Structural change in dopamine D2 receptor gene in a patient with neuroleptic malignant syndrome.

Dysfunction of the dopaminergic system has been suggested as a pathogenic mechanism in neuroleptic malignant syndrome. Therefore, we examined the complete coding sequences of the dopamine D2 receptor (DRD2) gene for structural abnormalities in 12 patients with a history of NMS, including two cases of familial NMS. Mutational analysis was performed by denaturing gradient gel electrophoresis (DGGE), a highly sensitive technique for detecting sequences differences. We found in one patient with a history of NMS a nucleotide substitution at codon 310 (CCG-->TCG) of exon 7 of the DRD2 gene which predicts the replacement of proline to serine in the third cytoplasmic loop of the receptor, a part of the receptor that interacts with G-proteins. A larger series of patients with NMS needs to be investigated to establish whether this allele is associated with an increased susceptibility to NMS.

[Influence of thoracic spinal subarachnoid microinjection of noradrenaline on the effect of electroacupuncture].

Influence of thoracic spinal subarachnoid microinjection of noradrenaline (NA, 40 mg/20 microliters) on the effect of electroacupuncture (EA) at Neiguan (PC6) was observed in 40 rabbits anesthetized with urethan-chloralose. Results showed that: 1) EA of Neiguan (PC6) could significantly accelerate the recovery of ST-segment and T-wave of electrocardiogram induced by acute myocardial ischemia (AMI); 2). Subarachnoid microinjection of NA also could promote the recovery of ST, T and blood pressure of post-AMI; and 3) Subarachnoid microinjection of NA could strengthen the effect of EA of Neiguan (PC6) to promote the recovery of ST and T of Post-AMI. It suggests that alpha-receptors of intra-thoracic spinal cord participate in the action of EA of Neiguan (PC6) ameliorating AMI, and the upper thoracic segement is one of the links for connecting Neiguan and the heart.

[Ultrastructural observation of tendonization of artificial tendon 109HH in rabbit].

Ten New Zealand rabbits were divided into 5 groups at random. Calcaneal tendons were cut bilaterally, then atificial tendon 109HH was used to connect the two ends of the cut tendon. Ultrastructural changes of control group and experimental groups at 2, 6, 12, 28 weeks after section were observed. The results showed that fibroblast proliferated and a lot of ribosome and RER appeared in plasm during 2 approximately 6 weeks, indicating artificial tendon caused fibroblast proliferation. During this period, fibroblast over synthesized collagenous protein and the synthesis of collagenous fibers peaked. During 12-28 weeks, the number of fibroblasts and the synthesis of collagenous protein decreased. Finally, fibroblasts became inactive tendon cells. With the formation of new tendons, the artificial tendens were degradated and absorbed, and disappeared after 12 weeks. The new tendon fibers became thicker and had the correct direction through reconstruction. The structure and function of new tendons could be restored to be consistent with normal values.

No structural mutation in the dopamine D2 receptor gene in alcoholism or schizophrenia. Analysis using denaturing gradient gel electrophoresis.

OBJECTIVE: To examine the dopamine D2 receptor (DRD2) gene coding sequences for abnormalities associated with schizophrenia or alcoholism and thereby help to resolve the controversy surrounding the reported association of alcoholism with a restriction fragment length polymorphism located close to the DRD2 gene. DESIGN: Mutational analysis of complete DRD2 gene coding sequences by denaturing gradient gel electrophoresis followed by direct nucleotide sequencing of detected variants. SETTING: Patients and controls from clinical and epidemiologic collections in the United States and Europe. PATIENTS: A total of 253 unrelated individuals, including 106 patients with schizophrenia, 113 with alcoholism, and 34 controls. For alcoholism we included patients from previously published series in which an association of illness with allele A1 was reported (Taql site 3' to the DRD2 gene) and from other published series in which nonconfirmations of this association were reported. Nearly all persons examined were white. MAIN OUTCOME MEASURES: Frequency of nonsilent variations in DRD2 gene DNA sequences in the different diagnostic groups. RESULTS: We found three infrequent DNA variants that predict altered amino acid sequence of the receptor. None of these is associated with either alcoholism or schizophrenia. CONCLUSION: No structural coding abnormalities in the DRD2 gene are present in alcoholism or schizophrenia.

Incorporation of excess wild-type and mutant tRNA(3Lys) into human immunodeficiency virus type 1.

