Examination of 29 cases of fracture of the distal radius with restricted motion or persistent pain in 22 patients showed that most had been caused by incongruity of the distal radioulnar joint or by rotational malalignment in supination or pronation. Imaging of the distal radioulnar joint without superimposition enables us to detect whether disorders of the joint facets are impeding movement. Exact knowledge of the malalignment aids in the planning of osteoplastic surgery.

After complicated distal radius fractures with persistent symptoms, a variety of radiological methods may be indicated, including plain films, magnification views, examination under stress and arthrography. CT accurately differentiates abnormalities of the distal radio-ulnar joint and aids in deciding on surgical treatment. Demonstration of degenerative changes and of subluxations is much more satisfactory in the third plane, since there is no superimposition.

Modern obstetrics should include not only an optimal labour monitoring but also an adequate pain relief during delivery. Due to the discovery of specific opiate receptors, for example in the substantia gelatinosa of the posterior horn in the spinal cord and the proof of direct pain-modifying effects on spinal level, new ways of the therapy of pain have been developed. The analysis was made with 40 parturients, who had decided for an epidural analgesia. They were given 0.1 mg fentanyl epidurally at a cervical dilatation of 3 to 5 cm and regular uterine contractions. The registrated onset time of fentanyl was between 6 and 15 minutes. A fentanyl dose of 0.1 mg applied epidurally proved to be sufficient for all parturients for the alleviation of pain in the period of dilatation. The interval without pain amounted to 130 mins. The motoricity was not impaired by the peridural application of fentanyl, because the parturient was able to be mobilized under continuous telemetric monitoring including telemetric management of the profile of the uterine activity. The period of the active and latent phases as well as the total delivery time was within the desirable ranges. An impairment of the general condition, e.g. vomiting, nausea or pruritus, was not noticed. There were no circulatory disturbances either. After the application of fentanyl the cardiotocograms remained uninfluenced in all parameters. All parturients declared that they had a positive birth experience under this kind of obstetric analgesia and that they would decide for this method again in case of a further delivery.

The valuation and treatment of labor pains require complete morphological and anatomical informations about the neuronal systems concerned. A review is given about modern model conceptions in perception, modulation and transmission of pain, based on the latest knowledges in the pain research. The identification of central control- and modulation systems is very important for the actual theory of pain. Pains and also labor pains are caused as a result of complicated nerval connective principles with slim relation to endocrinological system and are also connected with a lot of individual psychical influences. Essentially, four functional stages are passed from the place of nociceptive perception to the registration in the telencephalon. These are depicted in consideration of numerous afferent and efferent, biochemical and hormonal, stimulated and inhibited modulation effects. Various pain inhibitory mechanisms, such as biogenic amines and endogenous opioids are discussed by reference to interesting aspects in new therapeutical possibilities in treatment of pain and also of labor pains.
The brothers and sisters described above show all the important exterior and radiological signs of thanatophoric dwarfism type I. The other described complications of pregnancy such as hydramnios, premature birth or abortion did not occur. According to present day publications a homogeneous heredity have not been noticed, so the risk of a repetition after having a child with thanatophoric dwarfism is given as 1:50 to 1:4 for healthy parents. Ultrasonic controls frequently repeated at short intervals, especially with hydramnios or an atypical development of the skeleton combined with a radiograph and - in case of doubt-fetography enables a diagnosis of thanatophoric dwarfism to be made from about the 18th week of gestation. With a positive diagnosis an interruption of pregnancy is indicated because of the absolutely lethal prognosis.
The excretion of the C19O2-fraction of 17-oxosteroids in urine (C) and the concentration of testosterone in serum (T) have been compared in 15 women with hirsutism during tests with suppression of the adrenals and the ovary and with stimulation of the ovary. The behaviour of both parameters seemed to be only qualitatively similar in all three phases of the tests. In one of the test phases a statistically significant correlation between the percentage of decrease or increase of C and T has been observed (First phase: r = 0.242; second phase; r = 0.457; third phase r = 0.392). The mean relative changes of C in the adrenal suppression phase was more distinct (P less than 0.002) than that of T. Both parameters seem to be of different diagnostic meaning.
Reported in this paper is the use of PGF2 alpha to induce legal abortion in 43 patients with uterine risk factors (Group 1) or with general diseases manifest before (Group 2). The abortions were induced between the eighth and 24th weeks of pregnancy. PGF2 alpha was applied extra-amniotically, intra-amniotically or by combined approach, that is intra-amniotic or intravenous with extra-amniotic. - Twenty-seven women were included in Group 1, and in 25 of them pregnancy was disrupted without any complications. Complete expulsion of foetus and placenta, caused by rupture in the posterior labium of the os uteri, occurred to one of the patients with myomatous uterus. Acute ascension developed in another patient, three days from the intervention. - Group 2 was composed of 16 patients. Induced abortion was without any complications in 15, while one patient with psychosomatic epilepsy had an epileptic fit. Her pregnancy had to be terminated by hysterotomy. - The use of PGF2 alpha is considered an effective method to induce abortion in patients with risk factors.
