The effectiveness of group psychotherapy during the first three weeks of hospitalization. A controlled study.

Insight-oriented group psychotherapy is a common modality of treatment on inpatient psychiatric wards, yet is effectiveness for acutely hospitalized patients has not been adequately studied. A comparison was made of the effects of three experimental conditions (insight-oriented group psychotherapy, activity-oriented task group, and unstructured control condition) on 86 acutely hospitalized psychiatric patients at a United States Air Force teaching hospital. The group psychotherapy and task group patients did not show greater improvement after 20 days of hospitalization than control patients. In fact, significantly more psychotic patients scored worse in the group psychotherapy condition. It is suggested that insight-oriented group psychotherapy may not be an effective treatment modality during the first 3 weeks of psychiatric hospitalization, especially for psychotic patients.

Experimental syringomyelia: the relationship between intraventricular and intrasyrinx pressures.

The influence of ventricular pressure changes on syrinx pressure was investigated in this study of experimental canine kaolin-induced syringomyelia. The pressures of the ventricles, syrinx, and cervical subarachnoid space were measured. A complete ventriculosubarachnoid block occurred in the animals with syringomyelia. The baseline syringeal pressures exceeded those of both the ventricles and the subarachnoid space. Raising ventricular pressure elevated the syringeal pressure, but aspiration of ventricular fluid did not acutely lower the pressure. These findings suggest a ventriculosyrinx valve effect that may inflate the syrinx during transient rises of intracranial pressure. A respiratory pressure pattern was found in the syrinx similar to that of the ventricles and subarachnoid space. This wave was reduced but not abolished by ligating the subarachnoid space distal to the syrinx. The arterial pulse was much diminished within the syrinx at rest. These findings indirectly support the possibility that transmission of thoracic pressures to the spinal subarachnoid space with compression of the syrinx is a principal force that enlarges the syrinx.

Antiviral drops: comparative therapy of experimental herpes simplex keratouveitis.

The present, masked, controlled study compares the therapeutic efficacy of guttate 3% vidarabine, 0.1% idoxuridine, and 1% trifluridine in herpes simplex keratouveitis in rabbits. Vidarabine, whether given every hour, every two hours, or every three hours was significantly better than controls but significantly worse than idoxuridine or trifluridine. Trifluridine and idoxuridine were not significantly different from each other. Viral cultures taken at the end of the six-day treatment period were positive in the majority of all treatment groups except for the trifluridine group, which was 100% negative. Although previous studies have shown that vidarabine is very effective as an antiviral ointment, vidarabine drops are not as effective as guttate idoxuridine or trifluridine.

Left-handers' smiles: asymmetries in the projection of a posed expression.

Asymmetries in the expression of a posed smile and in a relaxed facial expression were observed in 24 left-handers. Neither writing position nor familial sinistrality predicted the variance of the results. Left-handers were found to smile more with their left than right side of face; an asymmetry which had previously been observed in right-handers. If anything, left-handers' smiles were more asymmetric, though in the same direction, than right-handers. When relaxed, however, the left-handers' face was judged more unhappy on its right than left side--a reversal of the direction of asymmetry previously noted in right-handers' relaxed expressions. No single neurological or psychological theory accounts for these results; it is suggested that hand preference may exert a myotonic effect which is reflected in judgements of relaxed facial expressions.

Glycine and experimental spinal spasticity.

Spasticity may result in part from segmental spinal disinhibition. We determined the content and specific activity of glycine (the putative neurotransmitter thought to mediate spinal postsynaptic inhibition) and serine (the probable precursor of glycine) in feline spastic spinal cord following the intra-aortic administration of two labelled precursors of glycine--14C-D-glucose and 14C-L-serine. The specific activities of both glycine and serine were significantly reduced in the ventromedial, central, and dorsal spinal gray matter in spastic animals. Glycine content remained at control values but serine content increased in spastic spinal cord. This study suggests that glycine turnover decreases in spasticity, owing to its diminished release, and supports neurophysiologic evidence of a decrement in postsynaptic inhibition.

