The chronic behavior of blood pressure of 151 ventricle were examined in a study which was set up in a prospective way. The implanted pacemakers were fit out with ventricle controlled Demand-Systems. The period of the observation lasted between 25 and 62 month. After the implantation a decrease of the systolic pressure value is to expect. Precautions to overcome this phase must be taken. The blood pressure from the greater part of the patients increased after three or five month to the values, which were taken before the implantation. The medicinal therapy has to consider for this change of the behavior of blood pressure. Only a little number of pacemaker patients (n = 10) showed during the long-term course a durable decrease of the systolic blood pressure.
During hemodialysis a continuous determination of free thyroid hormone and the basic TSH value was performed in 43 patients. a significant increase of free thyroxine and triodthyronine was found as well as a significant decrease of the TSH basal concentration in serum, which was already subnormal before hemodialysis. In 14 patients a TSH-TRH-test was performed before, during and after hemodialysis. A considerable reduction of the pituitary sensitivity to TRH administration during dialysis was found as compared with values before and after hemodialysis. Because of the increase of free hormone concentration, the reduction of TSH serum concentration and the decrease of pituitary sensitivity to TRH are considered to be a physiological reaction of the pituitary. On the other side a significant increase of the binding affinity of the T3 and T4 nuclear cell receptors correlated with concentration of heparin could be demonstrated. Because this modulation would induce an increasing occupation of the binding valences with thyroid hormone molecules, with a resulting decrease of the production of TSH, this process could also explain the results. Because ther is only a linear relation between the number of occupied nuclear binding sets and the TSH metabolism for T3, the physiological reaction of the pituitary is dependent on an intact conversion of T4 to T3 intracellular. A possible partly defect of this monodeiodination at hemodialysis patients could also explain the published different results concerning the basic concentration of TSH in the serum and the TSH-THR-test.
We examined the nerve conduction velocity (NCV) of 10 patients under long-term lithium therapy and 10 patients after a 3-month lithium therapy. In accordance with the literature concerning patients and normals under experimental treatment with lithium, we also found reduced NCV values. There were no significant changes in NCV during the first 3 months of lithium treatment. This could be due to the relative low lithium levels as well as to symptoms of manic-depressive Illness and the nerve examined.
A group of patients with endogenous depressive disorders at the beginning of a lithium treatment (Li group I) is compared with a second group of patients with the same diagnosis, who can be called lithium responders (Li group II). These two groups are compared in regard to their relationship to their relevant social partner, based on the 'self-concept' and 'ideal self-concept' obtained by means of the Giessen test. Li group II shows significant deviations only in scale 4 of the Giessen test, while the partner group II does not differ essentially in its profile from the partner group I. These findings will be used as a working hypothesis.