Human immunodeficiency virus (HIV) particles produced in COS-7 cells transfected with HIV type 1 (HIV-1) proviral DNA contain 8 molecules of tRNA(3Lys) per 2 molecules of genomic RNA and 12 molecules of tRNA1,2Lys per 2 molecules of genomic RNA. When COS-7 cells are transfected with a plasmid containing both HIV-1 proviral DNA and a human tRNA3Lys gene, there is a large increase in the amount of cytoplasmic tRNA3Lys per microgram of total cellular RNA, and the tRNA3Lys content in the virus increases from 8 to 17 molecules per 2 molecules of genomic RNA. However, the total number of tRNALys molecules per 2 molecules of genomic RNA remains constant at 20; i.e., the viral tRNA1,2Lys content decreases from 12 to 3 molecules per 2 molecules of genomic RNA. All detectable tRNA3Lys is aminoacylated in the cytoplasm of infected cells and deacylated in the virus. When COS-7 cells are transfected with a plasmid containing both HIV-1 proviral DNA and a mutant amber suppressor tRNA3Lys gene (in which the anticodon is changed from TTT to CTA), there is also a large increase in the relative concentration of cytoplasmic tRNA3Lys, and the tRNA3Lys content in the virus increases from 8 to 15 molecules per 2 molecules of genomic RNA, with a decrease in viral tRNA1,2Lys from 12 to 5 molecules per 2 molecules of genomic RNA. Thus, the total number of molecules of tRNALys in the virion remains at 20. The alteration of the anticodon has little effect on the viral packaging of this mutant tRNA in spite of the fact that it no longer contains the modified base mcm 5s2U at position 34, and its ability to be aminoacylated is significantly impaired compared with that of wild-type tRNA3Lys. Viral particles which have incorporated either excess wild-type tRNA3Lys or mutant suppressor tRNA3Lys show no differences in viral infectivity compared with wild-type HIV-1.

AML-1 gene rearrangement and AML-1-ETO gene expression as molecular markers of acute myeloblastic leukemia with t(8;21).

Rearrangements of the AML-1 gene on chromosome 21 as well as transcriptional expression of AML-1-ETO fusion gene were studied in 35 leukemic patients with t(8;21)(q22;q22). A panel of probes generated from the AML-1 gene regions flanking the breakpoint on chromosome 21 allowed us to detect the rearrangement in 24 out of 29 patients. A specific nested reverse transcriptase/polymerase chain reaction (RT/PCR) was developed to detect the t(8;21), either at diagnosis or as minimal residual disease. PCR amplification products were obtained in ten out of 11 patients investigated, and the sensitivity of the reaction was estimated to be between 1 x 10(4) and 1 x 10(-5) cell. An AML-1 rearrangement was also detected in one patient with 8q- and only one chromosome 21, but without 21q+. This indicated that the molecular rearrangement of the der(8) chromosome is more important than the reciprocal one in the malignant process.

[Influence of thoracic spinal subarachnoid microinjection of phentolamine on the effect of electroacupuncture].

The influence of thoracic spinal subarachnoid microinjection of Phentolamine (Phen, 200ug/20ul) on the effect of electroacupuncture (EA) at Neiguan (PC 6) was observed in 42 urethan-chloralose anesthetized rabbits. Results showed that: 1) EA of Neiguan (PC 6) could significantly promote recovery of elevated ST-segments and T-waves of electrocardiogram induced by acute myocardial ischemia (AMI). 2) subarachnoid microinjection of phentolamine had no marked influence on recovery of ST-segments and T-waves, but impeded recovery of blood pressure of post-AMI; and 3) subarachnoid microinjection of phentolamine could significantly weaken or eliminate the effect of EA of Neiguan (PC 6) in accelerating recovery of ST-segments and T-waves of post-AMI. It denotes that alpha-receptors of intra-thoracic spinal cord participate in the action of EA of Neiguan (PC 6) in improving AMI, and the upper thoracic segment of the spinal cord is one of the links for connecting Neiguan and the heart.

[Influence of electroacupuncture of neiguan (PC 6) on ami-induced changes in electrical activity of dorsal horn neurons].

