[Carcinoembryonic antigen (CEA) (author's transl)].

Serial determinations of CEA concentrations in serum were performed postoperatively in 303 patients with histologically confirmed adenocarcinoma of the gastrointestinal tract. The trend of the time course of computerised CEA curves made early diagnosis of recurrence or metastases possible. Diagnosis of recurrence by means of a rise in CEA concentration preceded positive clinical diagnosis by up to 10 months. In all 26 cases confirmation was obtained by second-look operation or other diagnostic means. Analysis of serial CEA measurements made it possible to distinguish between generalised metastasization and local recurrence of the tumour, on the one hand, and limited metastasization at the site of recurrence, on the other.

The effect of indomethacin, 6-hydroxydopamine, saralasin, and hemorrhage on renal hemodynamics.

This report describes the response of the renal circulation to prolonged hemorrhagic hypotension and reinfusion of blood, and to the effect of a variety of drugs (saralasin, indomethacin, and 6-hydroxydopamine, 6-OH-DA). Plastic microspheres were used to measure blood flow perfusing the entire kidney and also the outer cortex, inner cortex, and medulla of the kidney. Cardiac output was determined with a Doppler flow probe, and total and regional flows were calculated. Redistribution of blood flow from outer cortex to inner cortex and medulla occurred during hemorrhage and after administration of saralasin and 6-hydroxy-dopamine, while indomethacin did not alter intrarenal flow distribution. Total renal flow increased after reinfusion of blood and saralasin, but decreased after indomethacin. It did not change after 6-hydroxydopamine. The results demonstrate that changes in total and intrarenal flow occur independently and are probably due to different mechanisms.

Immunological tumour profile: organ-specific carcinoma diagnosis in patients employing the macrophage electrophoretic mobility test.

The macrophage electrophoretic mobility (MEM) test provides a highly sensitive in vitro technique for the detection of cell-mediated immunity in man. The principle involved is the lymphokine-mediated reduction of the negative surface charge of guinea pig macrophages shown by the slowing of the macrophages during cell electrophoresis. Lymphocytes from 162 patients were tested by MEM. They were exposed to a battery of KCl extracts from normal and malignant human tissues, to encephalitogenic protein (EP), to carcinoembryonic antigen (CEA), and to thyroglobulin. Variable lymphocyte responses to EP, CEA and KCl extracts from different cancers gave MEM reaction profiles common to patients with carcinomas of the same organ origin.

Synthesis and antimicrobial activity of azasteroid-type compounds and related systems. Effect of hydrophilic and lipophilic groups on activity.

Pyrazole-, pyrazolone- and isoxazole-containing systems were prepared from 3,4-dihydro-6-(hexyloxy)-1(2H)-naphthalenone, 3,4-dihydro-6-(hexadecyloxy)-1(2H)-naphthalenone,3,4-dihydro-6(2-dimethylaminoethyloxy)-1-(2H)-naphthalenone, 3,4-dihydro-7-hexyloxy-1(2H)-phenanthrone, and 3,4-dihydro-7-(2-dimethylaminoethyloxy)-1(2H)-phenanthrone. A number of compounds derived from 7, 8-dihydro-5(6H)-quinolinone were also synthesized and characterized. Both hydrophilic and lipophilic groups were incorporated into certain systems as well as cidal groups. The compounds were screened for their in vitro inhibitory activity against Bacillus subtilis and Pseudomonas fluorescens. Structure-acitivity relationships among the molecular systems are discussed.

Regional blood flow, contractility and metabolism in early myocardial infarction.

The effects of occlusion of the left anterior descending coronary artery on a variety of metabolic parameters was examined in both infarcted and noninfarcted areas of the dog heart. These included mitochondrial performance, glycolysis, in vitro contractility, and regional myocardial blood flow. Measurements were made at 1 and 3 h after onset of ischemia. Regional coronary blood flow was measured in infarcted, noninfarcted and borderline regions using radioactive microspheres. Blood flow through the ischemic area was reduced by an average of 69% after 1 h of ischemia, and 75% after 3 h. After 3 h the subendocardium of the borderline region also revealed a significantly reduced blood flow. Mitochondria isolated from the ischemic region of the heart exhibited a substantial decrease in the rate of respiration (QO2), and minor reductions in the coupling between oxidative phosphorylation and electron transport (RCI), and in the amount of ADP phosphorylated per oxygen reduced (ADP:O ratio). Levels of hexose monophosphates were elevated 1 and 3 h after ischemia was initiated. At the same time, the concentration of fructose-1,6-diphosphate declined markedly, reflecting inhibition of glycolysis at the phosphofructokinase level. Concentrations of the adenosine phosphate moieties, as well as creatine phosphate, were reduced, while levels of free fatty acids were elevated in ischemic tissue. The in vitro contractility of glycerinated ischemic muscle fibers was also depressed. Significant changes were found in maximal tension development (P0), maximal rate of tension development (dp/dtmax), time to peak tension (t0), and shortening velocity at zero load (Vmax).

[Pathology of the malignant ovarian tumours (author's transl)].

A review of the principles of histological classification of the malignant ovarian tumours is given. In the center of the discussion figure the tumours originating from the surface epithelium, which make up the majority of ovarian tumours. In dependence on the tumour type they differ in tendency to spread and in prognosis. The clinico-pathological serial examinations existing so far stress the importance of the histopathologic differentiation between facultatively malignant ovarian tumours (borderline tumours) and indubitable carcinomas. Close relations between the type of a tumour and its tendency to spread manifest themselves, for instance, by the different frequency of an affection of both the ovaries, which relates to the different types of carcinomas. Apart from the lymphogenic dissemination, an early development of metastases to the diaphragm seems to have a particular bearing on the spreading of ovarian cancers.

The effects of psilocybin on primary process content in language.

The hypothesis that psilocybin induced primary process thinking was assessed. On four separate occasions, a subject was asked to write before, during, and after the hallucinogenic experience induced by doses ranging between 80 and 200 microgram/kg of psilocybin. Texts produced at the drug peak contained significantly more primary process content and were significantly more stereotyped on several measures than those written before or after the drug peak.

Nonverbal dialogue with the brain-damaged elderly.

In a population of twelve 63- to 89-year-old residents of a nursing home, rank-ordered scores of copying errors on the revised Benton visual retention test were found to be significantly correlated with the rotational shifts in the Minnesota percepto-diagnostic test. Residents on both ends of the continuum of brain damage could paint interesting, expressive and/or original pictures, in spite of the fact that the first eight residents were on psychotropic medication and the last four were occasionally handicapped by constructional apraxia. Ten adjudicators ranked pairs of figure drawings for each resident in decreasing order of 'aesthetic pleasingness' and 'originality'. Degree of brain damage was found unrelated to both measures. Interadjudicator agreement was stronger on 'originality' than on 'aesthetic pleasingness'. Some of the beneficial and provocative results of art therapy with the elderly are elaborated ans a plea made for accepting their observed creative performance as part of the realm of legitimate art.

[Acute carbon tetrachloride poisoning (author's transl)].

Three cases of acute CCI4 poisoning are reported which came for admission in the stage of hepatorenal insufficiency. All three intoxications occurred in the home by inhalation as a result of disregarding the safety precautions. The patients remembered using the noxa only after renewed questioning by the doctors. All three patients were able to be discharged cured after appropriate therapy, including peritoneal or hemodialysis. A disturbance of the capacity for urine concentration could be demonstrated up to 3 years after the exposure, as a last limitation of function.
