[Fibrous pleural mesothelioma (pleura fibroma) -- a contribution to the differential diagnosis of pleural tumours (author's transl)].

Five cases of mesothelioma are described, their clinical and radiological findings are analysed and the results compared with those in the literature. The lesion is a benign pleural tumour which rarely causes symptoms; it is usually found accidentally, but can reach an extraordinary size. Surgical removal results in cure. Only a small proportion recur. Distant metastases are not found. Even when there is a recurrence, cure can be attained by a second operation.

The apical cap.

Apical caps, either unilateral or bilateral, are a common feature of advancing age and are usually the result of subpleural scarring unassociated with other diseases. Pancoast (superior sulcus) tumors are a well recognized cause of unilateral asymmetric apical density. Other lesions arising in the lung, pleura, or extrapleural space may produce unilateral or bilateral apical caps. These include: (1) inflammatory: tuberculosis and extrapleural abscesses extending from the neck; (2) post radiation fibrosis after mantle therapy for Hodgkin disease or supraclavicular radiation in the treatment of breast carcinoma; (3) neoplasm: lymphoma extending from the neck or mediastinum, superior sulcus bronchogenic carcinoma, and metastases; (4) traumatic: extrapleural dissection of blood from a ruptured aorta, fractures of the ribs or spine, or hemorrhage due to subclavian line placement; (5) vascular: coarctation of the aorta with dilated collaterals over the apex, fistula between the subclavian artery and vein; and (6) miscellaneous: mediastinal lipomatosis with subcostal fat extending over the apices.

Percutaneous aspiration biopsy of the lung. The double needle technique.

Percutaneous aspiration biopsy of the lung is a well established procedure. Use of a double needle technique allows a single needle pass through the pleura and multiple samples of the lesion. In a series of 440 patients, 91% of malignant lesions were diagnosed with low morbidity and no mortality.

Vineyard sprayer's lung - a rare occupational disease.

"Vineyard sprayer's lung" is an occupational disease found in Portuguese vineyard workers. The radiographic picture resembles that of silicosis with micronodular disease in the early stages and progressive massive fibrosis in later stages, eventually resulting in endstage lung disease. The patients have a downhill course due to respiratory failure. The incidence of bronchogenic carcinoma is also higher than in the general population.

Synthesis and blood pressure lowering activity of 3-(substituted-amino)-1,2,4-benzothiadiazine 1-oxide derivatives.

A series of (substituted amino)-1,2,4-benzothiadiazine 1-oxides has been synthesized and most members of the series have been shown to have blood pressure lowering effects in normotensive rabbits and in spontaneously hypertensive rats. The most active member of the series was 3-[4-(2-furoyl)-1-piperazinyl]-6,7-dimethoxy-1-methyl-1H-1,2,4-benzothiadiazine 1-oxide hydrochloride. This compound in animal tests was equipotent to the known antihypertensive Prazosin.

[Pulmonary blastoma (author's transl)].

Two cases of a rare lung tumour are described and compared with those reported in the literature. It is a tumour with malignant potential which occurs in the lung periphery, shows a preference for males and has similarities with embryonic lung tissue. The radiological features are stressed. The diagnosis depends on the pathology, since the radiological appearances are non-specific.

Fluid accumulation in preexisting pulmonary air spaces.

Four patients with fluid accumulation in preexisting pulmonary air spaces are presented. Three patients developed fluid in an infected bulla; the fourth patient bled into a bulla. The slow spontaneous resolution of the accumulated fluid and the success of a conservative therapeutic approach are emphasized. The characteristic radiologic features of fluid-filled intrapulmonary air spaces are described and criteria for the differentiation from lung abscesses and empyemas are outlined.

Cannabinoids. I. Behavioral effects.

The effects of two new cannabinoids, nabilone and canbisol, have been compared to delta 9-tetrahydrocannabinol (delta 9-THC) and chlordiazepoxide in behavioral tests in mice, rats, dogs and rhesus monkeys. Activity of mice was measured in a photocell device. Oral doses of 5 and 10 mg/kg of delta 9-THC and 200 mg/kg of chlordiazepoxide caused only a decrease in the initial high activity. Doses of 5 and 10 mg/kg of nabilone and 2.5, 5.0 and 10 mg/kg of canbisol decreased the initial high activity but increased the subsequent low activity. In rats delta 9-THC, nabilone and canbisol, but not chlordiazepoxide, slowed muricide and intracranial self-stimulation. Chlordiazepoxide, nabilone and canbisol, but not delta 9-THC, reduced reactivity of septal-lesioned rats. At ehe dosages studied only nabilone and canbisol reduced food consumption by rats. Ataxia in dogs was detected following as little as 0.062 mg/kg of delta 9-THC, 0.032 mg/kg of nabilone and 0.004 mg/kg of canbisol when given intravenously; orally, doses of more than 0.25 mg/kg of delta 9-THC, and 0.1 mg/kg of nabilone or canbisol were necessary. Rhesus monkeys working under multiple fixed-ratio fixed-interval schedules showed an increase in rate at some dose of all three cannabinoids but higher doses reduced responding, and responding was abolished following 3.0 mg/kg of delta 9-THC or nabilone or 0.3 mg/kg of canbisol. Chlordiazepoxide increased responding at all doses studied, 3.0 to 30.0 mg/kg. Nabilone and canbisol resemble chlordiazepoxide in som tests and delta 9-THC in other tests.

Nabilone: a pressure-reducing synthetic benzopyran in open-angle glaucoma.

Nabilone is a synthesized crystalline benzopyran that resembles the cannabinols but is not a tetrahydrocannabinol. Oral administration of 0.5 to 2 mg to patients with open-angle glaucoma reduced the intraocular pressure from 10% to 54% with an average of 28%. Administration of a topical ophthalmic nabilone solution (0.1 mg/drop) to both eyes of adult albino rabbits (1.5 to 3.5 kg) lowered the IOP an average of 25%. The peak action of the nabilone administered in this manner was 60 minutes, with a return to normal IOP by 180 minutes. Tolerance developed in rabbits after one week of topical administration.

[Pneumothorax caused by anaesthesiological manipulations (author's transl)].

Pneumothorax may occur following procedures such as the insertion of a subclavian catheter, intercostal blockade, supraclavicular brachial plexus blockade, endotracheal mechanical ventilation or assisted ventilation of the newborn by the "pulmarca" method. The incidence of pneumothorax was investigated over a 2 1/2 years period. Measurements to prevent, reduce and treat this complication are discussed and suggestions on the aetiology of bilateral pneumothorax in the newborn are described.
