Metaiodobenzylguanidine (MIBG) imaging is a well-established method for locating intra- and extraadrenal pheochromocytomas. We investigated whether preoperative injection of 123-I-MIBG might be useful for intraoperative staging of chromaffine tumor cells. This was performed in a 46-year-old patient in whom the diagnosis of a malignant pheochromocytoma had been established by 123-I-MIBG imaging and enhanced catecholamine secretion. The rationale for intraoperative staging in this patient was a discrepancy between computed tomography (CI) of the abdomen and the radionuclide imaging, because scintigraphy revealed a mass with MIBG uptake in the right lower abdomen that could not be visualized by CT. We thus applied a preoperative dose of 4 mCi 123-I-MIBG and determined tissue activity by direct organ measurement. A right abdominal mass was thus identified with an activity of 10 x 10(4) impulses/s as compared to normal tissue (15 x 10(2)). The left-sided tumor was found to be identified correctly by prior CT and MIBG imaging. We thus conclude that intraoperative application of this single probe measurement might help to identify chromaffine tumor cells that have not been located fully by CT.
Cine-MRT was performed in 8 healthy subjects and in 16 patients with myocardial infarction, in whom regional wall motion abnormalities had been demonstrated by left ventricular angiography. Short axis and long axis slices were obtained by angulation of the imaging planes according to the orientation of the cardiac axes. From the end-diastolic and end-systolic images, systolic wall thickening and radial wall motion were determined. In healthy subjects, regional variation in wall thickening was not significant. Patients with myocardial infarction demonstrated abnormal wall motion within the infarct region. Discrepancies between wall thickening and wall motion were observed in 8 patients.
Over the period of the past 9 years (1980-1988), 320 patients (mean age 60.9 +/- 13.2 years) suffering from various liver diseases have been examined. There were three main groups of patients: (1)--24 patients with primary liver cancer (PLC), 19 of them with hepato- and 5 with cholangiocellular carcinoma, (2)--153 patients with metastatic liver tumors (MLT), and (3)--143 patients with inflammatory liver diseases (ILD). The results of examination of alpha-fetoprotein (AFP), carcinoembryonic antigen (CEA), alkaline phosphatase (ALP) and gamma-glutamyl transpeptidase (GMT) in these patients have been analyzed with the aim to evaluate their contribution to the differential diagnostics of tumorous and inflammatory liver processes. For the diagnostics of malignant hepatoma AFP appeared to the most specific test. The significance of other tests for diagnostics of malignant hepatic diseases is obviously limited. These tests are recommended to be considered (in the case of their increase) in close connection with the clinical image and additional examinations. The importance of correlation between cirrhosis and malignant hepatoma is also to be noticed. In spite of all this, we believe that in the case of positivity of the above tests the patients have to be carefully examined and followed up, and that the clinical course and the dynamic of the mentioned tests has to be thoroughly observed. Because of the specificity of values of the AFP-test with malignant hepatoma, we find it useful to perform this test in all patients with chronic liver diseases.
Treatment with the selectively immunomodulatory drug 1-[2-methoxy-6-methyl-3-pyridinyl)methyl]-2-aziridine-carbonitrile (ciamexon, BM-42332), an aziridine derivative, of newly diagnosed type I diabetic patients is reported. In an open pilot study the treatment of Cx led to the independence of insulin in 33.3% of the treated patients.
The new veterinary drug triclabendazole (Fasinex) was tested for its anthelmintic activity against adult Paragonimus uterobilateralis in experimentally infected cotton rats, Sigmodon hispidus. After a single dose of 50, 100 or 150 mg/kg b.w. triclabendazole or after two doses of 5 or 10 mg/kg b.w. given at an interval of 24 hours, 56-61% of the flukes were dead at three to five weeks after treatment. Two doses of 50 or 75 mg/kg b.w. triclabendazole killed all flukes. The flukes, which were still alive three to five weeks after treatment, were significantly smaller than the un-treated controls. Histological sections from these flukes showed severely damaged tissues. It is assumed that these flukes would have died later.

