The Sézary syndrome. A case with fetal-type glycogen and lymphocytic lymphoma.

A patient had Sézary syndrome and lymphocytic lymphoma. Cytoplasmic inclusions within the Sézary cells were demonstrated by histochemistry and electron microscopy to be composed of fetal-type glycogen. Results of tests for the presence of virus were negative. It is suggested that the Sézary cell represents a reversion of metabolic systems to more youthful types. Previously reported differences in diastase reactivity of the PAS-positive cytoplasmic inclusions are probably based on whether the glycogen is in fetal or mature configuration.

Polymorphism of red cell enzymes in Alaskan ethnic groups.

1. Gene frequencies of esterase D, glyoxalase I and uridine monophosphate kinase in Alaskan populations were determined. 2. Improved methods for demonstrating phenotypes of glyoxalase I and uridine monophosphate kinase are presented. 3. Rare variants of adenylate kinase (AK1) and diaphorase (DIA) were found. One of the AK1 variants is new. 4. Gene frequencies were notably diverse within major ethnic groups. This variability was consistent with a population structure composed of small groups that were relatively isolated from one another.

Purification and substrate specificity of polymorphic forms of esterase D from human erythrocytes.

Esterase D (EsD), purified from human erythrocytes and tested with a variety of substrates, hydrolyzed only triacetin, tributyrin, and certain soluble aryl esters of aliphatic acids. Esters of 4-methylumbelliferone were easily the best substrates. When the three genetically different isozymes were compared, the less common forms, EsD 2 and EsD 2-1, were less stable than EsD 1. With some substrates, the Michaelis constant of the EsD 2 form differed from that of the EsD 1 form. The EsD 2-1 hybrid form was usually, but not invariably, intermediate in properties. The physiologic significance of the genetic variability of this enzyme is unknown.

Investigation of arterial bloodgases at altitude using constant-flow oxygen masks.

Arterial blood oxygen tensions up to 6700 m altitude (FL220) were measured polarographically while subjects breathed from various masks with constant-flow oxygen. The Sierra and Aro masks used for emergency decompression descent in commercial passenger aircraft, gave mean PaO2's of 130 +/- 7.6 and 130 +/- 12.1 torr at 6700 m (FL220) and 90 +/- 3.8 and 77 +/- 3.35 torr at 4260 m (FL140), respectively, when supplied with oxygen flows corresponding to those available in the Boeing 747. These oxygen tensions during descent are acceptable for normal physiological function in a heterogeneous population of air travellers whereas breathing ambient air during return to base at 4260 m (FL 140) (PaO2 of 48 or less) is not acceptable. The valveless Hudson 1007 and Puritan 114011 masks, used for air ambulance service, gave mean PaO2's of 110 +/- 2.7 and 98 +/- 4.5 torr at 6700 m and 80 +/- 3.0 and 77 +/- 2.5 torr at 4260 m under the same condition--significantly less than the Sierra mask.

Effect of preoperative scrub on the bacterial flora of the endocervix and vagina.

A study was designed to evaluate the effectiveness of a preoperative vaginal and perineal scrub in reducing bacteria. An iodophore soap followed by an iodophore solution was used for the scrub. Cultures were taken from the vagina and endocervix of 50 patients before and after the scrub, and cultured for aerobes, anaerobes, and mycoplasma. An average of 5.62 bacterial species per patient was cultured from the vagina before the scrub and 3.9 from the endocervix. The corresponding numbers after the scrub were 0.08 species per patient from the vagina and 1.84 from the endocervix. The reduction of organisms from the vagina was unrelated to the reduction in the endocervix. The scrub was by far more effective in the vagina than in the endocervix. The vagina was bacteria free in 92% of cases while the endocervix was bacteria free in only 8% of cases. No organisms could be recovered from the cervix of 8 patients who had removal of the endocervix by hot conization followed by the iodophore scrub. There was an average of 4.25 species per patient from the 8 cultures of the endocervix prior to the cone and scrub.

Scleromyxedema.

An unusual, nodulocystic form of scleromyxedema (lichen myxedematosus) developed in a 48-year-old man with a six-year history of psoriasis. The scleromyxedema responded to intermittent therapy with melphalan and prednisone. Dermabrasion smoothed and softened the skin and increased the mobility of the perioral skin. Two months after remission of the skin lesions, psoriasis recurred.

Allergic contact dermatitis from TEA-Coco hydrolyzed protein.

A 21-year-old woman developed a severe dermatitis of the face after using a proprietary skin cleanser. Patch testing showed delayed hypersensitivity to TEA-Coco hydrolyzed protein (the triethanolamine salt of the condensation product of coconut fatty acids with a complex of polypeptides and amino acids derived from collagen), but not to other ingredients of the cleanser. Further patch testing revealed positive results with other condensates of fatty acids and protein hydrolysates.

The discrimination of phenotypes for rate of disappearance of isonicotinoyl hydrazide from serum.

Expression of the rate of inactivation of isonicotinoyl hydrazide (INH) as a first-order constant gave better discrimination of slow and rapid inactivators than did the level of INH at the end of 6 hr. The most probable division point between the two classes was at k = 0.130. Rate of inactivation by obligatory heterozygotes was determined. Such heterozygotes could not be adequately differentiated from homozygous rapid inactivators. The gene frequency for slow inactivation was 0.46 +/-0.04 in Alaskan Eskimos and 0.62+/-0.06 in Athabaskan Indians.
