Client perceptions of significant events in prescriptive and exploratory periods of individual therapy.

This study compared the impact of helpful and hindering events, as perceived by 40 clients, in two forms of psychotherapy: an exploratory, relationship-oriented therapy, and a prescriptive, cognitive/behavioural therapy. All clients received eight sessions of each type of treatment in a crossover design. Events were obtained by self-report both during and at the end of each period, and content analysed for type of therapeutic impact by three trained raters. Results showed that during treatment the most commonly occurring helpful impacts across both types of treatments were 'problem solution', 'awareness' and 'reassurance', while the most commonly occurring hindering impact was 'unwanted thoughts'. Similar impacts were reported at the end of each period, with the addition of 'personal contact'. In addition, it was found that 'problem solution' and 'reassurance' impacts were more commonly reported in prescriptive treatment, whereas 'awareness' and 'personal contact' impacts were more prevalent in exploratory treatment. Only the prevalence of 'unwanted thoughts' was correlated (negatively) with outcome. Some possible reasons for the lack of correlation between reported impacts and outcome are suggested.

Brief Structured Recall: a more efficient method for studying significant therapy events.

Previously, significant therapy events have been studied using a time-consuming form of tape-assisted recall. Brief Structured Recall (BSR) was developed to overcome difficulties with this earlier method. We carried out two case studies of time-limited therapy following the protocol of the Sheffield Psychotherapy Project (eight sessions each of Prescriptive and Exploratory therapies). In the first case study, we developed a more efficient method of identifying significant therapy events. In the second case study we developed structured interview schedules for obtaining client and therapist ratings and descriptions of a broad range of factors involved in significant events. The events identified in the two cases were typically 5-10 min in length, occurred in the second halves of sessions, contained 2-4 'peak' (most helpful) speaking turns, and were most often attributed to the therapist. The most common predominant impact of these events was Problem Clarification, followed by Personal Insight and Problem Solution. Treatment differences were more marked in the therapist's ratings of event impacts than in the clients' ratings.

Relative incidence of Crohn's disease and ulcerative colitis in six Melbourne hospitals.

Information on the relative incidence of Crohn's disease and ulcerative colitis was obtained by a prospective investigation at six Melbourne teaching hospitals. One hundred and eleven patients who presented with chronic inflammatory bowel diseases between 1980-1981 were admitted to the study. Forty (36%) patients were diagnosed as having Crohn's disease and 63 (57%) patients as having ulcerative colitis. The type of chronic inflammatory bowel disease could not be determined in eight (7%) patients. These findings suggest that the relative frequency of Crohn's disease and ulcerative colitis in Melbourne hospitals is within the range that is reported for northern Europe and the United States.

Rapid construction of large synthetic genes: total chemical synthesis of two different versions of the bovine prochymosin gene.

We have tested several different synthesis designs and assembly methodologies to develop an improved gene synthesis strategy which enables significantly longer nucleotide sequences to be easily constructed. This strategy, based in part upon our ability to synthesize high-quality extended-length oligodeoxynucleotides (over 100-mer in length), together with the use of chemical 5'-phosphorylation, and simplified low-melting-temperature agarose gel purification methods, combines ease, speed and high overall efficiency. We show that it is now feasible to synthesize routinely even long genes (at least 1-2 kb). To demonstrate this capability we have chemically synthesized and assembled two different versions of the gene encoding the bovine enzyme prochymosin (prorennin). One gene is essentially the natural bovine prochymosin gene sequence. In the second gene the codons have been optimized with regard to the codon bias of highly expressed yeast genes. Each synthetic gene was in excess of 1100 bp, yet they were assembled from only 13 or 14 pairs of complementary oligodeoxynucleotides (oligos), the average lengths of which were 87 and 82 bp, respectively. The 'mutation' rate was low enough to assess that more than 75% of all such oligo pairs (160-170 total nt) were error-free.

Cloning and characterization of the aldA gene of Aspergillus nidulans.

We have cloned and sequenced the aldA (encoding aldehyde dehydrogenase) gene of Aspergillus nidulans. The gene contains two introns which are similar in size and structure to other fungal introns. The amino acid sequence of aldehyde dehydrogenase (497 residues) shows a significant level of homology with analogous sequences in other organisms. Comparison of the primary structure of the active sites of the mammalian cytosolic and mitochondrial enzymes shows that the Aspergillus enzyme closely resembles the mammalian mitochondrial enzyme. Analysis of the 5' non-coding region of the aldA gene shows a TATA-like sequence located 90 bp upstream from the initiation codon. Two messenger-RNA start points are located 36 and 42 bp upstream from the start codon.

