A threshold (1:10) rubella antibody hemagglutination inhibition (HAI) titer was obtained for 288 of 6537 (4.4%) obstetric patients. Random sera from 84 of these patients were compared for rubella antibody by both HAI and a very sensitive radioimmunoassay (RIA) technique. By RIA, 17% of the sera had no detectable rubella antibody, suggesting that those patients were truly susceptible to rubella. In addition, 8 of 55 paired prevaccine and postvaccine sera with titer increases demonstrated by HAI were analyzed by RIA. Seven of the 8 prevaccine sera were shown to have no rubella virus antibody by RIA, and RIA showed seroconversion in all 8 postvaccine sera.

Urinary and blood hormonal profiles were studied throughout a monthly cycle in a patient with familial breast cancer. Two comparison cohorts (one high-risk and one low-risk) were studied concurrently. Findings disclosed that our breast cancer-affected patient showed a distinctive hormonal pattern characterized by significant elevation throughout the cycle of plasma estrone, estradiol, and prolactin. Save for a depression in plasma FSH in the early follicular phase, this hormone, as wells as LH and progesterone patterns in our patient, were similar to the comparison cohorts. Urinary estrone and estradiol patterns in our patient were elevated early in the follicular phase. Our patient also showed a depression in urinary estrone, estradiol, and estriol following ovulation, which persisted throughout the luteal phase. Blood and urinary hormone patterns in the high-risk cohort were not demonstrably different from the low-risk cohort, with the exception of plasma prolactin. The results on the latter hormone showed an unexpected significant depression throughout most of the menstrual cycle in this low-risk cohort. We conclude that estrone and estradiol elevations, as clearly evidenced in our breast-cancer-affected patient, may provide clues that ultimately might be used as an etiologic discriminant for breast cancer risk and which may also play a pathogenic role in this disease. Since this involved a single patient, our conclusions must be interpreted cautiously.
The 18-year-old white male developed acute myeloblastic leukemia (AML) 25 months after diagnosis of poorly differentiated lymphocytic lymphoma, diffuse pattern (PDLL-D), involving cervical, supraclavicular, and mediastinal lymph nodes as well as bone marrow. Treatment of the lymphoma consisted of 2,000 rads to the mantel area and 18 months of chemotherapy with intravenous (IV) methotrexate (400 mg/m2), vincristine, and prednisone, alternating every two weeks with IV cyclophosphamide (1,000 mg/m2), vincristine, and prednisone plus monthly intrathecal methotrexate. Thereafter, a complete remission was maintained without therapy until the onset of AML. Several pseudodiploid clones containing multiple structural rearrangements and a hypodiploid clone were identified in the circulating blood at the time of diagnosis of AML. Induction therapy consisting of cytosine arabinoside, 5-azacytidine, vincristine, and prednisone was unsuccessful, and the patient died of sepsis two months after diagnosis. This case calls attention to the increased risk for subsequent acute nonlymphocytic leukemia in patients previously treated for nonhodgkin lymphoma.

Of 1,500 consecutive deliveries, there were 41 term infants with suspected sepsis in association with a positive culture for group B streptococci (GBS) from either the infant soon after birth, or the mother's urine routinely collected three days postpartum. A statistically higher incidence of pneumonia was found in these infants as compared to matched controls. Infants born to mothers with GBS in the urine had a high incidence of suspected sepsis (43.6%) as well as a high incidence of pneumonia (38.6%). We conclude that term infants colonized at birth with GBS or born to mothers with GBS isolated from their urine are at high risk of having pneumonia.

We describe 2 extended kindreds supposedly manifesting familial multiple adenomatous polyposis coli (FPC), but which show marked heterogeneity in the phenotypic expression of colorectal adenomatous polyps. In one family, 2 individuals had diffuse polyposis at very early ages (7 and 10 years), while 6 others (aged 23 to 72 years) had solitary polyps only. Of the patients with solitary polyps, 2 had associated colonic malignancies (ages 26 and 35), while another had a prophylactic colectomy performed at age 46. In the second family, 5 of the 11 patients with evidence of polyps showed the classical presentation of FPC, while the remainder showed marked phenotypic variation. The marked variability in frequency and location of colon polyps points to the need to reassess our traditional criteria for diagnosis of FPC. The high risk of early onset colon cancer in patients from these families who have the most minimal manifestation, namely isolated polyps, recommends more careful scrutiny of supposedly unaffected members of all FPC kindreds.

Obstetric patients with serologic evidence of syphilis should have cerebrospinal fluid (CSF) analysis to determine the presence of tertiary disease. In the present study, 19% of untreated and asymptomatic patients with serologic evidence of syphilis were found to have tertiary syphilis following examination of CSF. Despite maternal therapy, 11% of the infants born to mothers with syphilis had reactive serum and CSF involvement as documented by the presence of a fluorescent treponemal antibody absorption (FTA-ABS)-specific IgM.

Over a 20-year period at WHMC the annual incidence of acute antepartum pyelonephritis decreased from 4.0% to 0.8%. This decrease is correlated with the eradication of asymptomatic bacteriuria. Since urinary tract infections frequently recur, these antepartum patients must be monitored after therapy.
A kindred in which five first-degree relatives were initially known to exhibit colorectal, endometrial, and unspecified carcinoma for two generations, occurring at an early age and including three cases of multiple primary cancer, is consistent with criteria for the cancer family syndrome. Follow-up included a diligent surveillance program that led to the early diagnosis of colon cancer in a young member of the third generation who had been considered at high risk for this lesion. Manifestly, hereditary cancer syndrome identification may be expedited through longitudinal study of apparently cancer-prone kindreds, given limited historical information about tumor expression. However, cancer surveillance measures can and should be instituted early and predicated on clues that suggest, but need not be diagnostic of, such a hereditary cancer predisposing disorder.
A comparison of two of the indirect methods used for localization of the site of urinary tract infection was made for 218 obstetric patients. There was no correlation between urinary beta glucuronidase activity and the presence of a positive fluorescent antibody test. The ranges of beta glucuronidase activity were so variable that this test could not differentiate between the presence of renal, bladder, or absence of infection.
