Two hundred twenty-six accident reports and medical statements concerning circular-saw injuries were evaluated. All injuries occurred on hands. Left and right hands were hurt share and share alike. Among the 125 isolated one-finger injuries an obvious stepwise decrease in frequency from thumb to little finger and from fingertip to finger basis was found. The more proximal the wound lay, the more frequent a localization diagonal or parallel to finger axis was registered. Most accidents hurt skin or soft tissue. Bone lesions occurred less frequent and were characterized by splinters or fractures instead of cuts. Subtotal or total amputations were described in some cases. All lay on fingertips. Nearly the same was seen in two-finger cases. Among three- to five-finger injuries (often diagonal) a few cases of total amputation at the base phalanx were described. The severeness of injuries increases with the number of fingers hurt. A complete cut-like sharp-edged amputation was never seen in the material presented.
The methionine enkephalin (ME) induced responses of prolactin (PRL) and luteinizing hormone (LH) were studied in control and castrated rats bearing chronically-implanted cannulae in the right lateral ventricle. In control rats, 500 microgram ME caused a rise in PRL from a mean +/- SD basal value of 22.8 +/- 12.2 ng/ml to a peak of 195.2 +/- 93.8 ng/ml (p less than 0.001). The peak control occurred 10 min after ME administration and was short-lived. In the same rats, 14 days after castration, there was a significant reduction in both basal (p less than 0.001) and peak PRL response to ME (p less than 0.01), similar to that previously observed with other PRL-inducing stimuli. LH levels, on the other hand, were unaffected by ME treatment in both control and castrated rats.

A very unusual hand malformation is reported in which dorsal skin and nails exist on the volar aspect of the hands. This patient also exhibits mental retardation and microcephaly as well as a partial deletion of chromosome 6. Fifty percent of a small series of patients previously reviewed who presented with chromosome 6 abnormalities demonstrated abnormalities of the hands. Unusual deformities of the hands associated with mental retardation and microcephaly may be related to an abnormality of chromosome 6.
Using a sensitive and precise radioimmunoassay for human beta-endorphin, we have demonstrated the sustained secretion of opioid peptides from human pituitary tumour cells. Pituitary tumour tissue obtained from a patient with Nelson's syndrome was maintained in continuous monolayer culture and secreted both beta-lipotropin and beta-endorphin, with predominance of the latter. This is compatible with the idea that the beta-endorphin in normal human serum is secreted as such despite the predominance of beta-lipotropin compared with beta-endorphin in the anterior pituitary.

We describe a 3-year-old male with deletion of part of 6q. The karyotype was 46,XY,del(6q) in both lymphocytes and skin fibroblasts. The patient had: frontal bossing, epicanthal folds, broad nasal bridge, apparently low-set and posteriorly angulated ears, micrognathia, cardiac defects, cleft palate, unusual digital anomalies, developmental retardation, and obstructive sleep apnea.

The computed tomographic (CT) scans of five patients with surgically and histologically proven trigeminal neuromas demonstrated the varied appearance of this tumor. In this series, tumors appeared in the middle cranial fossa, the posterior cranial fossa, or extended into both. Prior to contrast infusion, the tumors showed low density, normal brain density, or mixed normal and low density on the CT Scan. After contrast infusion, all tumors were well circumscribed with marked enhancement, which was either homogeneous or inhomogeneous. None of the trigeminal neuromas had surrounding brain edema. The three larger tumors had significant mass effect with hydrocephalus and displacement of the fourth ventricle or compression of adjacent cisterns.
A community-wide study was conducted in metropolitan Baltimore in which the survival of 1,307 patients with acute myocardial infarction was examined according to use of anticoagulants. The adjusted in-hospital case-fatality rate was lower for patients receiving anticoagulants (18%) than for those not receiving this therapy (31%). This difference persisted in each period examined in the study (1966 and 1967 or 1971) and was found in 17 of 20 participating hospitals. For hospital survivors followed up for as long as ten years, a better survival was again found for those treated with anticoagulants in the acute phase compared with those not treated.
A community-wide study was conducted in metropolitan Baltimore in which the prognosis of 330 patients hospitalized with an acute myocardial infarction (MI) complicated by ventricular fibrillation or cardiac arrest (VFib/CA) was compared with that of 1,071 patients hospitalized with acute MI not complicated by VFib/CA. As expected, the in-hospital case-fatality rate among patients with MI complicated by VFib/CA was significantly higher than that in patients without VFib/CA. However, for patients discharged alive from the hospital, no significant differences in long-term survival were found between patients with MI with or without VFib/CA. These results suggest that attempts at preventing subsequent mortality should be diligently pursued in patients with MI who are discharged alive from the hospital regardless of the occurrence of VFib/CA in the acute phase.