The electrical activity of the dorsal horn (DH) neurons was recorded extracellularly by glass micro-pipettes in urethane-chloralose anesthetized rabbits to analyze the role of the thoracic segments (T2-T3) of the spinal cord in the integration of information from electroacupuncture (EA) at Neiguan (PC 6) and acute myocardial ischemia (AMI). Results were: 1) after AMI, of 83 DH neurons, 18 displayed a significant excitatory response, 14 an obvious inhibitory reaction and 51 no apparent change; 2) after EA at Neiguan (PC 6), of 10 neurons with AMI-induced excitatory response, the electrical activity of 9 neurons was suppressed and the other 1 still maintained its excitatory state; of 8 neurons with AMI-induced inhibitory response, 7 were relieved from the suppressed state, the other 1 had no change following EA; and 3) the receptive field and the responsive types to somatic stimulation were examined in 83 DH neurons. Results showed that signals from EA at Neiguan (PC 6) and AMI could converge to the DH neurons of T2-T3. It suggests that DH participates in the integrative course of information from EA and AMI and thus it is one of the connective links between Neiguan (PC 6) and the heart.

[Detection on BCR-ABL fusion gene in Ph1 chromosome positive leukemia by "nested" retrotranscriptase/polymerase chain reaction].

By using "nested" retrotranscriptase/polymerase chain reaction (RT/PCR) technique, we determined the expression patterns of the BCR-ABL fusion gene resulting from the chromosomal translocation t (9; 22) in leukemias with Ph1 chromosome. Three distinct isoforms of fusion gene transcripts were discovered: ela2, b2a2 and b3a2. In 10 cases of chronic myelogenous leukemia (CML), only two types (b2a2 and b3a2) were observed. However, in 8 cases of Ph1 chromosome positive acute lymphoblastic leukemia (Ph1 + ALL), all three types were detected, including two cases characterized by the coexistence of ela2 and b2a2 or b3a2 types. Moreover, the RT/PCR procedure established in the present study proved to be a very sensitive method, allowing the detection of one leukemic cell among 10(5)-10(6) normal cells. Thus, a positive RT/PCR result was obtained in two Ph1 + ALL cases just after chemotherapy-induced clinical remission (CR), suggesting the presence of residual disease. One case remained RT/PCR positive four months after achieving CR and relapsed in the fifth month, while the other case converted to RT/PCR negative eight months later and is now in CR for 18 months. Therefore the detection of BCR-ABL fusion gene is of importance not only in the study of the pathogenesis of Ph1 + leukemias but also in their diagnosis and monitoring of minimal residual disease during CR.

Acute promyelocytic leukemia: clinical relevance of two major PML-RAR alpha isoforms and detection of minimal residual disease by retrotranscriptase/polymerase chain reaction to predict relapse.

Recent data have shown that the PML-RAR alpha fusion gene resulting from translocation t(15;17) is a highly reliable molecular marker of acute promyelocytic leukemia (APL). In this study performed on 97 Chinese patients with APL, the retrotranscriptase/polymerase chain reaction (RT/PCR) was used to evaluate the clinical relevance of the long (L) or short (S) PML-RAR alpha fusion mRNA isoforms and to study minimal residual disease during clinical remission (CR). There were more early deaths during the all-trans retinoic acid (ATRA) induction treatment and more relapses within 2 years of CR in the S-type (6 of 19 cases) than in the L-type group (2 of 33 cases) (P < .025). Among 12 cases analyzed before and after the ATRA-induced CR, 9 cases (75%) showed positive RT/PCR, whereas only 3 cases showed a negative result, justifying the need for chemotherapy after ATRA-induced CR. Eleven of 62 APL patients in CR, after ATRA-induced CR and chemotherapy consolidation (follow-up, from 3 to 72 months), showed positive RT/PCR. Five of them relapsed within 1 to 6 months after the positive test; one converted to negative after further chemotherapy; and 5 remained in CR status without further PCR data. However, the latter 5 cases all received further intensive consolidation therapy after the PCR positivity. These results show that a positive RT/PCR of PML-RAR alpha is a sensitive predictor of relapse in APL.

Linkage analysis of fifty-seven microsatellite loci to bipolar disorder.

The authors' goal was to screen for genetic linkage with highly informative deoxyribonucleic acid (DNA) microsatellite markers on a series of moderately sized North American bipolar disorder (BP) pedigrees. These BP pedigrees were genotyped with 57 short tandem-repeat polymorphic systems (microsatellites) that were enzymatically amplified from genomic DNA. We did not find significant evidence for genetic linkage. We found isolated LOD scores greater than 2 on chromosome 1 at two loci in individual pedigrees. Simulation studies for multiple analyses under the assumptions of linkage and nonlinkage were performed. The simulations show that LOD scores greater than 2 could be expected even when linkage is absent. Significance levels need to be considered carefully in systematic linkage studies.

