A comparative study of the respiratory properties of bird blood.

Oxygen affinity and Bohr effect have been measured in the blood of four bird species (chicken, pigeon, Japanese quail and sparrow). Under physiological conditions (41 C, pH 7.5, PCO2 approximately 35 Torr) the oxygen half saturation pressure P50 are 50 Torr for the chickens, 38 Torr for the pigeon, 43 Torr for the Japanese quail and 44 Torr for the sparrow. Values for the physiological Bohr factors deltalogP50/deltapH are similar to those obtained in mammalian blood ranging from -0.57 in chicken blood to -0.42 for pigeon blood. A direct effect of CO2 on the oxygen affinity of chicken blood could not be observed, in agreement with results in hemoglobin solutions. However, at a constant pHpl of 7.5 the intracellular pH rise from 7.22 at 39 Torr PCO2 to 7.32 at PCO2 less than 1 Torr. This leads to a decrease of P50 at pHpl 7.5 from 52 to 46 Torr. A convincing correlation between body weight and P50 could not be established, even when including data from other authors. It is concluded that aside from metabolic rate, behavioural as well as environmental factors have influenced whole blood oxygen affinity during evolution.

Causes of high blood O2 affinity of animals living at high altitude.

We have measured the partial pressure of O2 at 50% saturation (P50) and the concentration of various phosphate compounds in the erythrocytes of the bar-headed goose and the guanaco to establish the cause of the high blood O2 affinity in animals who normally reside at high altitude. The same data were obtained in the blood of two goose species, that live at sea level, and in human blood. At standard conditions (pH 7.4, PCO2 40 Torr, 37 degrees C), P50 was 29.7 Torr in the blood of the bar-headed goose and was about 10 Torr higher in the goose species living at sea level. Since the concentration of organic phosphates was not markedly different in the erythrocytes of either goose species we conclude that the hemoglobin of the bar-headed goose reacts more weakly with organic phosphates, which can also be inferred from studies on purified hemoglobin solutions. Likewise, the low P50 of guanaco blood in comparison with human blood can be explained by a reduced interaction of 2,3-bisphosphoglycerate of guanaco hemoglobin compared to the human pigment.

[An intramural hematoma of the small intestine, during anticoagulant therapy: radiological course. Concerning one case].

The authors present a case of intramural hematoma of the small intestines during anticoagulant treatment. With reference to this case, they study the frequency, etiopathogenesis and anatomy of this hematoma and particularly look at the radiological manifestations. In this respect they distinguish three stages in the evolution. The first, when the straight X-ray of the abdomen and barium followthrough demonstrate an axial stenosis of the small intestines with dilation of the proximal loops; the second (between the 7th and 20th days) when the loop affected by the hematoma takes on a characteristic "palissade" or "spring" -like sausage appearance; finally the third (after the 3rd week), when only thickening of the haustrations persists with progressive return to normal. The radiological diagnosis is discussed, not only with intramural hematomas of the small intestines of other etiologies (traumatic, during pancreatitis, during disorders in hemostatis), but also with conditions giving rise to similar radiological pictures: malabsorption, inflammatory conditions, etc.

[Tracheal suture under jet ventilation in a 7-year-old child (author's transl)].

Anaesthesia in a 7-year-old girl with incomplete dissection of the trachea due to stabbing is reported. After nasotracheal intubation the lesion was isolated. Then transtracheal jet ventilation through the defect was performed. Thus sufficient ventilation as well as a clear surgical field for the intratracheal suture even of the pars membranacea was garanteed. The advantages of this procedure compared with the traditional intubation are discussed.

Insulin kinetic, glucose tolerance, and lipid metabolism in genetically spontaneous-hypertensive rats.

Genetically spontaneous hypertension in rats (SHR) showed a significant rise not only in systolic blood pressure in the early age groups, but also, from the 8th week of life onward, in diastolic blood pressure together with a rise in heart rates, when compared with control rats. Following an i.p. glucose injection a disturbed glucose tolerance, increased insulin level and a reduction in plasma triglycerides and cholesterol occurred. These findings in SHR allow to draw interesting analogous conclusions as to the protodiabetic disturbances in carbohydrate metabolism accompanied by insulin enhancement after glucose and diminished glucose- and increased lipogenetic effects, which had been demonstrated by R. BAUMANN et al. in 58% of all studied cases of juvenile human hypertension (Stage I and Stage II, NITSCHKOFF and R. BAUMANN), in which possibly hereditary factors may be involved.

[The maternal and fetal levels of human chorionic gonadotropin, human placental lactogen and prolactin during the perinatal period (author's transl)].

The levels of human chorionic gonadotropin (HCG), human placental lactogen (human choriosomatomamotropin HCS) and prolactin (PRL) were determined in the serum of 72 maternity patients and the serum of the newborn infants. The determinations were done with radioimmunologic tests (RIA). These three protein hormones were also determined in the amniotic fluid and in the maternal serum from 4-6 days prior to the delivery of the infant. The concentration of HCG or HCS in the serum of the newborn infants was a mean 0.43 or 0.37% of the level in the maternal serum. The concentration of PRL in the serum of the newborn was 118% and slightly higher than in the serum of the mothers. The concentration in the amniotic fluid was 1.5% for HCG, 5.8% for HCS, and 252% for PRL, compared to the corresponding levels in the maternal serum. The fact that the hormone concentrations in the amniotic fluid are significantly higher than in the serum of the newborn suggests excretion of the hormones from the fetal circulation via the fetal liver and the fetal kidney. The high levels of PRL in the maternal and the newborn serum may be caused by the high concentrations of estrogen or progesterone. Increased during the course of the pregnancy there was a significant sex linked difference in the level of HCG in the maternal serum correlated to the sex of the newborn infant.

[A propos of 210 emergency endoscopies in upper digestive hemorrhage].

The authors report their experience of 210 fiber endoscopies carried out as an emergency for upper digestive hemorrhage. They emphasize the necessity of early examination and only consider here examinations carried out within 12 hours from the start of the hemorrhage. In 91.4 % of cases, a correct diagnosis was possible. More than 20% of the patients had at least two lesions liable to cause hemorrhage. In cirrhosis, esophageal varices were the cause of hemorrhage in only 50% of cases.

[Endoscopic resection of colonic polyps. A propos of 45 polypectomies].

Endoscopic resection of polyps of the colon is, no doubt, a considerable therapeutic advance. It avoids surgical operation permits histological examination and makes simple biopsy unnecessary. It is indicated in the presence of pediculated polyps of a diameter or less than 3 cm, whatever the site of the polyp. Thus we were able to remove 45 polyps without complications. The method is not indicated in a large size polyp, sessile polyps or diffuse polyposis. The follow-up is still insufficient to assess : 1) the true risk of resection which can only be undertaken after long experience of colonoscopy ; 2) the frequency or recurrent adenomas. Endoscopic resection is at present the simplest method of detection and treatment of carcinoma in situ of the colon of which we observed four cases in our series. The problem of surgical reoperation is discussed.