In secretory azoospermia, plasma FSH is usually high and seminal carnitine in relation with epididymal secretion, normal. In excretory azoospermia, FSH is not increased and seminal carnitine is lowered. Here we report the case of 23 patients in whom FSH was not increased and seminal carnitine not decreased. Among these patients 17 had had surgery with vas deferens and epididymis exploration. Testis biopsy for histological analysis was performed. It turned out that most of the patients (13 of the 17) had a genital tract occlusion located at the junction of corpus--caput of the epididymis (9/13) or at the junction testis-epididymis (4/13). The 4 remaining patients showed arrest of spermatogenesis which was before spermiogenesis in 3 of the 4 cases. This study allows (1) to define the diagnosis value of the seminal carnitine assay, which remains the only test to affirm excretory azoospermia when there is a bilateral occlusion located on the genital tract between the epididymis corpus and the ejaculatory ducts, (2) to consider another treatment than epididymo-vasostomy in cases of occlusion caput--corpus (epididymo-epididymostomy), (3) to detect a population presenting a very early arrest of spermatogenesis without any pituitary effect, probably having a special regulation of FSH secretion.

Some test procedures of the computer assisted instrumentation system COMBITEST 2 were examined in order to evaluate the strength of diagnostical differentiation. For this purpose tests for evaluating the concentration capacity and the psychomotor performance, especially tapping, were used in a sample of 92 neurological patients (60 patients with and 32 without cerebral impairment). It became evident, that performance differentiations between patients with cerebral impairment and others are predominantly characterized by speed parameters (reaction time, duration of test), scarcely by parameters of accuracy (number of errors). Configuration - frequency - analyses emphasize deceleration of psychic/psychomotoric speed as diagnostic indicator in cerebral impaired patients: the best pattern was the "configuration" of reduced speed in concentration test and in tapping.

Human platelet microsomes supplemented with iodosylbenzene converted the stable prostaglandin H2 analog 15(S)-hydroxy-11 alpha,9 alpha-epoxymethano-5(Z),13(E)-prostadienoic acid (U46619) into three metabolites (17.5% yield) which were not formed in the presence of specific thromboxane synthase inhibitors. The same three products were also formed among others by incubation of U46619 with liver microsomes from phenobarbital-pretreated rats with NADPH/O2 or with iodosylbenzene. The NADPH-supported metabolism of U46619 was suppressed in the presence of carbon monoxide. Combined gas chromatography/negative-ion chemical ionization mass spectrometry analysis revealed for all three compounds the incorporation of one oxygen atom which according to the electron impact fragmentation pattern had to be introduced either at the 9-methylene group or at the cyclopentane ring. The identification of these metabolites as 9 beta,15(S)-dihydroxy-11 alpha,9 alpha-epoxymethano-5(Z),13(E)-prostadienoic acid and the R and S isomer of 15(S)-hydroxy-11 alpha,9 alpha-(C-hydroxy-epoxymethano)-5(Z),13(E)-prostadienoic acid is only tentative since no reference compounds were available, but clearly thromboxane synthase was acting as an oxene transferase in this reaction. In contrast to U46619, its 9,11-epoxymethano isomer U44069 was found to be only a poor substrate for the oxene transferase activity of thromboxane synthase (1% yield) which indicates a preference for the 9-methylene group of U46619 which is orientated close to the heme iron of thromboxane synthase as evidenced by spectroscopic studies. Low-level chemiluminescence detected following incubation of iodosylbenzene with partly purified thromboxane synthase is in agreement with the formation of an activated (FeO)3+ oxygen species. In summary, these results point to a common role of the thiolate ligand in the oxygen activation mechanism by thromboxane and prostacyclin synthase and liver cytochrome P-450 monooxygenases.
