A report on radiation-induced gliomas.

Radiation-induced gliomas are uncommon, with only 73 cases on record to date. The disease that most frequently occasioned radiation therapy has been acute lymphoblastic leukemia (ALL). Three more cases are added here, two after irradiation for ALL and one after irradiation for tinea capitis. In a review of the relevant literature, the authors stress the possibility that the ALL-glioma and the retinoblastoma-glioma links point to syndromes in their own right that may occur without radiation therapy.

[Study of the APUD cells of the gastric mucosa].

The results of an histochemical and microscopic study of the APUD cells carried on gastric biopsies are reported. Endocrine cell hyperplasia, microscopically detected, was particularly evident in atrophic gastritis (type A chronic gastritis) and adenomas. The presence of the APUD cells confirmed by electronmicroscopy, which revealed granules of 200 NM with a central nucleus and a membrane-separating electron-lucent ring, gives a further contribution to the hypothesis of a possible association between endocrine cell hyperplasia and intestinal metaplasia.

Proper bandaging is crucial to effective wound healing. Many factors, including oily skin, body contours, and hair must be addressed to insure proficient bandaging and wound healing.

[Histo-epidemiological observations on advanced gastric carcinoma].

480 cases of advanced gastric carcinoma are classified according to Laurén or WHO and correlated to age and sex of the patients. The modal value of the incidence for age of the mucinous histologic type is 61-65 years for the men and 71-75 years for the women, while in the signet ring cell histologic type is 50 years in both sexes. These relationship, obtained only by the WHO classification, suggest that mucinous and signet ring cell type constitute distinct histoepidemiological entities.

[Epithelial neoplasms of Vater's ampulla: anatomico-clinical and immunohistochemical study].

3 adenomas and 5 carcinomas of the ampulla of Vater are studied to evaluate their histogenesis and diagnostic significance of the antigen defined by the monoclonal antibody CA 19/9. In the adenomas, some dysplastic glands show a faint positivity for CA 19/9 antigen, whereas in the ampullomas this antigen is present in adenomatous residue and in well differentiated adenocarcinomatous areas. The present observations indicate that the endoscopical and macro-microscopical examinations are fundamental to evaluate the location of the ampullary neoplasia. Moreover, the immunohistochemical finding of the CA 19/9 antigen in adenomas and carcinomas suggests their histogenetic relationship, but it is not useful in their differential diagnosis.

Pneumocephalus and tension pneumocephalus after posterior fossa surgery in the sitting position: a prospective study.

Thirty patients subjected to posterior fossa surgery in the sitting position were evaluated postoperatively by CT scans in order to assess the true incidence of pneumocephalus and subsequent development, if any, of tension pneumocephalus. This prospective survey shows that the postoperative incidence of intracranial air was 100%, while no progression towards tension pneumocephalus producing severe neurological deterioration was observed. It can thus be claimed that, although the sitting position may favour the development of tension pneumocephalus also other factors contribute to its pathogenesis. Finally, neither nitrous oxide nor temperature play any significant role in the development of this complication.

Fibrinogen/fibrin deposits in mesenteric lymphadenitis due to Yersinia pseudotuberculosis type I: morphologic, immunohistochemical and electron microscopic studies of one case.

A case of mesenteric lymphadenitis due to Yersinia pseudotuberculosis type I is described in a young adult. The diagnosis is suggested by histological appearance and confirmed by serological tests. Histological and semithin sections reveal granulomas with central microabscesses and concentric coagulative necrosis. By histochemical and immunohistochemical techniques, fibrinogen/fibrin deposits are observed as a prominent intercellular meshwork around microabscesses or as loosely intertwining strands in small granulomas. Electron microscopy shows fibrinogen/fibrin deposits closely associated both fibrous long-spacing (FLS) fibers and thin collagen fibers. The pathogenesis and type of granuloma as well as the significance of fibrinogen/fibrin deposits are discussed with reference to current literature.

Circulating immunocomplexes and laryngeal carcinoma.

Serum levels of circulating immunocomplexes (CIC) were studied in a group of 37 patients with laryngeal carcinoma using two polyethylene glycol-precipitation methods. The preoperative values of this group showed higher levels of CIC when compared with 140 normal controls. No correlation was noted between tumoral stage, oncotype, and serum levels of CIC. Previous studies have shown that the status of several different types of human tumors can be monitored by serial determinations of levels of CIC. We believe that this technique can be used to evaluate the efficacy of therapy and to detect the recurrence of laryngeal carcinoma.

Retinal toxicity after high-dose cisplatin therapy.

Because of increasing complaints of visual dysfunction, 13 patients with refractory or recently diagnosed ovarian carcinoma were evaluated for possible cisplatin-induced ophthalmologic toxicity. All patients had received high-dose cisplatin (200 mg/m2 in five divided daily doses) over two to four cycles. Eight patients (62%) developed symptoms of blurred vision and three (23%) also developed altered color perception. Retinal toxicity in the form of cone dysfunction was documented by electroretinography and color vision testing in 11 patients. Three patients were studied prospectively. Two patients who developed cone dysfunction had normal ophthalmologic exams before the initiation of chemotherapy or after one cycle of cisplatin, suggesting a causal relationship between cisplatin therapy and subsequent retinal abnormalities. Though visual acuity improved off therapy, color vision abnormalities persisted as long as 16 months beyond therapy.

[Necrotizing enteritis due to hypersensitivity reaction (similar to Arthus phenomenon)].

Necrotizing enteritis is a clinico-pathological entity, marked by segmentary necrosis of jejunum and ileum, variously extended, and is a rather rare picture, the etiopathogenesis of which is prevailingly supported by troubles in the intestinal circulation or by intestinal locations of reactions of hypersensitivity (Arthus-similar allergic reaction). The authors start from a case they had the opportunity to observe and successfully surgically treated, and discuss its etiopathogenetic and therapeutical implications in the light of an up-to-date reviewing of the literature thereabout.

[A case of vascular leiomyosarcoma of the superficial soft tissues].

The authors report a case of vascular leiomyosarcoma of the superficial soft tissues, and point out its epidemiologic (rareness of the report), biologic (high degree of malignancy, trend to local relapse and remote conversion into metastases), histopathologic aspects. They consider significant as to prognostic value the appraisal of the "mitotic index" and the opportunity of a radical intervention, followed by radiotherapy. They, moreover, think it useful, in consideration of the frequency of the relapses, to perform a periodic checking of the patient.
