[Prostaglandins PGF2alpha in the fight against bleeding and hemorrhage the uterus during the climacteric period].

This article is about usage prostaglandins to fight against bleeding and haemorrhage from uterus in climacteric period. Prostaglandins are used widely by gynecologist and obstetrics as most powerful contracting drugs, namely in this situations when we observe returning, plentiful bleeding from uterus. Women with climacteric bleeding had administer prostaglandins intravenous or immediate intra-cervix. They are very good drug, strongly and a long time hold up bleeding. This method give us time to better preparing patients to operation and diminish losses of blood. Side effects are small and not trouble. Prostaglandins are very simply in usage and characterise very good haemostatic effect, recognised faster then in typical methods like hormonal, mechanical antifibrinolytic etc.

[Clinical study on the contraceptive effectiveness of trisiston from the Jenapharm firm].

Now triphasic contraceptive preparations are used widely. Some news low dose formulations are available on the market, among them Trisiston. The study was conducted to confirm clinically acceptance and effectiveness of Trisiston. There were 437 volunteers mostly multiparous, with a mean age of +/- 25 years who participated in a multi center study. They were taking pills for a 3 to 12 cycles. Cumulatively 4026 cycles were observed. The investigation was conducted according special protocol. At the admission a complete medical history was obtained and a general physical and gynecological examination was performed. All subject were controlled every 3rd month included laboratory tests (blood levels of Hb, WBC, RBC, ESR, carbohydrate metabolism, clotting factors, bilirubin, creatinine, protein, glucose, albumin, LDL, HDL). There was no case of pregnancy. Pearl index = 0. The Trisiston was well tolerated by most of women (80%). Some effects like headache, vertigo, mastalgia, spotting and breakthrough bleeding, loss of libido, premenstrual syndrome were observed in 20% patients, soma of our patients (20%) discontinued contraceptive.

[Our method of treating Asherman's syndrome].

The paper discusses the problem of treating Asherman's syndrome, which due to common procedures in gynaecology and obstetrics becomes more and more frequent occurrence in gynaecological practice. The genesis of this syndrome is often a result of a too intensive excochleation of the uterus, accompanying infection and lack of endometrial reaction to oestrogens. The authors discussed 14 cases of patients in whom, after excochleation of the uterus, there appeared intrauterine adhesions followed by amenorrhoea. The therapy consisted in removing the adhesions by means of a hysteroscope and inserting an intrauterine device the shape of which was selected depending on the character and localization of adhesions and applying oestrogens locally into the uterus cavity. In order to have the menstrual cycle reconstructed, the patients were administered orally hormones during 4 successive artificially reconstructed cycles. Among the patients undergoing therapy, 86% of them began to menstruate again and 57% of them got pregnant. The use of hysteroscopy reduced maximally the risk of breaking the continuity of the uterus during the resection of large adhesions. Inserting a proper device prevented the recurrence of adhesions and local administering oestrogens into the uterine cavity provided good conditions for the reconstruction of the endometrium.

[Current views on the treatment of endometriosis].

The authors present modern views on the therapy of endometriosis, paying much attention to pathophysiological aspects. They present both conventional methods, as hormonal therapy or surgery, and non-conventional methods as laser laparoscopy, microsurgery and fertilization in vitro. The authors stress the lack of casual treatment of endometriosis, which does not result in full effectiveness of the therapy and sometimes there is recurrence of the disease.

[Immunologic aspects of endometriosis].

The paper presents modern views on immunological conditioning of endometriosis. It seems that lowered cellular response, intensified humoral response and increased activation of peritoneal macrophages play an important role in the pathogenesis of endometriosis. Immunological research may lead to pharmacological causal treatment of endometriosis considering abnormal function of the immunological system. The paper also discusses the importance of the first biochemical marker of endometriosis--antigen CA-125--in the diagnostics and monitoring the disease.

[Coexistence of multifocal bowen disease of the vulva and cervix carcinoma].

The authors described a case of a young woman with vulva carcinoma with features of multifocal pigmentary Bowen disease coexisting with atypical flat condylomas in the uterine cervix with focuses of carcinoma in situ. Both within the vulva and the uterine cervix there were histological features of carcinoma in situ, virological examination revealed the presence of virus HPV 16 genome. Sparing vulvectomy, the removal of pigmentary changes and electrocoagulation were performed. The authors discussed this rare case and presented therapeutic procedures.

[Analysis of the correlation between the dose-response curve for prostaglandin F2alpha and the progress of pregnancy, cervix ripening, pharmacologic estrogen premedication and Partusisten administration in prostaglandin-induced labor].

Correlation between sensibility of PGF2 alpha (dose-response) curve and parameters of inductibility - duration of gestation, cervix score, application of estrogens and Partusisten were investigated. 90 patients in high risk pregnancy were observed. It was stated that the sensibility toward of PGF2 alpha is reciprocal proportional to the age of pregnancy and cervix score and proportional direct to pretreatment of Partusisten. Estrogens don't change the sensibility of the uterus to the PGF2 alpha.
