Localization and neurophysiological properties of motoneurones of the M. triceps surae of the rat after retrograde labelling with Evans blue.

Evans blue was injected into either the medial and lateral gastrocnemius or the soleus muscle of adult albino rats to mark retrogradely the corresponding motoneurones in the spinal cord. The labelled motoneurones were identified by the red fluorescence of their perikarya in the ventral horn of segments L4--L6. In addition, a monosynaptic reflex action potential was recorded only in the ventral roots L4--L6 after stimulation of the nerves to the medial and lateral gastrocnemius and the soleus muscle. Excitability and reflex latencies of labelled and unlabelled motoneurones of segment L4--L6 as well as the conduction velocities of their axons were measured. Apart from a small but statistically nonsignificant increase in excitability, no functional differences were found between labelled and control neurones. Thus, retrograde labelling of motoneurones with Evans blue prior to performing electrophysiological experiments has the advantage that the marked motoneurones can be identified under the fluorescence microscope without the need of additional staining or fixation.

[The influence of general operation risks on the indications for operations for gynaecological cancers (author's transl)].

The increase of high general operative risks in women presenting for gynaecological operations suggested the investigation of the influence of these risks on the choice of treatment of gynaecologic cancers appropriate for the stage. The primary treatment of 426 gynaecologic and breast cancers of the year 1977 is reviewed. There were 53 endometrial carcinomas, 29 ovarian carcinomas, 185 breast cancers, 142 carcinomas of the cervix and pre-invasive carcinoma of the cervix. There were 17 other carcinomas. 354 patients were selected for operative treatment. 72 patients were selected for primary radiotherapy. A considerable number of patients (40-60%) were selected for treatment appropriate for the stage of the cancer by a system of diagnosis of risks, classifications of risks, evaluation of risks and pre-operative treatment of operative risks. The choice of the cancer treatment appropriate to the stage is not influenced by single risks but by the sum of the general operative risks. Typical mistakes in evaluation of operative risks versus indications for the operations are described in case reports. The reports include avoidable and unavoided risks.

Familial centronuclear myopathy: a haploid DNA disease?

In the same family three boys (ages 8, 11, 14 years), suffering from a slowly progressive weakness of the flexors of the head and distal muscle groups of the extremities, were found to have a myopathy characterized by the presence of central nuclei in type I fibres. Their mother showed no histopathologic signs but a myopathic pattern on electromyography could be demonstrated. Cytophotometric examination of the anterior tibial muscle of these siblings revealed a reduction in DNA content by 50% in the central and subsarcolemmal nuclei (haploid DNA content) when compared with subsarcolemmal nuclei (diploid DNA content) of the same muscle of their mother. Nuclear DNA concentration in fibrocytes did not differ significantly between members of the family. Therefore, it is assumed that the nuclei of muscle fibres are selectively affected by the disease process which is indicated by a haploid DNA content.

[Peripheral nerve changes in Tangier disease. Comparative light-, electron-microscopic and cytometric studies (author's transl)].

A sural nerve biopsy was performed in a 55-year-old male patient with Tangier disease (familial-lipoprotein deficiency). Light-microscopy showed an increase in the endoneural connective tissue and a loss of nerve fibers indicating a chronic peripheral neuropathy. Electron-microscopy revealed an accumulation of lipid droplets within Schwann cells of myelinated and unmyelinated nerve fibers. When compared with age-matched controls the myelinated fiber density was reduced with a relative preponderance of small myelinated fibers. In addition, distributional cytometric studies of nerve fibers in relation to the perineurium and endoneurial capillaries showed: Contrary to 4.6-7.5 micron thick nerve fibers, which accumulated in the center of the nerve fascicle, small (0.5-4.5 micron) and large (7.6-10.0 micron) fibers lay nearby the perineurium. The measured increase in small myelinated nerve fibers around endoneurial capillaries may be explained as a sign of regeneration.

Reactivity patterns of nerve cell-glia-complexes in mice during pentylenetrazole-induced seizures. Cytometric-cytophotometric studies.

The relationships between nerve cell-glia-complexes (NGCs) and surrounding capillaries were studied by combined cytometric-cytophotometric methods in male mice up to 60 min after injection of 70 milligrams/kilograms of pentylenetetrazole (PTZ). The nuclear area in neurons from cortical layers IV and V of the motoric area 8 increased by 13%, and in neurons from the hypoglossal nucleus there was an increase of 23%, associated with phasic changes in the nuclear area of their glial satellites. The number of perfused capillaries increased in both brain regions and there was a 20%-elongation of erythrocyte-filled capillary segments. The cytoplasmic RNA content enlarged in the hypoglossal neurons but decreased in cortical neurons from area 8. Peptidergic neurons and their glial satellites (supraoptic nucleus) showed nuclear shrinkage. Fifty percent of these peptidergic neurons reacted by reduction of their cytoplasmic RNA content. The measured cell reactions seem to be specific for different groups of NGCs which were defined by their histotopography, their neuronal interconnections, and a prevalence of secretory or nonsecretory neurons.

[Studies of 57-co-bleomycin in patients with carcinoma colli uteri (author's transl)].

The kinetics of 57Co-Bleomycin was studied in twelve patients with histologically proven carcinoma colli uteri and in ten patients with benign gynaecological conditions. Measurements were carried out on the whole body, on parts of the body and on urine and blood using a whole body counter. Excretion of the tumour-seeking radio-pharmaceutical is predominantly through the kidneys and can be described by a double compartment model. After 48 hours, 2-4% of the applied activity is still present in the body. Radiation exposure to the body, the kidneys and bladder was calculated from the measurements and MIRD tables as 0.02 rad/mci (total body), 0.4 rad/mCi (kidneys) and 0.2 rad/mCi (bladder). In view of the rapid elimination of 57Co-Bleomycin, these figures are lower than data data quoted in the literature. There was no evidence of significantly higher uptake of 57Co-Bleomycin in the carcinomatous epithelium of the collum uteri compared with benign diseases, using the total body counter in the scan node. As possible explanations for this, the authors suggest inadequate geometric resolution of the system and a low tumour/non-tumour distribution ratio.

[The preventive treatment of general risks prior to gynaecological operations (author's transl)].

The operative treatment of high risk patients is of increasing importance in gynaecology. The preoperative preventive treatment of latent general problems is an important tool in the prevention of intraoperative and postoperative complications. The experience of the department for gynaecology in Hanover, Germany, during 1976 is reported. Among 1583 major operations 158 were carried out on patients in the risk class III. Guidelines are described for the preventive treatment of cardiac, bronchopulmonary, hypertensive and diabetic risks. The importance of obesity and advanced age is discussed in our cases.
