Consultation-liaison scheme for self-poisoned patients in a general hospital.

In a prospective clinical trial 276 self-poisoned patients consecutively admitted to hospital were randomly allocated to medical teams or to psychiatrists for an initial psychiatric assessment and a decision about "disposal." Junior doctors and nurses received instruction in this work. While awaiting the outcome of the trial the randomisation was continued for 13 months and 729 allocations were made altogether. Physicians requested psychiatric opinions for roughly one in five of their patients. In other respects medical teams performed similarly to psychiatrists. Provided that due attention is given to teaching junior staff and to ensuring that psychiatric treatment and social-work support are available once patients have been assessed, such a consultation-liaison scheme could be adopted in other hospitals. This would help to change unfavourable attitudes towards self-poisoned patients and contribute to the general training of doctors and nurses.

Two immunofluorescent methods compared with a radial immunodiffusion method for measurement of serum immunoglobulins.

We found high correlations when the values for immunoglobulins G, A, and M, obtained from the analysis of a group of 50 or more human sera by a measured-time radial immunodiffusion method, were compared to values found by two new commercial immunofluorescent methods. The Fiax and the Immuno-Fluor systems make use of fluorescently labeled antibodies and solid-phase immunoadsorbents and offer simple, precise, and objective methods for immunoglobulin quantitation. The Fiax procedure is less time consuming than is the Immuno-Fluor and gives IgG and IgA concentrations that are in close agreement with those found by radial immunodiffusion and IgM values that are significantly lower than the radial immunodiffusion values. The IgG results determined by the Immuno-Fluor method compare well with the radial immunodiffusion results, uhile the IgA values are significantly higher and IgM values significantly lower than those obtained by radial immunodiffusion.

Psychological and social evaluation in cases of deliberate self-poisoning admitted to a general hospital.

In a prospective clinical trial 312 cases of self-poisoning (276 patients) consecutively admitted to hospital were randomly allocated to medical teams or to psychiatrists for an initial psychiatric assessment and a decision as to "disposal." Junior doctors and nurses received some instruction in this work. Both groups of assessors asked for help from social workers when necessary. Once the medical teams had completed their assessments, psychiatrists provided most of the hospital treatment. Follow-up at one year showed no significant difference between the two groups of patients in the numbers who repeated their self-poisoning or self-injury (or both), or committed suicide. Provided junior doctors and nurses are taught to assess self-poisoned patients, we think medical teams can evaluate the suicidal risk and identify patients requiring psychiatric treatment or help from social workers, or both. Contrary to the Department of Health's recommendation that all cases of deliberate self-poisoning should be seen by psychiatrists, we have reached the conclusion that physicians should decide for each of their patients if specialist psychiatric advice is necessary.

The characterization of some of the antigens and allergens in ragweed pollen.

Ragweed extract (RW) and antigens E, K, BPA-R and Ra3 were characterized by electrophoresis on starch and polyacrylamide gels. RW extract and antigen E were also subjected to crossed immunoelectrophoresis. All the antigens contained isomers and contaminating proteins. RW separated by starch gel electrophoresis was skin tested and allergens unrelated to antigens E, BPA-R, Ra3 and K were found.

Characterization of some of the enzymes in ragweed pollen.

Ragweed pollen contains 11 esterase, 5 acid phosphatase, 2 alkaline phosphatose, 2 hexokinase, 2 glucose-6-phosphate dehydrogenase isozymes and one leucine amino peptidase band which can be separated by starch gel electrophoresis. The isozymes were distinguished from one another by their electrophoretic mobility, heat inactivation temperatures and antigenic differences.

[Lithium prophylaxis].

The author considers some difficulties and discrepancies in the diagnosis of schizophrenia, schizo-affective psychoses on the one hand and manic-depressive psychosis on the other in a group of out-patients receiving treatment with lithium for the prophylaxis of affective disorders. Factors common to both groups and being the basis for the selection of lithium prophylaxis are considered. Special attention in these cases is paid to the course of the disease.

Amphetamine and other non-opioid drug users attending a special drug dependence clinic.

This paper reports the findings in 104 users of nonopioid drugs (mainly amphetamines) attending a drug dependence clinic in London between March 1968 and February 1969 in relation to demographic characteristics, forensic, psychiatric, and medical histories, clinical groups, management policy, and the use of biochemical tests for the presence of drugs in the urine.Nearly one-third gave a history of starting amphetamine misuse while at school (13.5%) or within a year of leaving school (16.3%). Amphetamine psychosis had occurred in 35% of cases and was a more frequent complication of intravenous than of oral abuse. Except for a small proportion of older patients who have become dependent on drugs originally prescribed for therapeutic purposes, "maintenance therapy" is most unlikely to be effective in the treatment of amphetamine dependence. Nevertheless, about one-quarter of the younger patients in this series had obtained prescriptions from general practitioners after starting their drug abuse illicitly. There is a strong case for the compulsory notification under the Misuse of Drugs Act (1971) of persons dependent on amphetamines.