Two model systems for hypercyclic organisation constructed from a series of uni- and bimolecular reaction steps were studied under the condition of unlimited growth by means of qualitative analysis and numerical integration of the corresponding differential equations. It is shown that both models lead, within wide ranges of parameter and initial conditions, to the same characteristic dynamical behaviour as the elementary hypercycles and hypercycles with translation introduced in 1978 by Eigen and Schuster.
In 43 hemodialysis patients different thyroid function parameters have been assessed. A characteristic combination of findings occurred in most cases. These were reduced values of total hormones in serum with normal free hormone levels. There was a reduced occupation of binding valences for TBG and a subnormal TSH. The TSH-TRH tests were normal. These changes in serum concentration can be explained at best by accepting a modulation of binding valences of transport proteins in the sense of reduced binding affinity. The various possible inducing mechanisms are discussed and the high sensitivity of binding valences to changes of the acid-alkaline equilibrium and temperature underlined. Both factors may play a role in uremic patients. Since not only in uremic patients but also in patients with various other diseases such changes in serum level can be demonstrated, there is a good chance that different mechanisms induce the same effect.
In order to determine the hormone concentration in a dialysate, the commercially available radioimmunoassay test of Dow-Lepetit for determination of free thyroid hormones in serum had to be modified. The results raised the suspicion that additional protein-bound hormone had passed the membrane. This could be demonstrated by the addition of Thiomersal. In 64 patients examined 1 and 5 hrs after the beginning of hemodialysis the mean total hormone concentration in the dialysate was 12.05 pg/ml for thyroxine and 5.47 pg/ml for triiodothyronine. The resulting loss of total hormone in hemodialysis patients, calculated for 1 week, amounted to 2.25 microgram triiodothyronine and 4.87 microgram thyroxine. The comparison with the physiologic renal elimination of thyroid hormone shows that the hormone elimination in hemodialysis patients is negligibly small. The quantitative determination of the protein concentration in the dialysate gave 384 ng/ml and was close to the limit of sensitivity of the method and consequently uncertain. It can be said, however, that the loss of protein is without clinical relevance.
Several investigators have examined thyroid function in hemodialysis patients. Similar results were obtained: reduced total triiodothyronine and subnormal thyroxine values in serum. This raised the question whether a deficiency in hormones during hemodialysis could be responsible for these findings. The present studies were ade in an attempt to prove or disprove this assumption by determining hormone loss. The loss of thyroxine during hemodialysis was 1.1 microgram. This order of magnitude excludes a hormone deficit caused by dialysis. The assumption that an increased loss of hormone during hemodialysis is the cause of pathologic changes in the serum as compared with the period before dialysis can be considered as disproved.
Range-corrected (according to Lykken) physiological data from the parameters of heart rate (HF), respiratory rate (AF), skin potential (skin potential level; SPL; skin potential response, SPR) and skin resistance level, SRL; skin resistance response, SRR) were obtained under an acoustic activation paradigm in the course of a longitudinal investigation on 7 women with endomorph depression treated with thymoleptic medication. These data form the matrix for two- and threefold analyses of variance as well as for concordance analyses according to Kendall. During six measurements (three each taken in the morning and evening) one finds differing levels of activation: relatively low ones at the beginning and at the end, high ones during the 'labilization phase' of the treatment. The varying 'reactivity' to the diverse examination sequences of an acoustic activation program is little affected by the degree of these changes; patient-specific 'characteristics' of the course of the activation are just as recognizable throughout the course of the investigation as are 'stimulus-specific' reactions. Each of the parameters is appropriate for the estimation of different physiological variables; SPR seems rather to be a situation-specific variable, HF and AF are probably suitable for establishing the level of activation, while changes in SPL seem to represent patient-specific characteristics.
Congenital partial pericardial defect with herniation of the left atrial appendage is a rare anomaly. Clinical appearance of a 25 years old man with this anomaly is described. Although the physical examination showed no pathological signs and the routine laboratory data were within normal limits, the chest X ray showed a prominent shadow of the left cardiac border; the cineangiocardiographic findings revealed that the prominence was due to a large left atrial appendage. Since strangulation of the left atrial appendage is a well known and described potential hazard; thoracic surgery was performed without any complication.

Carcinoembryonic antigen (CEA) was assayed in 18 samples of non-malignant and 21 samples of malignant effusions. In 95% of benign effusion CEA activity was found within normal limits and increased only in one case of Klebsiella pneumonia. In 57% of the 21 malignant effusions CEA levels were elevated and in 4 cases elevated titers were first indications of the malignancy of the effusion. In some cases remarkable differences between serum titer and effusion titer were found. Combined measurement of CEA activity in plasma and effusion increases diagnostic value.

Congenital partial pericardial defect with herniation of the left atrial appendage presents a rare anomaly. Clinical appearance of a 25-year-old man with this anomaly combined with adherance to the left lung hilus and the pericardial sac is described. He suffered from cardiac arrhythmias and dyspnea during exertion. The chest X-ray film at unremarkable physical examination and routine laboratory data within normal limits showed a prominent shadow of the left cardiac border, the cineangiocardiographic findings revealed that the prominence was due to a large left atrial appendage. Since strangulation of the left atrial appendage is a well-known and described potential hazard, thoracic surgery was performed without any complication.
