Diagnostic use of the immunoperoxidase technique for cutaneous plasma cell lesions.

Distinguishing cutaneous neoplastic plasma cell infiltrations from inflammatory reactions containing predominantly plasma cells can present diagnostic problems when conventional histological techniques are used. Eight cases of cutaneous lesions, composed almost exclusively of plasma cells, are presented. We discuss the use of the peroxidase-antiperoxidase technique to distinguish malignant plasma cells that produce only one type of immunoglobulin from benign lesions that are polyclonal and produce several immunoglobulins.

Liver granulomas: a possible paraneoplastic manifestation of hypernephroma.

A patient with hypernephroma presented with signs of liver dysfunction. Liver biopsy disclosed the presence of granulomas. Extensive search for a known cause of hepatic granulomas was negative. The presence of granulomas in lymph nodes draining the neoplasm, the improvement of liver dysfunction after nephrectomy, and its aggravation preceding overt metastatic manifestation support the existence of a cause and effect relationship between the renal cell carcinoma and the liver granulomatous disease.

The effect of lithium chloride on tumour appearance and survival of melanoma-bearing mice.

The possible effect of lithium chloride, a compound which reduces the incidence of infection in cancer patients, was investigated on murine melanoma. C57 BL syngeneic mice were inoculated i.p. with B16 melanoma cells. The animals were divided into 4 groups, receiving daily i.p. treatment with saline--group 1, controls; lithium chloride--group 2, bleomycin and vinblastine--group 3, and lithium chloride with bleomycin and vinblastine--group 4. Animals in group 4 had a significant delay in tumour appearance, a higher degree of tumour necrosis, and a longer survival rate. In addition a significant reduction of serum lithium concentration was noted in animals of this group in comparison with animals in group 2, treated with lithium chloride alone. There was no lithium-induced leukocytosis.

Demonstration of AA-protein in formalin-fixed, paraffin-embedded tissues.

AA-protein was identified by SDS-acrylamide electrophoresis in amyloid fibrils fixed in formalin after isolation from fresh-frozen tissues obtained from patients with familial Mediterranean fever (FMF) amyloidosis and idiopathic AA-amyloidosis and, following deparaffination, rehydration and homogenization of embedded formalin-fixed tissues of old autopsy cases of the hereditary amyloidosis of FMF and amyloidosis acquired in association with tuberculosis, bronchiectasis, and rheumatoid arthritis. That AA-protein is unaltered by formalin was firmly established by agar gel diffusion using specific rabbit anti-AA serum. By contrast, AL proteins could not be demonstrated either in formalin-fixed amyloid fibrils derived from fresh-frozen tissues of a patient with presumably AL-amyloidosis dominated by cardiomegaly and one with AL-kappa amyloidosis or in blocks of cases of familial neuropathic amyloidosis, multiple myeloma, and idiopathic amyloidosis with cardiopathy. AA-protein is not denatured by formalin and retains its typical electrophoretic, chromatographic, and immunologic characteristics even 30 years after fixation and paraffin-embedding.

The role of activity in anticipating and confronting stressful situations.

This paper is an analytical review of studies comparing stress reactions in threatening situations during which the person can or cannot take action. The studies reviewed were conducted mainly in real-life situations and involved diverse forms of coping activities under various conditions of danger and distress. To achieve a better understanding of the effect of activity on stress reactions, several distinctions are made: between various types of stress reactions; between various indices and measurements of stress reactions; between activity which is directly related to the source of threat and activity which is not; and between various personality dimensions. The adaptive potential of activity in stressful situations is discussed in terms of: the sense of mastery and control provided by activity; activity as a means of attention diversion; and activity as a means of discharging energy generated by mobilization. It is suggested that taking action under stressful conditions, as opposed to remaining passive, is a powerful coping tool.

Assessment of seasickness and its consequences by a method of peer evaluation.

A Seasickness Questionnaire, based on a peer-rating technique, was administered to 172 Israeli Navy sailors. Use of interrater agreement as a reliability index and two cutoff points for the partition of the sample resulted in the elimination of about one-third of the initial sampl and the formation of two subsamples-the "sick" (N equals 45) and "not sick" (N equals 73) sailors. By using differentially weighted scores for various intensities of seasickness, it was demonstrated that two normally confounded aspects of seasickness can be assessed independently: physical symptoms, which constitute the apparent severity of the sickness (A.S.S.), and level of actual performance (A.P.), which reflects the individual's attempts to cope with the sickness. A.S.S. and A.P. showed differential relationships with variables such as personality disposition of active coping, military rank, and sociometric status.

Plasma cell granuloma of the thyroid gland.

Plasma cell granuloma is a tumor-like lesion consisting of mature plasma cells, sometimes with Russell bodies and reticuloendothelial cells, supported by a stroma of granulation tissue. These tumors are most common in the lungs and were sporadically reported in extrapulmonary sites. A rare case of plasma cell granuloma of the thyroid is reported. The value of immunohistochemical staining is demonstrated.

Nucleolar organizer regions in follicular tumors of the thyroid.

