Lymphocyte transformation in vitro in patients with immunodeficiency diseases: use in diagnosis, histocompatibility testing and monitoring treatment.

We have used lymphocyte stimulation in vitro to characterize the degree of cell-mediated immunodeficiency in different patients. The effect of treatment of patients with immunodeficiencies is illustrated by lymphocyte transformation in vitro before, during and after therapy (eg bone marrow transplantation of severe combined immunodeficiency and transfer factor treatment in Wiskott-Aldrich syndrome). The mixed lymphocyte culture test has been used for selection of bone marrow transplant donor for a patient with CID when an HL-A identical sib was not available. The results of the transplantation in this patient with graft from a donor who differed from the patient in respect of both HL-A haplotypes (but MLC identical with the patient) is reported. The MLC data on another family with 2 children with Nezelof disease are reported. The data indicate that one of the patients could be a chimera after intrauterine grafting of maternal immunocompetent cells.

Functional studies of Peyer's patches: evidence for their participation in intestinal immune responses.

The pattern of responsiveness of lymphocytes from Peyer's patches, spleen, mesenteric, and inguinal nodes of guinea pigs was compared after oral and parenteral immunization. After oral immunization with BCG, Peyer's patch lymphocytes showed the best in vitro proliferative responses to PPD (tuberculin). Responsiveness to Clostridia, normal constituents of the intestinal microflora, also was significant in Peyer's patches, and low or absent in the other lymphoid tissues examined. In animals immunized parenterally with heat-killed Mycobacterium tuberculosis, lymphocytes from the spleen reacted best to PPD and those from Peyer's patches least. Selective elimination of B or T cells from PPD-reactive Peter's patch lymphocyte populations showed that the T-cell-rich fraction remained responsive to PPD, the B cell-rich fraction became unresponsive; but both fractions reacted to Clostridia. We conclude from this experiment that a functional T cell population is present in these organs. Peyer's patches in the guinea pig are immunocompetent lymphoid organs which seem to be primarily engaged in immune responses to antigens presented from the intestinal lumen.

Deficiencies of the complement system in man.

Deficiencies of components of the complement system in man are described. Clq was reconstituted in patients with severe combined immunodeficiency after bone marrow transplantation. Clr deficiency is associated with vascular disease and upper respiratory infections. Reconstitution of complement occurred in one patient with Clr deficiency after kidney transplantation. Our C2-deficient patient who developed systemic lupus erythematosus had elevated levels of C3. Following blood transfusion, ie by providing C2, dramatic activation of complement occurred in vivo.

Two separate genes controlling stimulation in mixed lymphocyte reaction in man.

The genetic control of strong stimulation in the mixed lymphocyte culture reaction is determined by a separate gene (MLR-S) closely linked to the FOUR-locus of the HL-A chromosomal region. Three additional examples of siblings with recombination between FOUR-locus and MLR-S locus are presented which confirms the independent genetic control of mixed lymphocyte reaction from the control of HL-A antigens. The occurrence of two recombinant children in one family with four other children representing all possible HL-A haplotype-combinations, strongly supports the genetic mapping of the MLR-S determinants outside the HL-A chromosomal region. The experiments presented show that additional genes located within the HL-A region itself contribute with a weak stimulation of allogenic mixtures. These data are discussed in relation to the marginal stimulation of the mixed lymphocyte culture reaction which can be seen between unrelated individuals. It seems that a group of relatively histocompatible individuals can be defined by identity of the MLR-S locus but with differences on the weak MLC-determinants, and that this group for the purpose of clinical transplantation behaves as histocompatible individuals.
