[Integral photodynamic therapy of multifocal bladder cancer. Initial clinical experiences].

Experimental results and initial clinical experience indicates that even carcinoma in situ can be destroyed by integral photodynamic therapy of multifocal bladder carcinoma following tumor-selective photosensitization with a hematoporphyrin derivative. The employment of a light-scattering medium in combination with a special bladder catheter and photodynamically effective red laser light (630 nm) guarantees homogeneous irradiation of the bladder, which thus reaches all tumor foci. The complications observed were transitional dysurias and, in a particular case, regressive shrinkage of the bladder.

[Vascular and ocular complications in a child with homocystinuria].

Homocystinuria, only partially responsive to pyridoxine, was first diagnosed at the age of 14 years in a boy with high myopia, spherophakia and subluxated lenses, when surgical removal of a dislocated lens became necessary. 2 years previously a fracture of the left tibia, treated conservatively, had been followed by ischemia of the leg necessitating amputation. Postoperatively, deep vein thrombosis had developed in the right leg. Plasma homocysteine was lowered to undetectable levels by treatment with betaine in addition to pyridoxine, folic acid and reduction of protein intake. During the two years on treatment no further thromboembolic complications have occurred.

Haloperidol. Plasma levels and prolactin response as predictors of clinical improvement in schizophrenia: chemical v radioreceptor plasma level assays.

The relationship between clinical response of schizophrenic patients to haloperidol and (1) blood levels of the medication, determined by both gas-liquid chromatography (GLC) and radioreceptor (RR) assays, or (2) prolactin response to the medication, was examined in an inpatient study using several fixed doses of haloperidol. Regression analysis disclosed a substantial curvilinear relationship between steady-state GLC-determined plasma haloperidol levels and decrease in Brief Psychiatric Rating Scale (BPRS) Psychosis factor scores; however, no substantial relationship was found between clinical response and RR plasma haloperidol levels or serum prolactin response to haloperidol. Our results suggest that steady-state plasma levels of haloperidol determined by the GLC chemical assay are a better predictor of decreases in BPRS Psychosis factor scores than RR assayed plasma haloperidol levels or prolactin response to haloperidol.

Plasma and red cell levels of thioridazine and clinical response in schizophrenia.

The relationship of plasma and red blood cell (RBC) levels of thioridazine to clinical response in schizophrenia was evaluated in a fixed-dose study. Steady-state plasma and RBC levels of thioridazine, mesoridazine, and sum of thioridazine and mesoridazine, determined by gas-liquid chromatography, were not significantly correlated with clinical response as measured by improvement on the Brief Psychiatric Rating Scale. RBC thioridazine levels were not substantially more strongly correlated with clinical response than plasma levels.

Abnormalities of brain structure and density in schizophrenia.

Quantitative and qualitative measures of brain morphology were derived through CT scans using computer-assisted methodology in patients with schizophrenia or schizo-affective psychosis and headache controls. Schizophrenics had significantly higher density of white matter, together with greater right vs. left asymmetry in density of white matter than controls. Schizophrenics tended to have larger widths of cortical sulci than headache patients. In our sample of schizophrenics, however, no significant differences were found on measures of lateral ventricle (LV) width, LV area, VBR, or other measures of ventricular size compared to headache controls. There were no differences between CT scan measures taken in patients with schizophrenia vs. schizo-affective psychosis.

Analysis of the effect of kinetic therapy on intracranial pressure in comatose neurosurgical patients.

The use of oscillating beds in the management of immobilized patients has been reported to minimize some of the complications associated with immobility. In this study, 10 patients admitted with a clinical presentation of raised intracranial pressure (ICP) underwent monitoring of their ICP while receiving kinetic therapy as provided by the Roto-Rest treatment table. An analysis of the possible effect that the constant change in position might have on the ICP was carried out. ICP readings were obtained with the patient in each of three bed positions (extreme left, extreme right, and supine) every half hour. Results from correlational analyses indicated that changes in bed position did not differentially affect ICP. The results of this study suggest that the presence of raised ICP should not limit the use of kinetic therapy in an attempt to minimize the complications of immobility in comatose neurosurgical patients.

Socket design and manufacturing technique for through-knee stumps.

This paper describes a socket design and manufacturing technique for through-knee stumps, based on 14 years experience with 290 prostheses. The bulbous shape of the stump and its full end-bearing quality requires a socket which has very little resemblance to above-knee sockets. The requirements of the through-knee socket are listed and the manufacturing technique is described and illustrated in detail. Emphasis is put on the quality of the negative plaster mould. The socket is manufactured according to the double wall technique commonly used in below-knee sockets. It provides a maximum of comfort and cosmesis without extra costs.

Failures in through-knee amputation.

Long-term results over a period of more than 10 years give evidence of the superiority of the through-knee compared to the above-knee stump. However, failures in through-knee stumps before, during and after operation and pitfalls in prosthetic fitting are still too frequent. They are mostly due to errors because the peculiarities of the stump and the prosthetic management are not recognised. This paper emphasizes frequent causes of failure in the selection of the level of amputation, the operative technique, post-operative treatment and prosthetic rehabilitation in order to reduce the complication rate.

Methylmalonic acidaemia due to mutase apoenzyme defect: responsive to vitamin B12 in intact fibroblasts but not in vivo.

Congenital methylmalonic acidaemia (MMA-aemia) was diagnosed in an 8-month-old girl who presented with severe metabolic acidosis, hypoglycaemia and hyperglycinaemia. Vomiting, failure to thrive and apathy first appeared when breast feeding was replaced by a cows' milk formula at the age of 3 months. The patient, unresponsive to OH-Cbl therapy, was successfully treated with dietary protein restriction and with Shohl's solution. Aged 4 years 9 months, she is in good health. Studies in cultured fibroblasts revealed a defect of the MMA-CoA mutase apoenzyme. Mutase activity in cell extracts was barely detectable both with and without added coenzyme (Ado-Cbl). Addition of OH-Cbl to the culture medium improved overall propionate metabolism in intact fibroblasts but had no effect on mutase activity in cell extracts. These observations point to the presence of a very labile mutant enzyme, suggesting that the patient reported here may be suffering from yet another variant of MMA-aemia.
