriptyline. In imipramine-treated patients this finding was associated with a decreased rate of drug elimination from plasma. These findings provide at least a partial explanation for the increased susceptibility of the older patient to tricyclic antidepressant side effects and also provide a pharmacological rationale for use of lower dosages in this age group.

Epidermal mast cells.

Tissue from 134 patients with neurodermatitis and prurigo, pseudoepitheliomatous hyperplasia, pemphigus, and urticaria pigmentosa was examined qualitatively for epidermal mast cells. Epidermal mast cells were found in all of the diseases that were studied except dermatitis herpetiformis. Pemphigus vegetans and dermatitis vegetans were frequently associated with the presence of epidermal mast cells. In other pseudoepitheliomatous diseases, such as tuberculosis verrucosa cutis, blastomycosis, and bromoderma, epidermal mast cells were present in many cases. Four of eight patients with acral dermatitis with elevated IgE blood levels had intraepidermal mast cells; the number was much lower in patients with usual neurodermatitis and prurigo nodularis. Only two of ten cases of alopecia mucinosa showed epidermal mast cells. A single epidermal mast cell was found in ten cases of urticaria pigmentosa. Chronic inflammation associated with epidermal cell proliferation appeared to correlate with the presence of epidermal mast cell.

Free erythrocyte porphyrin: hemoglobin ratios, serum ferritin, and transferrin saturation levels during treatment of infants with iron-deficiency anemia.

Free erythrocyte porphyrin:hemoglobin (FEP:Hb) ratios were determined on 20 infants with iron-deficiency anemia. FEB:Hb ratios were compared with simultaneously drawn serum ferritin and serum transferrin saturation levels. FEP:Hb ratios dropped steadily during treatment of the anemic infants, but remained elevated compared to age-matched nonanemic infants, until the anemia was corrected. FEP:Hb ratios detected iron deficiency when acute inflammatory disease was present. Serum ferritin levels and transferrin saturation failed to detect iron deficiency after iron therapy was started or when acute inflammatory disease was present. Measurement of FEP:Hb ratios is a reliable and practical way to make a prompt diagnosis of iron-deficiency anemia in infants.
