[Comparative study of the efficacy of hepatic arterial and portal venous infusion of adriamycin for prevention of liver metastases in rabbit VX2].

To compare the efficacy of hepatic arterial (HA) and portal venous infusion (PV) anticancer drugs in the prevention of the liver metastasis of cancer, the following experimental study was performed. VX2 tumor cells (1 x 10(7) cells) floating in physiological saline were injected into the portal vein of Japanese white rabbit. Adriamycin (ADR) was administered to the liver via HA or PV at 3, 5, 7 (1 mg/kg) days, and immediately, 2, 4 days after tumor inoculation, respectively. The rabbits were sacrificed 3 weeks after tumor inoculation and the total number of liver metastases was counted. Results were as follows: 1) The numbers of metastatic VX2 tumor nodules in HA group were significantly smaller than in PV group (p = 0.05). 2) Starting ADR injection immediately after tumor inoculation enhanced the inhibition of tumor growth. These results suggest that immediate hepatic arterial infusion of anticancer drugs after resection of primary gastrointestinal cancer is most useful to prevent liver metastases.

[Is radiofrequency catheter ablation available for treatment of ventricular tachycardias?--Effects of sequential unipolar applications using multiple electrode catheter].

The purpose of this study was to examine the effects of radiofrequency energy delivered to the canine endocardium in sequential unipolar applications using multipolar electrode catheter for treatment of ventricular tachycardias. Radiofrequency energy generated by the newly designed device (500 kHz) was delivered to the ventricular myocardium by a quadripolar electrode catheter. The device could indicate output and voltage and could calculate myocardial impedance. In order to determine the power of the ablation in vivo, sequential unipolar application was performed with various output in vitro. A continuous endocardial ablation was achieved with output beyond 15 W and with duration of 5 sec per one pole. According to our measurement, the myocardial impedance in vivo was about three times as large as that in vitro. Therefore, minimal output of 45 W in vivo was applied in the same manner in vitro, which resulted in continuous belt-shaped endocardial ablated lesion. Ventricular stimuli after sequential unipolar applications did not induce ventricular tachycardia in any of the dogs. We conclude that radiofrequency sequential unipolar application using the newly designed device produces a large ablated area and may be suitable for ablation of the foci of ventricular tachycardias.

Characterization of a new minor lymphocyte stimulatory system. I. Cluster of self antigens recognized by "I-E-reactive" V beta s, V beta 5, V beta 11, and V beta 12 T cell receptors for antigen.

In the mouse, two sets of V beta gene products have been shown to be associated with T cell recognition of endogenous self Ag. One of these is the set of V beta associated with T cell reactivities to stimulatory Mls gene products, Mlsa (V beta 6, V beta 8.1, V beta 9) or Mlsc (V beta 3); another is the set of V beta, such as V beta 5, V beta 11, V beta 12, or V beta 17a, which were originally found to be related to I-E recognition. Although the Mls system has been well characterized, little is known about the nature of the ligands for the second set of V beta. In this work, we describe the evidence that the natural ligand or ligands of V beta 5, V beta 11, and V beta 12 may be novel Mls determinants that are recognized by naive T cells at a high precursor frequency and function as the ligand for clonal deletion of self-reactive T cells by negative selection. However, surprisingly, unlike the conventional Mls system, in which all V beta associated with Mlsa recognition or Mlsc recognition are uniformly deleted in those animals expressing the relevant Mls type, expression of these three V beta segregates independently among strains. Based on these observations, the nature of T cell recognition for this new Mls gene product(s) is discussed.

Characterization of autoantibody-secreting B cells in mice undergoing stimulatory (chronic) graft-versus-host reactions. Identification of a CD44hi population that binds specifically to hyaluronate.

This study was undertaken to identify and isolate the pathophysiologically important B cell subpopulation which is activated to Ig secretion and autoantibody production in stimulatory (chronic) graft-vs-host (GVH) reactions. We recently demonstrated that IL-5 stimulation in vitro induces the appearance of a distinct CD44hi Ialow CD45Rlow B cell subpopulation that has aquired the ability to bind to hyaluronate (HA), one of the ligands for CD44, and that this B cell subpopulation is enriched in both proliferative and Ig-secretory responses. In the present report, CD44 expression was examined in B cells which were activated in the course of stimulatory GVH reactions. Compared with normal mice, B cells from mice undergoing stimulatory GVH reactions contained a novel CD44hi B cell subpopulation which exhibited enhanced binding to HA. The CD44hi HA-adherent B cell subpopulation from GVH mice spontaneously secreted large amounts of Ig, particularly IgG, including autoantibody specific for ssDNA. These findings demonstrate that CD44 expression distinguishes those B cells that are activated in vivo in the course of GVH to proliferate and differentiate into Ig-secreting cells. These CD44hi, HA-adherent, cells may play a prominent role in the hypergammaglobulinemia and immune complex glomerulonephritis that occur during chronic GVH reactions.