[Effect of EA on the enzymes of microvasculature and CA fluorescence in acute myocardial ischemia].

It was aimed at observing the histochemical basis of physiological experiment about the effect of electroacupuncture (EA) on acute myocardial ischemia (AMI) in this paper. The capillaries of myocardium were reflected with both alkaline phosphatase (ALP) and Mg(2+)-ATPase. Thirty min after ligating the left ventricular branch (LVB), the capillaries stained by ALP were much decreased in the ischemic group without EA, while in EA group they were increased distinctly. The number and the total length of the capillaries in a 220 x 320 microns ischemic area of the posterior wall (section 10 microns) were measured and compared. In ischemic group the number was 17.2 +/- 1.65, the length was 634.62 +/- 66.24 microns, in EA group 22.5 +/- 1.44, 1187.57 +/- 103.69 microns, respectively. Both counts were significant differences (P < 0.05, P < 0.001). It reflected that EA could improve the microcirculation and metabolic function of AMI. Mg(2+)-ATPase was shown on the plasma membrane of endothelial cell of microvasculature and analysed quantitatively with a Univar scanning microspectrophotometer. After occluding LVB for 30 min its optic density of AMI was decreased to 106.83 +/- 14.06, while it was increased to 210.83 +/- 24.88 in EA group, P < 0.05. The activity of Mg(2+)-ATPase could increase in transporting Na+, k+ ions through the membrane of the endothelial cells of ischemic myocardium. The result was consistent with the physiological experiment, because EA could regulate the changes of mean repolarization rate induced by AMI and adjust the ion concentration of transmembrane. The catecholamine (CA) in myocardium was located at sympathetic adrenergic terminals with varicosities containing norepinephrine (NE).(ABSTRACT TRUNCATED AT 250 WORDS)

[Influence of electroacupuncture on electrical activity of dorsal horn neurons of the thoracic spinal cord in the rabbit].

In the present paper, influence of electroacupuncture (EA) on electrical activity of neurons of dorsal horns of T2-T3 spinal segments was analysed in chloralose-urethane anesthetized rabbits. Results were: 1). after EA of "Neiguan" (PC 6), of the 31 dorsal horn neurons, 11 increased significantly in their firing rate, 4 decreased obviously and other 16 had no marked change; 2). after EA of "Zusanli" (ST 36), of the 28 neurons, only 2 increased significantly in firing rate, 14 decreased and other 12 had no clear change; 3). of the 6 neurons which all responded to EA of "Neiguan" (PC 6) and "Zusanli" (ST 36), 5 were excited by "Neiguan" (PC 6) and suppressed by "Zusanli (ST 36); the other one was inhibited by both; 4). the receptive field on the body surface and physiological types of 18 neurons were also examined. The results indicate that information from EA of "Neiguan" and "Zusanli" can converge to the neurons of dorsal horns of T2-T3 spinal cord, and different acupoints have a relative specificity.

[Influence of EA on the effective refractory period in rabbits with AMI].

A method of an incomplete ischemia of the lateral wall of the left ventricle produced by pulling the ventricular branch of the left coronary artery of rabbits was employed in this study and the dispersion of the effective refractoriness used as an index. It was noted that the effective refractory periods at the different sites in AMI region were mainly shortened and the dispersion of refractoriness increased obviously. Nevertheless, EA at Neiguan acupoint could inhibit reduction of the effective refractory of ischemic myocardium and improve the dispersion of refractoriness markedly, which would be beneficial to synchronization of the myocardial excitatory state. It suggests that EA have some effect of anti-arrhythmia.

Change of thyroid function in 50 patients with aplastic anemia.

The serum thyroid hormones were measured in 50 patients with aplastic anemia (AA). Serum T3 and T4 levels were significantly lower, especially in the elderly and in those in chronic AA, while they were normal in acute AA. There was no obvious difference with thyroid-stimulating hormone (TSH). It was suggested that the result may be caused by euthyroid sick syndrome, a protective adaptation that helps in classifying and evaluating the prognosis of the disease.
