Heparin-induced thrombocytopenia.

Two patients with underlying thromboembolic disorders developed severe thrombocytopenia while receiving heparin sodium; one of these patients developed recurrence of the thrombocytopenia and possible heparin-induced pulmonary emboli when heparin was restarted. In a prospective study of patients receiving heparin in a coronary care unit (CCU), nine of 37 patients developed transient mild thrombocytopenia (platelet counts ranging from 88,000 to 150,000/cu mm). Heparin added to citrated platelet-rich plasma caused platelet aggregation in the two original patients. In three of six CCU patients tested, and in 17 of 87 other subjects, with maximum aggregation at concentrations of heparin likely to be present in vivo during therapy. We herein discuss evidence that suggests that heparin may cause or aggravate thrombosis by causing platelet aggregation. The occurrence of severe heparin-induced thrombocytopenia is well documented, and mild transient thrombocytopenia may be more common than has been recognized. Studies of heparin efficacy should take these responses into account.

Pineal vasotocin: release into cat cerebrospinal fluid by melanocyte-stimulating hormone release-inhibiting factor.

The intracarotid injection of both synthetic melanocyte-stimulating hormone release-inhibiting factor (MIF) and purified MIF prepared from bovine hypothalami induces arginine vasotocin release into cerebrospinal fluid of cats and significantly decreases the pineal arginine vasotocin content at 5 minutes after the injection. The present results demonstrate an extrapituitary endocrine effect of synthetic and purified bovine MIF.

Middle components of the AER to tone-pips in normal-hearing and hearing-impaired subjects.

Tone-pips of 500, 1000, and 3000 Hz were presented at 0-, 10-, 20-, 35-, and 50-dB HL to 10 normal-hearing subjects and at 0-, 10-, 20-, 35-, and 50-dB SL to 10 subjects with conductive, sensorineural, or mixed hearing losses. Middle component (latencies 8-90 msec) averaged electroencephalic responses to the tone-pips were analyzed in terms of peak latencies and peak-to-peak amplitudes. Properties of the responses were generally the same for both normal-hearing and hearing-impaired subjects except that the hearing-impaired subjects showed slightly greater amplitudes overall. The small reduction in latencies with increasing stimulus frequency seen in the normal-hearing subjects was not observed in the hearing-impaired subjects.

Chromatographic evidence for vasotocin biosynthesis by cultured pineal ependymal cells from rat fetuses.

Cultured pineal ependymal cells from rat fetuses aged 17 to 19 days post-coitum release into their media a substance that has hydroosmotic, antidiuretic and rat uterine activities. Paper chromatography of concentrates from culture media demonstrates that the substance possessing all the above activities was eluted from a slow moving region (Rf 0.15-0.35) with a peak at 0.20-0.30. This region is not significantly different from the Rf of synthetic arginine vasotocin (AVT) used as standard. Neither hydroosmotic nor rat uterine activities could be detected in the chromatographic eluates from the fast moving region corresponding to the Rf of synthetic oxytocin (0.55-0.65) used as standard. The chromatographic mobility of biological activities from culture media, the ratio of the activities as well as their susceptibility to tryptic digestion, demonstrates the presence of a basic peptide indistinguishable from synthetic AVT. The total amount of AVT released into the medium during 43 days of incubation is about 40 times greater than the amount contained in non-incubated pineal glands of the same age, strongly suggesting de novo synthesis of AVT.

Diurnal rhythm of vasotocin in the pineal of the male rat.

The pineal content of arginine vasotocin in the male rat is subject to diurnal changes, having a high value around noon and a low value around midnight. Since in rats exposed to 24 h constant light the pineal content in arginine vasotocin decreases, and in the rats exposed to 24 h constant darkness the pineal content in arginine vasotocin increases, it appears that during the night both the synthesis and release of arginine vasotocin is enhanced.

Pediatric esophageal perforation.

Although esophageal perforation in children is associated with a spectrum of disease different from the one in adults, management is essentially the same for both groups. Over the past 11 years, 12 patients ranging in age from 2 days to 10 years were treated for 13 perforations. Perforation in the adult is associated with a high mortality rate. All children in this series survived. Injury was secondary to instrumentation in 10 cases and three resulted from the chronic erosion of a trapped foreign body. Of the 10 acute perforations, five occurred 12 days to 5 months after lye ingestion. Anastomotic narrowing was a factor in three other cases. Seven of the acute perforations involved the thoracic esophagus and were associated with a high morbidity rate. Management consisted of operative as well as nonoperative approaches. Consideration must be given to several clinical variables including location, cause, predisposing factors, underlying illnesses, associated injury, and promptness of medical attention.

Antibody response to intravenous immunization following splenic tissue autotransplantation in Sprague-Dawley rats.

The response to intravenous challenge with sheep erythrocytes was determined in Sprague-Dawley rats following autotransplantation of splenic tissue into the subcutaneous tissue, peritoneal cavity, or a surgically created omental pouch. There was a marked rise in heterophil antibody titer following intravenous challenge in ten control animals, and no rise in titer in nine of ten asplenic animals. Heterophil antibody titers increased in all of the animals with transplants, but the response in some was less than that in control animals. Thus autotransplanted splenic tissue demonstrated immunologic responses similar to those of normal, intact spleen.