Synthesis and absorption of cysteine from the hindgut of the horse.

The extent to which cysteine synthesised by microbes within the hindgut of the horse is incorporated into plasma cysteine was estimated by an isotopic technique in two horses fed four different diets. The results showed that between 1 per cent and 6 per cent of the plasma cysteine was of microbial origin. It is argued that the maximum contribution of microbial cysteine, and presumably other amino acids of microbial origin, to the plasma pool is 12 per cent of the net supply. These data support the hypothesis that microbial amino acid synthesis within the hindgut of the horse does not significantly affect its amino acid status.

Development of a taxonomy of therapist difficulties: initial report.

While psychotherapists discuss amongst themselves and in supervision the struggles they experience face-to-face with their patients, empirical study of therapists' experience of psychotherapy has been scant and there has been little investigation of therapists' difficulties. We require tools for such investigation and in this initial report we describe the development of a taxonomy of situations that psychotherapists experience as difficult and indicate lines of research to which the taxonomy might be applied. We also suggest that our procedure might serve as a model of collective self-study for other groups of practitioner-researchers. Using accounts of our own difficulties as therapists, we constructed a consensual set of experimental categories that could be applied to describe them. These were designated as Therapist (T)-Incompetent, T-Damaging, T-Puzzled, T-Threatened, T-Out of Rapport, T's Personal Issues, Painful Reality/T's Ethical Dilemma (a combined category), T-Stuck, and T-Thwarted. We then employed a fresh set of 30 accounts, which were also drawn from our own experience but had not contributed to the development of the taxonomy, to study the taxonomy's reliability. Each of us rated the presence and predominance of the categories for each of the 30 accounts. Both presence and predominance of all nine categories in the taxonomy could be reliably identified, with about three raters required to ensure good reliability for all categories; alphas (for our group of seven raters) were in the range 0.88-0.95 for presence-absence and 0.83-0.94 for predominance. We conclude that the taxonomy is a serviceable tool for further investigation. We also show that we are discriminable from one another as therapists in terms of the kinds of difficulty we reported, differences amongst us conforming to what we know of one another's therapeutic practice. We note implications for the diagnostic use of the taxonomy in training and supervision.

Isolation of DNA encoding sucrase genes from Streptococcus salivarius and partial characterization of the enzymes expressed in Escherichia coli.

Restriction enzyme fragments containing two sucrase genes have been isolated from a cosmid library of Streptococcus salivarius DNA. The genes were expressed in Escherichia coli cells, and the properties of both enzymes were studied in partially purified protein extracts from E. coli. One gene encoding an invertase-type sucrase was subcloned on a 2.4-kilobase-pair fragment. The sucrase enzyme had a Km for sucrose of 48 mM and a pH optimum of 6.5. The S. salivarius sucrase clone showed no detectable hybridization to a yeast invertase clone. Two overlapping subclones which had 1 kilobase pair of DNA in common were used to localize a fructosyltransferase gene. The fructosyltransferase had a Km of 93 mM and a pH optimum of 7.0. The product of the fructosyltransferase was a levan. A fructosyltransferase clone from Bacillus subtilis did not hybridize to S. salivarius DNA. The properties of the enzymes were compared with those of previously characterized sucrases.

Prevalence of hemochromatosis in a random sample of asymptomatic men.

The prevalence of hemochromatosis was studied in 343 randomly selected male World War II veterans aged 58 years and older. Those men with a serum ferritin level of greater than 700 micrograms/l or a transferrin saturation of greater than 55% underwent liver biopsy. Hepatic iron concentration was assessed histochemically and by atomic absorption spectrophotometry. Histology was assessed blindly by one histopathologist. Of 21 veterans biopsied, four men had hemochromatosis, confirmed by venesection response, and the remaining biopsies all showed some histological abnormality. All four men with hemochromatosis had a transferrin saturation of greater than 55% while only three of the four had a serum ferritin level over 700 micrograms/l. The prevalence of hemochromatosis detected in this sample was 1.2%, which is higher than that suggested by overseas studies.