Genetic analysis of the Mls system. Formal Mls typing of the commonly used inbred strains.

In order to elucidate the biological role of minor lymphocyte stimulating (Mls) gene products, we have been investigating the fundamental immunogenetic characteristics of the Mls system. In this report, we describe the distribution of stimulatory Mls products, Mlsa and Mlsc, in a panel of laboratory inbred strains based on the response pattern of H-2-compatible naive T-cell populations as well as monospecific Mlsa- or Mlsc-reactive T-cell clones. In addition, the expression of four different T-cell receptor (Tcr) b-V segment Tcrb-V3, -V6, -V8.1, and -V9, which were recently reported to be associated with T-cell recognition of Mls gene products in these strains, was examined. The results indicate that the majority of commonly used laboratory strains including those originally typed as Mlsa are also expressing Mlsc determinants and that very few independent inbred strains are non-Mlsc. Moreover, the pattern of Tcrb-V expression in spleen as well as in thymus suggests that the association between Mls expression and clonal deletion of self Mls-reactive T cells appears to be the general rule in inbred strains. Based on these results, implications for the nondetectable Mls-like gene products in other species besides the mouse are discussed.

New "leap-frog" technique for autologous blood donation.

In order to obtain a sufficient volume of autologous blood for elective surgery, we used a Sterile Connection Device (SCD, DuPont, Mass. U.S.A.) and a liquid storage method that is called the leap-frog. From 300 to 2800 mL (mean 1158 mL) of blood were collected from 126 patients during an average of six donations over 14-125 days (mean 33 days). 101 patients (80%) required no homologous blood during their surgery. No bacterial contamination was observed in the connected tubes. We submit that it is possible to obtain an adequate volume of blood with a simple technique using this method.

A complicated translocation involving five chromosomes (Nos. 9, 11, 12, 21 and 22) in a patient with chronic myelocytic leukemia (CML).

A new complicated Ph1 translocation involving five chromosomes, t(9;22;21;11;inv ins(12)-(q15p12p13))(q34;q11;q22;q13;q15), was found in a 64-year-old Korean woman with chronic myelocytic leukemia (CML). At presentation, the patient was found to be in the accelerated phase, she entered the chronic phase after six cycles of chemotherapy including a vincristine (VCR) and prednisolone (PSL) regimen (VP). The chronic phase continued for 2 years, and 33 months after her first admission she died due to severe pneumonia and congestive heart failure in the re-accelerated phase. In the literature, the frequency of the involvement of chromosome No. 11 in three-way Ph1 translocations (4.9%) is lower than that in four- and five-way Ph1 translocations (33.3%). It may be worth noting that chromosome No. 11 is easily involved in highly complicated Ph1 translocations.

A case of mucosal prolapse syndrome.

A case of mucosal prolapse syndrome in the rectum is reported. A 61-year-old female presented complaining of bloody discharge and constipation for 2 months. Colonoscopy revealed an elevated lesion on the left anterior wall of the rectal canal. Biopsy showed glandular proliferation and fibromuscular obliteration of the lamina propria, findings characteristic of mucosal prolapse syndrome. It is a rare disorder affecting the rectum because the lesion looks macroscopically like a protruding adenoma.

[Protection of the two types of severely hypertrophied myocardium induced by pressure-loading and by volume overloading].

Although the difficulty of preserving hypertrophied myocardium from induced arrest during aortic clamping has long been recognized, the difference of response to ischemia between the two types of hypertrophy induced by pressure-loading (P) and by volume overloading (V) has not been fully elucidated. In the present study, to assess the two types of hypertrophied myocardium we investigated the 44 patients undergoing valve replacement; the 23 patients of aortic stenosis (AS) without aortic regurgitation (AR), and the 21 patients of AR without AS. The patients of AS (group-P) were divided into two groups regarding the thickness of posterior wall of left ventricle; over 15 mm in P-1 and under 15 mm in P-2. The patients of AR (group-V) were also divided into two groups, which is, preoperative LVEDVI over 250 ml/m2 in V-I and under 250 ml/m2 in V-2. In all four groups myocardial temperature was kept about 5 degrees C with GIK and continuous cold blood coronary perfusion during aortic clamping time. There was no operative death. Postoperative serum enzymes (GOT and CPK) in P-1 were higher than in P-2. Per cent increase of LVEF in P-1 were greater than P-2 and V-1 greater than V-2, respectively. [Comparative study on the comparative efficacy of hepatic arterial or portal venous infusion of adriamycin to control hepatic carcinoma growth].