BACKGROUND: Nucleolar organizer regions (NORs) are loops of ribosomal DNA that occur in nucleoli and that transcribe to ribosomal RNA. NORs have been identified by means of the Ag-NOR technique in routinely processed tissues, and were found to be of discriminative value between some types of benign and malignant lesions. METHODS: Follicular lesions of the thyroid (17 adenomas and 25 carcinomas) were examined. Ten normal thyroids served as the control group. All slides were stained by the Ag-NOR technique and the number of Ag-NOR dots were counted in 50 randomly selected cells. The mean number of Ag-NORs was calculated for each case. Data were statistically analyzed by the Student's unpaired t test. RESULTS: The mean Ag-NOR counts were statistically higher in follicular carcinomas as compared to either follicular adenomas or the normal thyroid. Higher Ag-NOR counts were found in the more aggressively behaving tumors. CONCLUSIONS: It is suggested that the Ag-NOR technique could be of use as an adjunct to diagnostic histopathology and as an indicator of biologic behavior in follicular tumors of the thyroid.

Metastatic carcinoma of oral soft tissue.

Metastatic carcinomas to the oral region are rare. Most of the lesions are located in the bones of the jaw. The medical literature contains quite a few examples of metastatic lesions located strictly in the oral soft tissue. We present two cases of oral soft tissue metastatic carcinoma of urogenital origin.

Brush cytology and biopsy in the diagnosis of colorectal cancer. A comparison.

OBJECTIVE: To determine the usefulness of brush cytology of colorectal lesions as compared to biopsy examination. STUDY DESIGN: One hundred brushing cytologies and biopsies were performed on patients who underwent colonoscopic examination for different symptoms. The cytologic smears were classified into five cytologic diagnostic categories. The histologic diagnoses were adenocarcinoma, adenoma and nonneoplastic lesion. RESULTS: Twenty-six cases were cytologically positive for malignant cells, and all were histologically diagnosed as adenocarcinoma. Nineteen cases were suspicious for malignancy on cytology; histologically, eight of them showed adenocarcinoma. Two other cases proved to be adenocarcinoma in subsequent biopsies. Nine cases were adenomas, with severe dysplasia in five of them. Fourteen cases that were cytologically negative with minimal glandular atypia showed seven adenomas and seven nonneoplastic lesions on biopsy. Forty cases negative for malignant cells showed 19 adenomas and 21 nonspecific changes in the biopsy examination. CONCLUSION: Colonoscopic brushing cytodiagnosis is a sensitive technique for the detection of colorectal cancer. The combination of brushing cytology and biopsy improves the accuracy of diagnosis.

Michaelis-Gutmann bodies in a healing brain infarct.

A case of Michaelis-Gutmann bodies in a healing brain infarct is described. Morphologically this was consistent with cerebral malacoplakia. There are only 6 previously reported cases of cerebral malacoplakia, and only 1 of them was a consequence of postpartum stroke. The histologic and ultrastructural features of the malacoplakia lesion are reported and compared with the previously reported cases.

Lymph node revealing solution: a new method for lymph node sampling: results in gastric adenocarcinoma.

Staging of gastric carcinoma depends on exact lymph node status. However, very small nodes are not easily found as they are obscured by the surrounding adipose tissue. The purpose of the present study was to demonstrate the usefulness of a Olymph node revealing solutionO (LNRS) in gastric cancer. The perigastric adipose tissue of ten OproblematicO cases of gastric carcinoma, in which <10 lymph nodes were found using the traditional method, was immersed in LNRS for 6-12 h. Subsequently, the lymph nodes stood out as white chalky nodules. They were excised and processed routinely. The traditional method yielded a total of 30 lymph nodes with a mean size of 6.69 +/- 3.43 mm. The LNRS revealed 89 additional nodes with a mean size of 3.03 +/- 3.43 mm, which was significantly smaller. The Node (N) stage was changed in four cases from Nx to N0, in one case from N1 to N2, and in one case from N0 to N2. LNRS seems to be the technique of choice for staging of patients with gastric adenocarcinoma in whom <10 lymph nodes were found with the traditional method and accurate staging was not possible.

Nucleolar organizer regions in conjunctival nevi and melanomata.

The objective of this study was to compare the argyrophil nucleolar organizer region (Ag-NOR) counts of conjunctival nevi and melanomata and to compare the efficacy of this method in their differential diagnosis. Nine histologically diagnosed conjunctival nevi and three conjunctival malignant melanomas were studied. Representative sections were stained using the AgNOR technique. Fifty cells of each melanocytic lesion were randomly selected without knowing their histologic diagnosis. The AgNORs were visualized at a magnification of x1000. They consisted of clusters >1 micron in diameter and of satellites <1 micron in diameter, which were counted and recorded for each cell separately. The mean AgNOR count for the nevi was 1.12 clusters and 1.72 satellites per cell, while the count for the melanomas was 1.6 clusters and 6.8 satellites per cell. These results are statistically significant. There was no overlap between the number of AgNOR clusters and satellites in conjunctival nevi and melanomas, indicating that the AgNOR count might be a useful tool in distinguishing benign from malignant conjunctival melanocytic lesions.

Histological changes in the cervical lymph nodes after radiotherapy.

Radiotherapy to the neck is frequently employed in the treatment of malignancies, sometimes alone and sometimes before or after surgery or with or without chemotherapy. We checked the histopathological changes in the neck after radiotherapy, dividing the changes into two groups. One with short-term changes between six months to one year, and the second group with long-term changes after more than six years. We compared these two groups with a control group without radiotherapy. The changes in the long-term group were more pronounced than in the short-term group, with more stromal fibrosis, vascular changes, and specially lymph nodes smaller than one centimeter. Thus, bigger lymph nodes (more then one centimeter) are more radiosensitive and become smaller after six years.
