[Nephroblastoma in adults].

Nephroblastoma is very rare in adults and must be differentiated from renal cell carcinoma. The diagnosis can only be made by means of histological examination, which can cause problems because of the various morphological types of the tumor. In this paper an account is given of the authors' experience in the diagnosis and treatment of four Wilms tumors in stages I (n = 1), II (n = 1), III (n = 1) and IV (n = 1) in adult patients. One patient died of a relapse, two patients are taking part in a follow-up study and one patient is undergoing chemotherapy.

[Diagnosis and therapy of single kidney cysts].

By means of observation of 191 own patients suffering from a symptomatic single renal cyst and a literature review, epidemiology, clinical presentation, diagnostic procedures and therapy are presented. The significance of ultrasonography and computerized tomography are emphasized. For single, symptom-free cysts besides regular ultrasonographic examinations no further therapy is necessary. In case of a required therapeutic procedure, an open resection of the cyst should be preferred to percutaneous a required therapeutic procedure, an further therapy is necessary. In case of a required therapeutic procedure, an open resection of the cyst should be preferred percutaneous approach.

[Splenogonadal fusion].

The splenogonadal fusion is a rare but benign anomaly. The diagnosis is usually given at operation or histologically after operation. In about 50% of the patients who undergo surgery for scrotal tumor, an unnecessary orchidectomy is performed due to clinical unawareness of this rare condition. A case of splenogonadal fusion belonging to the continuous type is presented to illustrate problems of this anomaly and point out the importance of intraoperative microscopic sections.

Application of luminol-dependent chemiluminescence to assay opsonizing antibodies to procyclic forms of Trypanosoma congolense in the sera of dogs experimentally infected with heterologous stocks.

Luminol-dependent chemiluminescence (LCL) responses of dog granulocytes were used to assay opsonizing antibodies to procyclic culture forms of T. congolense. A high degree of sensitivity was demonstrated and LCL levels were high, when the phagocytic cells were incubated with the sera of infected dogs even at dilutions as high as 1:400 as compared to pre-infection or negative sera. The levels of opsonizing antibodies were elevated in all the dogs throughout the time of observation. The technique was sensitive, could be automated and, therefore, allows a rapid evaluation of large numbers of serum samples for Trypanosoma specific antibodies.

[Paraspinal conduction anesthesia from the viewpoint of the patient--exemplified by a selected urologic patient sample].

Patients operated because of benign prostatic hyperplasia in spinal cord close block anesthesia were interviewed by a questionnaire. The well known and accepted medical advantages of this method are completed by the results of an examination of the psychic situation. Only 6 out of 66 patients have negative feelings of awakeness during the operation.

[Para-pelvic cysts: difficulties in differential diagnosis].

According to their different etiology, parapelvic cysts must be differentiated from simple renal cysts, pararenal pseudocysts, intramural renal pelvis cysts and suprarenal pseudocysts. An important criterium of parapelvic cysts is their epithelial lining. According to their radiological features, these cysts must be described as "parapelvically located cysts" until the definite histological diagnosis is made by the pathologist. The exact differentiation between a parapelvic cyst and a pararenal pseudocyst may be of forensic importance in posttraumatic patients.

[Ectopic adrenal cortex tissue].

A case of a 40-year-old man with an adreno-cortical ectopia in the region of the spermatic cord is presented. The genesis and complexity of this anomaly is discussed. The seldom occurring, but sometimes important clinical relevance of these ectopies is demonstrated.

[Radiological diagnosis and therapy of postoperative abdominal lymphoceles].

Eighty-one patients with postoperative abdominal lymphoceles were examined by CT and ultrasound. Both methods provide accurate information regarding localisation and extent of the lymphocele. In 51 patients conservative treatment under sonographic or CT observation was carried out. In seven patients, irradiation lead to complete regression. In 21 patients, interventional radiological therapy was carried out with simultaneous irradiation in seven cases. In five patients surgery was necessary. Asymptomatic lymphoceles should be treated conservatively and only after the appearance of clinical symptoms should interventional radiological or surgical measures be carried out.

[Fibrous polyp of the ureter].

Benign tumors in the upper parts of the urinary tract are rare. The clinical symptoms of flank pain and/or macrohematuria often present problems concerning differential diagnosis. As radiological sign the smoothly bound filling defect with possible ureter obstruction is characteristic but not convincing. Therefore the diagnosis can only be made by operation and a simultaneous histological analysis. This report is based on a female patient with symptoms of flank pain and macrohematuria. By operative exploration a fibrous ureteral polyp was found which was then excised through an ureterotomy.

[Significance of drug prevention of thromboembolism in urologic risk patients].

The phlebothrombosis and pulmonary embolism are severe postoperative complications. Urological patients are particularly at risk with thrombosis. Therefore it becomes necessary for urology to perform a consequent prophylaxis of thromboembolism which, when an increased risk is present, must contain medicamentous measures. As it is confirmed by evident studies, the low-dose heparin prophylaxis proved as optimum method. A still more effective thrombosis protection can be achieved by the combination of small heparin doses with dihydroergotamine. Retrospectively, 2 comparable groups of urological risk patients with and without heparin prophylaxis were examined. In the adenomectomies under heparin prophylaxis a trend to the reduction of the events of postoperative thromboembolism is revealed. The heparin prophylaxis does not lead to an increase of complications of haemorrhages which need therapy. The prophylaxis of thromboembolism with heparin and a heparin-DHE-combination, respectively, is recommended for the application in urological risk patients.

Antenatally detected Klinefelter's syndrome in twins.

An increased incidence of twinning has been reported among patients with Klinefelter's syndrome (XXY) and their relatives, but no data have been reported about the incidence of XXY among twins. Results from our institution's antenatal testing clinic provide data about twins and XXY. In 1842 pregnancies tested, 21 were noted to be twin gestations. Five fetuses with XXY were detected in all pregnancies, of which there was a pair of MZ twins concordant for XXY and a pair of DZ twins discordant for XXY. The incidence of XXY in twins was thus 7,1%. The fetal phenotype of XXY was examined in aborted MZ twins and found to be mild. Notably, the testicular histology was normal. These cases confirm the previously reported association of twinning and XXY. The association of twinning and XXY implies a commonality of causation; however, a maternal age effect cannot be excluded. Our data suggest that there is an increased risk of XXY among twins.
