A review of the research on general health status indexes.

During the mid-1960's, recognition of the spiraling cost of health care motivated Congress to enact several major pieces of legislation designed to underwrite efforts to improve the delivery of health services. The increased level of federal fiscal participation in the health service system has forced greater consciousness of the need for better accountability of the effectiveness and efficiency of the allocation of the dollar. Demands were articulated for precision tools which could evaluate the imput to output linkage between need and response; however, the tools were nascent and had limited applicability. Cost-effectiveness and cost-benefit analysis are two procedures that have since been applied with varying levels of success. A third tool, also developed as an outgrowth of the desire for a more accurate characterization of the planning concerns of an efficient/effective health service system, is the general health status index. The purpose of this article is to provide the reader with a comprehensive review of the literature on general health status indexes. Common objectives and constraints are presented, as well as a discussion of the expanding role of general health status indexes.

Fatty acid composition of some cellular membranes of fetal rat liver.

The fatty acid composition of lipids of inner mitochondrial membrane, rough and smooth endoplasmic reticulum of adult and fetal rat liver has been determined. Subcellular membranes of fetal liver show a higher content of palmitic acid and oleic acid and a lower content of stearic acid and arachidonic acid as compared to subcellular membranes of the adult liver. The activity of citrate lyase and acetyl-CoA carboxylase of rat liver cytosol has been determined as a function of age. It is concluded that the differences are due to a relative deficiency of the fatty acid elongation system. The higher degree of saturation of the fatty acids of the phospholipids of the fetal membranes may be the cause of altered permeability properties of these membranes, as illustrated by the slower rate of isoosmotic swelling in the presence of the ammonium salt of some of the Krebs cycle intermediates in fetal rat liver mitochondria.

[The incidence of constitutional chromosome aberrations].

The incidence of constitutional chromosomal abnormalities in livebirths has been estimated at 0.62% from 7 surveys pooling 45,183 newborns. The repartition of these abnormalities is reported and compared to the incidence observed in spontaneous abortuses, perinatal deaths and in normal adults.

Concordance of collagen-based radiocarbon and aspartic-acid racemization ages.

By determining the extent of racemization of aspartic acid in a well-dated bone, it is possible to calculate the in situ first-order rate constant for the interconversion of the L and D enantiomers of aspartic acid. Collagen-based radiocarbon-dated bones are shown to be suitable samples for use in "calibrating" the racemization reaction. Once the aspartic-acid racemization reaction has been "calibrated" for a site, the reaction can be used to date other bones from the deposit. Ages deduced by this method are in good agreement with radiocarbon ages. These results provide evidence that the aspartic-acid racemization reaction is an important chronological tool for dating bones either too old or too small for radiocarbon dating. As an example of the potential application of the technique for dating fossil man, a piece of Rhodesian Man from Broken Hill, Zambia, was analyzed and tentatively assigned an age of about 110,000 years.
