A paradigm for computer-assisted radiology resident rotation scheduling.

RATIONALE AND OBJECTIVES: The scheduling of radiology residents remains a major annual undertaking of chief residents. In this article, we describe a paradigm to implement interactive computer programs to reduce the inefficiencies and inequities of planning the yearly schedule. METHODS: We used the programming language, Prolog, to develop a compact program that provides faster and more flexible performance than those reported in the literature. This interactive program stores scheduling requirements in data files separated from the control program and runs on a Macintosh computer. RESULTS: The schedule of any residency year is generated within 3-7 sec. The fast computation and query capabilities of this scheduling program have helped chief residents to identify conflicting requirements that were previously overlooked. CONCLUSION: Using our programming paradigm, we have developed a portable Prolog-based scheduling program that is quick and easy to use.

Correlation distance measurements of the female breast.

Ultrasonic waves propagating through soft tissue experience wavefront distortion. Adaptive wavefront compensation algorithms attempt to correct such distortion. A valuable design parameter is the isoplanatic patch size of the imaging medium. Its lateral extent is the FWHM of the correlation function. The range extent is defined similarly. The significance of the isoplanatic patch is that a new wavefront correction vector must be obtained whenever the ultrasound beam is moved a patch length. This paper reports measurements of wavefront correlation functions as well as statistics of the lateral correlation distance rho d (half the FWHM) within the female breast obtained from a population of 22 women (44 breasts) and measured at 3 and 4 MHz with a large acoustic aperture (9.6 cm). A set of complex wavefronts radiated from single pointlike sources was measured from the opposing side of the breast. The propagation distance was 12 cm. rho d is the distance between two sources at which the correlation between their wavefronts drops to 0.5. The mean value at 3 MHz was found to be less than 1.5 mm for the premenopausal dense breast, 2.5 mm for the premenopausal fatty breast, and 2.0 mm for the postmenopausal breast. The mean value dropped by a factor of 2 at 4 MHz for a group consisting of premenopausal fatty and postmenopausal breasts.

Why bother with a computerized scheduling system?

Patient scheduling is often included as a module available in modern information management systems. The functionality of a scheduling module can vary greatly. A complete scheduling system combines rules related to each examination room with rules concerning the procedure itself. Duplicate exams can be avoided while picking the most optimal time slot available. The benefits of such a scheduling system include reduction in scheduling delays, a reduction in patient waiting time, improved patient through-put, improved clinical history information, and improved billing accuracy and completeness.

Prospective analysis of the value of scrotal ultrasound.

Ultrasound was evaluated in 69 patients for its usefulness in distinguishing scrotal abnormalities requiring surgery from those that may have clinical follow-up only. Distinction was possible in the majority of cases. On imaging, tumor showed decreased echogenicity within the testicle, which was usually enlarged. Epididymal enlargement, marked increase in peritesticular fluid, and skin thickening suggest a non tumorous condition. The ultrasound characteristics of various scrotal abnormalities are described.