Scoliosis and hydrocephalus in myelocele patients. The effects of ventricular shunting.

Developmental scoliosis is a common cause of increasing disability and deformity in long-term myelocele survivors, and is believed to result from a paralytic collapsing spine. The possible etiological role of compensated hydrocephalus and hydromyelia was assessed by determining the effect of ventricular shunting on 11 myelocele patients with developmental scoliosis. After successful shunting, one patient with a 47 degrees curve continued to deteriorate. Three cases with curves greater than 60 degrees were stabilized for short periods, but eventually required spinal fusion. Seven cases with curves less than 55 degrees were improved from a mean scoliosis of 29 degrees to 13 degrees during a 20-month follow-up period. Several patients had pre-existing shunts that were found to be non-functional on shuntogram. These findings suggest that the spinal complications of hydrocephalus may be more common than previously recognized in myelocele patients and that advanced developmental scoliosis may be avoided by early recognition and ventricular shunting.

IgG2a-producing variants of an IgG2b-producing mouse myeloma cell line.

12 variant cell lines producing an IgG2a (kappa) immunoglobulin derived via different routes from the IgG2b (kappa) synthesizing MPC 11 were studied. These variants all have the parental MPC 11 idiotype as shown by a radioimmunoassay. A comparison of the variants by charge, peptide maps, and assembly patterns has shown that most of them differ from one another, and some can be grouped. One group consists of three primary variants generated with two mutagenic agents: these three have almost indistinguishable peptide maps. Two other primary variants which arose in a similar fashion differ markedly from each other and from this group. A second group is comprised of the four secondary variants which arose from two short heavy chain producing primary variants. Other secondary variants and the one spontaneously arising variant cannot be grouped. Possible genetic mechanisms such as translocation, expression of previously silent genes and recombination are discussed.

Acyclic antimetabolite therapy of experimental herpes simplex keratitis.

In a masked controlled study we compared 3% acycloguanosine, 0.5% idoxuridine, and 3% vidarabine ointments in therapy of experimental herpes simplex virus keratitis in rabbits. The results of the acycloguanosine group were significantly better than the control groups and both other treatment groups, while producing none of the toxic side effects of increasing iritis, conjunctivitis or stromal keratitis, with continued drug application.

Rates of aggregation, loss of anchorage dependence, and tumorigenicity of cultured cells.

The net rate of spontaneous aggregation of cells suspended with EDTA was measured for various cell types including spontaneous transformants and cells transformed with DNA and RNA viruses. The anchorage dependence as determined by growth in methyl cellulose and the tumorigenicity in vivo were also determined. All cells that had lost their anchorage dependency and were tumorigenic showed a high net rate of spontaneous adhesion. A31 was the only nontransformed cell line to have a high net rate of adhesion. The net rate of spontaneous aggregation of cells is a quick and reliable index of tumorigenicity and offers a new approach to understanding the mechanisms of cell surface changes associated with transformation.

Mechanisms of resistance to Corynebacterium kutscheri in mice.

Involvement of the mononuclear phagocyte system in the mechanism of resistance to Corynebacterium kutscheri was studied in C57B1/6 and Swiss Lynch mice. A major difference between the two mouse strains was that the phagocytic cells of the livers of C57B1/6 mice destroyed the bacteria with much greater efficiency. There was no evidence of serum factors which might have led to this greater bacterial killing observed in the C57B1/6 mice, and in vivo phagocytosis of C. kutscheri was identical in both mouse strains. Resistance to C. kutscheri could be transferred with spleen and/or bone marrow cells from resistant (Swiss Lynch X C57B1/6) F1 mice to 650-rad irradiated susceptible Swiss Lynch mice. Nonspecific and acquired mechanisms of resistance to C. kutscheri are discussed.
