Platelet activating factor-induced ischemic bowel necrosis: the effect of PAF antagonists.

We have reported a model of ischemic bowel necrosis produced in the rat by injecting platelet activating factor (PAF) or PAF with bacterial endotoxin (LPS) into the mesenteric vasculature. In the present study, we examined the protecting effects of three PAF antagonists, i.e. (R,S)-3-[2-[(2-octadecylaminocarbonyloxymethyltetrahydro-2-fura nylmethoxy) -hydroxyphosphinyloxy]-ethyl]-thiazolium hydroxide inner salt 4-oxide (SRI 63-072), (+/-)-3-[4-[3-octadecylaminocarbonyloxy-2-methoxy)-propoxy]-butyl] -thiazolium bromide (SRI 63-119) and 1-O-hexadecyl-2RS-O-ethyl-3-O -(7-thiazolinoheptyl)-glycerol-methanesulfonate (ONO-6240), on PAF-induced bowel necrosis. The antagonists were injected into the vein 10 min before PAF. Two microgram of PAF or 20 micrograms LPS with 1 microgram of PAF were injected into the aorta above the renal arteries. The parameters assessed included blood pressure, hematocrit, white blood cell count, extent of bowel perfusion and microscopic changes of the bowel. We found that SRI 63-072 (3 mg/kg), SRI 63-119 (3 mg/kg) and ONO-6240 (2 mg/kg) significantly improved the initial hypotension, hemoconcentration and leukopenia caused by PAF. All three drugs also corrected the sustained hypotension and hypoperfusion and gross lesions of the bowel, although microscopic examination still revealed focal mild lesions. SRI 63-072 was also active at a much lower dose (0.3 mg/kg).

The effect of post-sintering heat treatments on the tensile properties of Ti-6A1-4V alloy.

Previous studies have shown that the application of a porous coating to a solid substrate offers several advantages over current methods of implant fixation. However, the heat treatments required to sinter porous metal coatings have also been shown to cause significant decreases in the mechanical properties of the substrate. With Ti-6A1-4V alloy, sintering above the material beta transus results in a transformation from the as-received, equiaxed microstructure, recommended for surgical implants, to a lamellar alpha-beta microstructure. This lamellar structure has been shown to have inferior mechanical properties. In the present study, microstructural analysis and mechanical testing were performed on Ti-6A1-4V alloy subjected to various post-sintering heat treatments in an attempt to improve the mechanical properties. The microstructures examined were a fine and a coarse acicular alpha in a retained beta matrix. Tensile tests were performed on specimens containing these structures and results were compared with the lamellar and equiaxed microstructures. The fine acicular alpha structure was shown to exhibit the best tensile properties for the post-sintering Ti-6A1-4V alloy microstructures examined, displaying a 9.8% elongation value, as compared to the as-received, equiaxed microstructure value of 13.5%. This represents a significant improvement over the 5.1% value obtained with the lamellar microstructure.

Congenital hybrid basal cell adenoma--adenoid cystic carcinoma of the salivary gland.

A congenital parotid gland tumor involving the facial nerve was excised, revealing histologic and ultrastructural features of a basal cell adenoma and adenoid cystic carcinoma. An asynchronous regional metastasis was successfully eradicated with adriamycin. Seven other similar appearing tumors occurring in neonates are documented in the literature and serve to establish this tumor as a clinicopathologic entity. The surrounding parotid gland had "dysplastic" changes attributed to obstruction during morphogenesis.

Histologic and microradiographic analysis of a revised porous-coated anatomic (PCA) patellar component. A case report.

A retrieved uncemented, porous-coated human patellar component was evaluated histologically and microradiographically. The metal portion of the component and porous coating were made of cobalt--chromium-based alloy. Because of adhesive capsulitis and inadequate knee flexion, the patient had revision surgery 11 months after the original total knee arthroplasty. Clinically, the device was not grossly loose. Contact microradiography of the porous material showed no calcified tissue ingrowth. Undecalcified histologic sections demonstrated only fibrous tissue ingrowth, with obvious bone-resorptive activity as well as numerous multinucleated giant cells adjacent to the porous metal.

The effect of surface treatments on the interface mechanics of LTI pyrolytic carbon implants.

A mechanical and histological evaluation of LTI pyrolytic carbon implants was undertaken to determine the effect of various surface treatments on the retention characteristics of the implants. Five types of surfaces were evaluated, including as-deposited, fine grit-blasted, coarse grit-blasted, ground, and plasma oxygenated. The four surface treatments were chosen in an attempt to emulate the morphology of the as-deposited implants. The implants were evaluated in vivo by placement transcortically in the femora of adult mongrel dogs for periods of 12 to 24 weeks. Although the as-deposited implants exhibited the greatest interface strength at 12 weeks the results of mechanical testing after 24 weeks implantation indicated no statistically significant difference among the interface strength values or among the interface stiffness values of the implants. The histologic response of the implants was similar; while all implants exhibited areas of direct implant-bone apposition, the as-deposited implants exhibited this behavior to the greatest extent. Thus the ability to duplicate the biological response to the as-deposited LTI carbon surface appears possible by one or more of the treatments evaluated.

Toxic compensation bills.

