Treatment of stenosis of the iliac artery by balloon catheter dilation.

Percutaneous balloon catheter angioplasty appears to be an effective method of treatment for localized iliac artery stenosis. Results of our experience demonstrate that it can be accomplished with an acceptable morbidity in carefully selected patients and can be done and even repeated with minimal inconvenience to the patient.

Treatment of vein graft stenosis by balloon catheter dilation.

Percutaneous balloon catheter dilation was used to treate 12 high-grade stenoses in ten autogenous vein bypass grafts since September 1978. Four lesions occurred in femoropopliteal grafts, seven in femorotibal grafts, seven in femorotibial grafts, and one in an axillofemoral graft. Balloon catheter dilation was successful and surgery avoided in 11 of the 12 lesions. Pedal pulses were restored in eight of the ten patients, and the pulse in a femorotibial bypass graft was improved in another. Postdilation arteriography demonstrated a substantial increase in luminal diameter, and ankle pressure indexes were improved in all successful cases. The tenth had an operative correction of the stenosis. There were no complications with the procedure. Percutaneous balloon catheter dilation appears to be an effective method of treating stenosis in autogenous vein grafts and a useful alternative to surgical revision.

Absence of ocular pathology after repeated exposure of unanesthetized monkeys to 9.3-GHz microwaves.

Unfettered monkeys (Macaca mulatta) have been trained to expose the face and eyes to pulsed microwave radiation at a frequency of 9.31 GHz and an average power density of 150 mW/cm2. Performance of an operant response required the monkeys to maintain the head within the field of the radiation source. Twelve monkeys were individually irradiated during 30 to 40 sessions and then were observed for a period of one year. No deleterious effects such as cataracts have been observed.

A single-bedtime-dose self-medication system.

A medication system in use at the Florida Mental Health Institute is based on the unit-dose procedure and weekly reviews of patients' medications. The program is aimed at reducing the use of polypharmacy and of concentrates and stabilizing patients on a single daily bedtime dose of medication. Once patients are stabilized on a single dose, they are trained by the nurses and through a drug group to manage their medications independently after discharge. The system has produced savings of up to 90 per cent of medication costs per patient and is gaining acceptance in other mental health facilities in the state. The authors believe it has the potential for even greater therapeutic and financial benefits by reducing the recidivism that occurs when discharged patients discontinue their drugs.

Diagnosis and treatment of pansinusitis: report of case.

A case of acute pansinusitis with the complication of orbital cellulitis has been described. The patient initially had pain around the maxillary right first molar. Accurate diagnosis allowed for prompt, vigorous treatment, culminating in surgical intervention. The need for dental and oral surgical practitioners to be thoroughly familiar with all of the manifestations of paranasal sinus disease cannot be overemphasized.

General pharmacological properties of 4-(p-chlorophenylthio)butanol (W-2719).

4-(p-Chlorophenylthio)butanol (W-2719), a compound that inhibits histamine release from sensitized leukocytes also decreased tracheal smooth muscle tonus, inhibited synthesis and release of immunologically induced SRS-A (slow-reacting substance of anaphylaxis) in the rat and antagonized the in vitro activity of SRS-A. Furthermore, it significantly inhibited the platelet release reaction induced by collagen in vitro but not ex vivo. On the other hand, W-2719 did not possess marked CNS activity nor did it adversely affect the blood pressure, heart rate or respiratory rate. There was no evidence for sympathetic or parasympathetic activity. It had weak antipyretic activity, but exhibited no anti-inflammatory or analgesic activity.

Past history and degree of depression in paraplegic individuals.

The purpose of this study was to attempt to collect sufficient data to substantiate the clinical impression that the degree of depression in the paraplegic and his past history are correlated. It was hypothesised that the poorer the past history, the greater the degree of depression in paraplegic individuals. This study was conducted on ten paraplegic subjects from the Spinal Cord Clinic at Columbia Presbyterian Medical Center, Department of Rehabilitation Medicine, based on data obtained from the administration and scoring of the Depression Scale (D-scale) of the Minnesota Multiphasic Personality Inventory (MMPI) and by the clinical observation by the consulting psychiatrist with the Spinal Injury Clinic. Past history was assumed measurable as 'favourable' to 'poor' by the use of an original questionnaire. The results of this investigation suggested a relationship between a paraplegic individual's past history and the degree of depression. The study was deemed significant in presentation of an original past history questionnaire and in the prediction of future difficulties in the rehabilitation of paraplegics so that preventive measures could be instituted.

Development of Hodgkin's disease in a patient with leprosy.

We present a patient with leprosy who developed Hodgkin's disease of the nodular sclerosing type. There are two previous reports describing the combination of leprosy and Hodgkin's disease in a single patient [3, 9]. Hodgkin's disease was diagnosed 14 months after the complete disappearance of mycobacterium leprae from the skin lesions, under treatment with DDS (diamino-diphenyl-sulfone). Hodgkin's disease was treated by irradiation and chemotherapy. Obstructive jaundice developed which resolved under treatment by irradiation of the hilar area of the liver, chemotherapy and hormones. During two years of immuno-suppressive therapy, without DDS, no exacerbation of the leprosy occurred.

Replacement lipomatosis of the kidney.

Replacement lipomatosis and renal sinus lipomatosis are parts of a spectrum of fatty replacement of destroyed or atrophic renal tissue. When long-standing inflammation exists in a kidney, especially with calculus disease, replacement lipomatosis may be the end result. Awareness of this process along with the specific radiologic findings will allow a correct preoperative diagnosis.
