Congenital cholesteatoma of the ear.

Congenital cholesteatoma of the ear is a capricious and challenging problem for the otologist. We present 21 cases managed at the Mayo Clinic from 1961 to 1983. In all but one unusual case, the cholesteatoma developed behind an intact tympanic membrane and there was no history of aural infections. Preoperative symptoms and signs were predominantly conductive deafness with either a normal tympanic membrane or an opaque, whitish appearance to the drum. Disease was located in the middle ear or in the middle ear and mastoid, except in one patient who had cholesteatoma extending into the petrous apex. Surgical management depended on the extent of the lesion, and recurrences were unusual, suggesting that congenital cholesteatoma may not be as aggressive as acquired cholesteatoma.

Granular cell tumors of the tongue.

We have presented a series of 19 new cases of granular cell tumors of the tongue. This benign condition is often difficult to diagnose if an adequate biopsy specimen has not been obtained. Insufficient biopsy material that includes only the overlying pseudoepitheliomatous hyperplasia may distort the pathologic picture. Review of frozen sections should provide an accurate diagnosis, and local surgical excision should resolve the problem completely.

Correlation of preoperative immunologic test results with recurrence in patients with head and neck cancer.

Numbers and percentages of T and B cells in peripheral blood; blastogenic responses to PHA, Con A, and PWM; and serum levels of albumin, total protein, and IgA were assessed in patients with stages I to III operable squamous cell carcinoma of the upper aerodigestive passageways in the head and neck. The patients were categorized into two groups: those who remained tumor free and those in whom cancer recurred. It was found that pretreatment PHA- and Con A-induced blastogenesis was often depressed in patients in whom clinically apparent recurrences developed regardless of stage or nodal involvement. This point is consistent with the idea that cellular immune function (blastogenesis) can identify, within the clinical staging system (TN), subpopulations of patients who are less likely to survive after conventional therapy.