Congress has demonstrated interest in toxic compensation legislation, but not enough agreement to make significant progress. Advocates of reform claim that the legal system is heavily weighed against victims who seek compensation through the courts. Proposed reforms include a compensation fund and a cause of action in federal court. Critics have questioned whether these changes in the law would represent an improvement. Existing income replacement, medical cost reimbursement, and survivor insurance programs largely cover the losses of individuals with chronic disease. Thus, the need for an additional compensation is not clear. Furthermore, experience with compensation funds such as the Black Lung Fund suggests that political rather than scientific criteria may be used to determine eligibility. Finally, under the proposed financing mechanisms the compensation funds that are being debated would not increase incentives for care in the handling of hazardous wastes or toxic substances.

Centrarchid fish as paratenic hosts of the giant kidney worm, Dioctophyma renale (Goeze, 1782), in Ontario, Canada.

Infective larvae of Dioctophyma renale were found in the hypaxial musculature of pumpkinseed (Lepomis gibbosus L.) from three lakes in Algonquin Provincial Park, Ontario, Canada. This represents the first report of D. renale in centrarchid fish. In the three lakes surveyed prevalence and mean intensity ranged from 5 to 23% and one to two larvae respectively. Larvae elicited a mild granulomatous reaction in pumpkinseed. Two ferrets were each given five larvae from pumpkinseed. Adult D. renale were recovered from the right kidney capsule of ferrets 108 and 134 days post-infection. An opening in the ventral surface of the right kidney capsule was present in one ferret. Chronic peritonitis was associated with eggs of D. renale and cellular debris which probably entered the abdominal cavity from the right kidney capsule.

Tissue reaction to a failed Bion hip prosthesis in a dog.

The tissue reaction in a dog to a failed constrained total hip replacement system with a Bion elastomer component was evaluated. The device consisted of Ti-6A1-4V alloy acetabular and femoral components connected with a Bion component. At necropsy the Bion component was found to have fractured. The tissue surrounding the Bion was granulomatous with numerous blackened regions. Histologically a severe chronic inflammatory response was present in the surrounding tissue with numerous giant cells and Bion laden macrophages.

Retrieval and analysis of intramedullary rods. A follow-up study.

From 1980 to 1983, 16 AO intramedullary femoral rods were retrieved and analyzed clinically, radiographically, and metallurgically. Cracking and/or fracture was observed in four of the 16 specimens. All cracks occurred at the point of maximum stress at the end of the slot. In two cases a structurally weaker weld zone coincided with this location. The weld zone and slot were also found to coincide in five of the uncracked rods. Two of the four cracked rods were removed because of pain, while only three of the 12 uncracked rods caused pain. Structural and material characteristics (i.e., the location of the weld at a point of maximum stress and significant variability in microstructure), rather than surgical technique or time in situ, were found to be responsible for the implant mechanical failures. Improvements should be made in fabrication techniques and material properties. For the present, careful assessment of a painful intramedullary rod and routine removal after healing are advisable.

Characteristics of natural infections of the stomach worm, Obeliscoides cuniculi (Graybill), in lagomorphs and woodchucks in Canada.

Wild lagomorphs and woodchucks collected predominantly in southern Ontario, Canada were examined for subspecies of Obeliscoides cuniculi (Graybill). Obeliscoides cuniculi multistriatus was found in snowshoe hares (Lepus americanus). Obeliscoides cuniculi cuniculi was found in cottontail rabbits (Sylvilagus floridanus), European hares (Lepus capensis) and woodchucks (Marmota monax). Prevalence of Obeliscoides cuniculi multistriatus in snowshoe hares was 100% and mean intensity (and range of intensity) was 760 (9-4, 198) in Lindsay, Ontario in 1980. Mean intensity in hares varied trimonthly. The highest mean intensity of worms occurred in spring when most worms were adult. Transmission occurred mainly in spring. Most worms present in fall (70%) and winter (54%) were fourth stage. Immature fifth-stage and gravid females were present in hares during fall and winter. Prevalence and mean intensity of O. c. cuniculi in cottontails was 15% and 29 (1-118). Prevalence and mean intensity of O. c. cuniculi in woodchucks was 6% and 56 (16-118). European hares were infected with O. c. cuniculi, prevalence was 10% and mean intensity was 60 (36-83). In Ontario woodchucks and European hares were common in areas frequented by cottontail rabbits and probably acquired sporadic infections of O. c. cuniculi from infected cottontails.

Development of the stomach worm, Obeliscoides cuniculi (Graybill), in lagomorphs, woodchucks and small rodents.

The parasitic development of Obeliscoides cuniculi multistriatus Measures and Anderson, 1983 was studied in New Zealand white rabbits (Oryctolagus cuniculus). Third-stage larvae exsheathed within 24 hr. The third molt occurred 3 days and the fourth 8-11 days post-infection. All worms were fifth stage 14 days post-infection. Males were mature at 16 days and copulation occurred in 15-16 days. Females were gravid at 18 days. The prepatent period was 16-22 days. The patent period was 61-118 days and males lived longer than females. All stages were found in the mucosa except the fifth which was found lying on the mucosal surface within a layer of mucus. Petechiae were the only lesions seen in experimentally infected rabbits. Patent infections of O. c. multistriatus were established experimentally in infected woodchucks (Marmota monax), snowshoe hares (Lepus americanus) and cottontail rabbits (Sylvilagus floridanus). Patent infections of O. c. cuniculi were established experimentally in snowshoe hares. Patent infections of O. c. multistriatus did not appear in experimentally infected HPB white (Swiss Webster) mice (Mus musculus). Wistar rats (Rattus norvegicus), H.O.R.F1 Syrian hamsters (Mesocricetus auratus), H.O.R.F1 smooth-haired guinea pigs (Cavia porcellus) and H.O.R.F1 gerbils (Meriones unguiculatus).