To compare the efficacy of intrahepatic arterial and intraportal venous infusions of anticancer drugs, the following two experiments were performed. 1. Adriamycin (3 mg kg) aqueous solution (ADR) was administered into hepatic artery (HA group) and portal vein (PV group) of a rabbit with 2 cm diameter VX2 liver tumor. The ADR levels of the tumor tended to be about 3-fold higher in HA than in PV group (p less than 0.1) one hour after ADR infusion. No significant difference was seen in the ADR levels of liver, heart, kidney, and bone marrow between the two groups. The ADR concentration ratio of tumor to liver was significantly higher in HA group than in PV group. 2. VX2 tumor cells (1 x 10(7) cells) were transplanted through the portal vein of the rabbit. ADR aqueous solution (1 mg/kg, 5 ml) was infused into the rabbit liver via HA and PV, at 3, 5, and 7 days after tumor inoculation. The rabbits were killed and the livers were excised 21 days after tumor implantation. The number of VX2 tumor nodules on the liver surface was less in HA group than in PV group. These results suggest that hepatic arterial infusion of anticancer drug is more useful to inhibit the growth of liver tumor than portal venous infusion.

[Multihospital co-operative study of post-operative adjuvant chemotherapy in the treatment of gastric cancer-(Part 3). Comparisons between tegafur and UFT. North Kyushu Co-operative Study Group for Cancer Chemotherapy].

The North Kyushu Co-operative Study Group for Cancer Chemotherapy (21 participating facilities) carried out a comparative study of post-operative adjuvant chemotherapy in the treatment of 243 patients after surgery of gastric cancer in stages II, III and IV over 27 months period starting from October 1983. The patients were divided randomly into group A (treated with 20 mg of MMC on the day of the surgery and 10 mg of MMC on the following day, and with a daily dose 600 mg of orally administered tegafur starting from two weeks after the surgery. In addition to the above treatments, patients with Stage IV were treated with 10 mg of MMC every three months starting from four weeks after the surgery) and group B (treated with MMC in a similar manner to group A, and with 600m g of UFT instead of the tegafur). Thirty-three patients (13.6%) were excluded or dropped out, and the remaining 210 cases were analyzed. No severe side effects appeared in either of the groups, and a long-term administration was safe. The five-year survival rate among the radical surgery patient was significantly higher in the UFT group compared with the tegafur group (p less than 0.05). Significantly higher survival rate in the UFT group was observed (p less than 0.05) for the cases of poorly-differentiated adenocarcinoma. From the above results it appears that an addition of UFT to the treatment with MMC is useful in post-operative adjuvant chemotherapy for gastric cancer.

[An experimental study on the alteration of myocardial tissue by radiofrequency catheter ablation: differences according to the frequency and the device].

To evaluate the alteration of myocardial tissue by radiofrequency (RF) catheter ablation according to differences of the frequency and the device, we applied RF catheter ablation to canine right ventricular myocardium in vitro. The devices delivering RF energy were LL-49(0. 49MHz, fixed voltage mode), RA-50 (13.56MHz, fixed watt mode) and HAT200 (0.52MHz, fixed watt mode with the capability of temperature adjustment). After RF catheter ablation were performed with a unipolar electrode catheter at various output under the same condition of contact, the ablated lesions by each device were compared. Histologically, we recognized coagulation layer with the necrosis of endocardium and the dissociation of myocardium in each experiment. The degree and the size of ablation layer was significantly larger in lower frequency delivery (p less than 0.05) and was significantly smaller by using the device with temperature adjustment as compared with the others (p less than 0.05). It was suggested that the alteration of myocardial tissue ablated with RF energy differ according to the frequency and the mode of output adjustment of the device.

[Hepatic arterial infusion of farmorubicin (FARM) and 5-fluorouracil (5-FU) in a patient with massive hepatic metastasis from gastric cancer].

A patient with massive hepatic metastasis from gastric cancer was treated with hepatic arterial injection of FARM and 5-FU one month after total gastrectomy. Response rate was 88% at 3 months after arterial infusion therapy. Severe side effects were not seen during treatment. It is suggested that the hepatic arterial infusion of combined FARM and 5-FU is a useful method for treatment of hepatic metastasis of gastric cancer.

[Study of bone metabolism for breast cancer by MD (microdensitometry) method].

Bone is one of the involved organs of distant metastasis in breast cancer. We examined bone metabolism by microdensitometry (MD) method in 94 patients with breast cancer (40 with bone metastasis and 54 without it) and 10 with benign breast diseases. There was no significant difference among the three groups in the background factors of menopausal status and hormone receptors status. MCI, GSmin, sigma GS/D and bone pattern by MD method in breast cancer patients with bone metastasis were significantly lower than those in the other groups. There were slight reverse correlation between sigma GS/D and serum ALP. MCI and sigma GS/D indices corrected by ages were also significantly lower in bone metastasis (p less than 0.01). These indices in most of the patients with bone metastasis gradually decreased as the lesion progressed. The results revealed that microdensitometric analysis of bone metastasis for breast cancer was of use for the diagnosis and evaluation of therapeutic effect in the follow-up study.
