Search for a nontoxic surgical scrub solution for periorbital lacerations.

The purpose of this study was to identify a skin wound cleanser that could be used safely to cleanse lacerations of the periorbital region. The irritation and toxicity to the eye of the commercially available skin wound cleansers were quantitated using numerical values and weights for the ocular lesions in experimental animals. Shur Clens did not elicit lesions in experimental animals or in a series of 20 patients with periorbital lacerations. On the basis of this study, Shur Clens is recommended as a safe skin wound cleanser for periorbital lacerations.

Evaluation of an emergency medical service referral system for burn patients.

It is the purpose of this report to examine the process of burn care of hospitalized burn patients in the Commonwealth of Virginia over a 21 month period. Eighty-nine per cent of the hospitals within the state participated in the study, the results of which provide a positive indication of the performance of our statewide emergency medical service referral system. This system was successful in redistributing patients, with the more severely burned patients being treated in the specialty burn treatment facilities. The process of burn care within the hospital setting was also consistent with the patient's severity of injury.

Quantitation of skin-flap survival: a computer-based method.

We have proposed a standard scheme for presenting data on experimental skin flap survival in pharmacologic studies that will aid in comparing results. In addition, we have presented a method using desktop computer accessories for accurately measuring the area of any flap.

Puncture wounds.

Puncture wounds are common and challenging problems facing the physician. In general, puncture wounds can be classified into two distinct types. In one type a child usually sustains a puncture wound of the foot. The other group of puncture wounds occurs among hospital employees. These seemingly innocuous injuries may have serious sequelae that can be averted by prompt appropriate care of the wound. The fundamental trends of treatment of puncture wounds include cleansing the wound, debridement of any necrotic or devitalized tissue, and appropriate immunoprophylaxis.

A user-oriented information system for emergency medicine.

A user-oriented Emergency Medical Services Information System ( EMSIS ) has been developed in our medical center that can readily be established by any interested medical center. This minicomputer based system provides online registration of poison center calls, access to the Poisindex (C) data base, concise patient information sheets, access to EMS treatment protocols, assistance in the differential diagnosis of acute poisoning, dosage recommendations for drugs with a narrow therapeutic margin, and kinetic analysis of drug overdoses. Video terminals access EMSIS in the Emergency Department, pharmacy, outpatient medicine clinic, pediatric clinic, medicine wards, poison control center, and biomedical engineering. A PDP 11/70 with 256 Kb core and 48 ports running under RSTS /E V7 .0 supports an average of 20 simultaneous users.

Biocidal braided sutures.

Once sutures become contaminated it is difficult for local tissue defenses or antibiotics to eliminate the bacteria and prevent infection. Impregnation of sutures with antibiotics before implantation is one way to prevent bacterial seeding of the suture surface. In this study, braided silk and Dacron sutures were impregnated with the antibiotic complex, neomycin palmitate. Using our standard wound model in the mouse, the efficacy of these biocidal sutures was determined in the presence of Staphylococcus aureus, Escherichia coli, Proteus mirabilis, and Pseudomonas aeruginosa. Implantation of these biocidal sutures into tissue contaminated with 10(7) organisms resulted in substantially decreased numbers of bacteria as compared with that of tissue receiving control sutures. In most cases, the tissue bacterial counts in the presence of biocidal sutures were also considerably lower than that for similarly contaminated tissue without a suture. Consequently, the presence of the neomycin palmitate eliminated the infection-potentiating effect of the suture. The therapeutic benefit of the biocidal sutures was related to the dose of antibiotic complex and was not adversely affected by delaying suture implantation.

Early ambulation after skin grafting of depressed wounds of the leg.

Silicone molds can be fitted precisely to skin grafted, depressed surgical defects of the leg and can be secured in place with a tight-fitting elastic support. The gravitational effect and resultant edema under the skin graft can be reduced. Earlier ambulation and better results than have been reported in the past can be obtained.

Knotting and handling characteristics of coated synthetic absorbable sutures.

The purpose of this study was to evaluate the knotting and handling characteristics of the new coated synthetic absorbable sutures. When compared to the coated polyglactin 910 sutures, the coated polyglycolic acid sutures displayed a lower coefficient of friction, encountered less tissue drag forces, and exhibited less flexural rigidity. In the case of sizes O, 2-O, and 3-O coated polyglycolic sutures, knot security was achieved with one less throw than with similar sizes of coated polyglactin 910 sutures. On the basis of these comprehensive mechanical performance tests, the knotting and handling characteristics of the coated polyglycolic acid sutures were judged to be superior to that of the coated polyglactin 910 sutures.

Mechanical performance of wound closure tapes.

On the basis of these in vitro and in vivo studies, Shur-Strips are recommended for wound closure. These tapes have a non-woven microporous structure with an adhesive that aggressively bonds to underlying skin. The level of adhesion of Shur-Strips to the skin can be enhanced by adhesive adjuncts. Shur-Strips are strong enough to resist breakage during clinical use and elongate sufficiently to prevent blister formation. The microporous structure of Shur-Strips permits rapid air transmission even when wet.

An improved design of the pneumatic counter-pressure trousers.

Inflation of the circumferential pneumatic counter-pressure trousers was associated with the development of hemiparesis in a multiple trauma patient. We attribute this complication to the marked movement of the spinal cord that results from inflation of the circumferential abdominal compartment of the trousers. The design of the circumferential pneumatic counter-pressure trousers was recently modified so that the inflation portion of the abdominal compartment was restricted to the anterior portion of the abdomen. Inflation of the abdominal compartment of the modified trousers resulted in considerably less movement of the spine than with the trousers having the circumferential abdominal compartment.

Epidemiology of minor burn injuries.

The incidence of minor burn injuries has been determined for the Commonwealth of Virginia during a 17-month period. The data set included all patients with burn injuries receiving treatment in the emergency department that did not require hospitalization. The risk of burn injury was calculated by age, race and sex. Examination of the data revealed large differences in the magnitude of age-specific incidence rates between men and women and the white and non-white population. Differences in the economic status of the population may explain a large proportion of the variation in the crude burn rates.
