[Activity of itraconazole against clinical isolates of Aspergillus spp. and Fusarium spp. determined by the M38-P NCCLS method].

The antifungal activity of itraconazole was studied in 101 clinical isolates of Aspergillus fumigatus, A. flavus, A. niger, A. terreus, A. nidulans, A. candidus, A. glaucus, A. clavatus, Fusarium solani, F. oxysporum and F. semitectum. The minimum inhibitory concentrations (MIC) were determined according to the protocol of the M38-P National Committee for Laboratory Standards (NCCLS) document using a microdilution method in 1640 RPMI liquid medium (visual reading at 48 and 72 h incubation). In general, the MIC did not vary with time of incubation, except in a Z. fumigatus strain in which the MIC went from 2 to 16 mg/l. The geometric mean of the MIC and MIC(90) of itraconazole for Aspergillus spp. was 0.44 mg/l and 0.5 mg/l, respectively; and for Fusarium spp. it was 14.1 mg/l and 16 mg/l, respectively. With 0.5 mg/l 75% of the Aspergillus spp. strains were inhibited, and 100% of these strains were inhibited with 2 mg/l. A. niger and A. fumigatus were the most resistant species (MIC(90) 2 mg/l). The MIC of all the Fusarium strains essayed was between 4 and 16 mg/l.

Airway ignition during CO2 laser laryngeal surgery and high frequency jet ventilation.

We present a case of a patient submitted for extirpation of a neoplasm of the larynx, by means of carbon dioxide laser surgery. High frequency jet ventilation was applied by means of orotracheal intubation with two Teflon catheters, 2 mm in external diameter and 30 cm in length, attached with three equally placed strips of adhesive paper tape. One catheter was used to inject the jet volume and the other used to measure the airway pressure. The adhesive strips were moistened and FiO2 was lower than 50%. After 30 min using the laser, an airway fire was noticed. Ventilation was interrupted and the catheters were removed. The patient was reintubated with an endotracheal tube of 6 mm ID and the surgical procedure was continued until the tumour was removed. Two factors contributed to the airway fire: the ignition of the lowest adhesive strip that had dried and the use of the laser in the mode of continuous pulsation.

Women's reports of severe (near-miss) obstetric complications in Benin.

This study examines the validity of a survey instrument on near-miss obstetric complications. Three groups of women--with severe complications, with mild complications, and with a normal delivery--were identified retrospectively in three hospitals in South Benin and interviewed at home. The concept of "near-miss" was used to identify women with severe episodes of morbidity. The questionnaire was able to detect, with some accuracy, eclamptic fits, abnormal bleeding in the third trimester for a recall period of at least three to four years, and all episodes of bleeding independent of timing within a period of two years. Questions concerning dystocia and infections of the genital tract generated disappointing results except when information on treatment was included. Overall, better results were achieved for antepartum and acute events. Severity made a positive difference only in the case of eclampsia, with an increase in sensitivity. The implications of the results for using women's recall of obstetric complications in surveys are discussed.

[Determination of the in vitro antifungal susceptibility of clinically important yeasts using the Sensititre system].

Using Sensititre (AccuMed, USA) we studied the in vitro antifungal activity of amphotericin B, fluconazole, itraconazole, ketoconazole and 5-fluorocytosine against 250 clinical yeast isolates taken from different hospitals, including Candida (151 C. albicans, 15 C. krusei, 14 C. parapsilosis, 11 C. tropicalis, 10 C. glabrata, 4 C. guilliermondii, 3 C. rugosa, 2 C. viswanathii, 2 C. famata and 2 C. kefyr), Cryptococcus (32 C. neoformans and 1 C. laurentii), Trichosporon (2 isolates) and Rhodotorula rubra (1 isolate). All the strains were susceptible to amphotericin B and showed an MIC <1 mg/l. The susceptibility of C. albicans (MIC(90) <256 mg/l), C. krusei (MIC(90) <64 mg/l), C. glabrata (MIC(90) <64 mg/l) and C. neoformans (MIC(90) 32 mg/l) to fluconazole was lower (14% isolates being resistant and 16.8% susceptible depending on the dose). The largest number of strains resistant to itraconazole was observed in C. albicans and C. glabrata (17.2% resistant and 24% susceptible and susceptible depending on the dose, respectively). Ketoconazole and 5-fluorocytosine were not effective in vitro against 12.8% and 2%, respectively, of all the isolates studied. Nine C. krusei and seven C. neoformans (12.9%) showed dose-dependent susceptibility to 5-fluorocytosine.

Lithium treatment causes gliosis and modifies the morphology of hippocampal astrocytes in rats.

The biological basis of the clinical efficacy of lithium in the treatment of mental illness has been extensively studied in neurones, but little is known about the effects of the drug on glia. Recently we showed that treatment of rats with clinically relevant doses of lithium chloride results in a 35% increase in the immunocontent of the astrocyte marker GFAP in the hippocampus. Here we studied the cytology of this phenomenon. Rats were treated for 4 weeks with a lithium diet which resulted in serum Li+ concentrations of 0.6-1.2 mmol/l. GFAP immunocytochemistry of the hippocampus revealed a mild gliosis in the CA1 area and the dentate gyrus which was associated with a change in the orientation of astrocytic processes. In control animals astrocyte processes were mainly orientated perpendicular to the stratum pyramidale, whereas in treated animals the cells were predominantly stellar in appearance.

Functional aspects of cord blood lymphocytes response to polyclonal and allogeneic activation.

With the purpose of contributing to a better knowledge of the phenotypic characteristics and functional activity of umbilical cord blood (UCB) lymphocytes, we have carried out extensive immunophenotyping of these cells, evaluated their immune response to polyclonal and allogeneic activation and then compared these results with those obtained with peripheral blood lymphocytes of healthy donors (PBHD). Our results showed, in CD4+ UCB lymphocytes, an increase of CD38 and CD45RA and a decrease of CD11a (S6F1), CD54 and HLA-DR double positive cells. An increase of CD38, CD45RA and CD56, and a decrease of CD28, CD57 and HLA-DR were observed in CD8+ UCB lymphocytes. Most natural killer UCB cells are CD16+, CD56+, CD57-, and among the UCB cells there is a lower number of CD8+ and TCRgammadelta+ (either CD8- or CD8+), and higher number of CD4+ lymphocytes. After allogeneic stimulus the majority of these phenotypic differences disappeared, which seems to be in agreement with the normal allogeneic response (assessed through MLR, frequency of CTL and helper T lymphocytes precursors) presented by UCB lymphocytes. Regarding the response to polyclonal activation, among the mitogens used, only PHA induced a different result: a lower IFNgamma production by UCB cells.

[Congenital cyst adenomatoid malformation of the lung].

The authors report two cases of Congenital Cyst Adenomatoid Malformation diagnosed by ultrasound in the 24th and 27th weeks of pregnancy. The pregnancies had no other problems until their term and deliveries were normal. Surgical resection of the malformation was made on the 8th day of life in one child and on the 11th day in the other. After 2 years, the first child shows a good evolution in weight and height, without lung pathology. After 2 months, the other child also shows a similar condition.

Relatedness among Basques, Portuguese, Spaniards, and Algerians studied by HLA allelic frequencies and haplotypes.

HLA-A, -B, -DRB1, -DQA1, and DQB1 alleles were studied in Iberian and Algerian populations by serology and DNA sequence methodologies. The genetic and cultural relatedness among Basques, Spaniards, and paleo-North Africans (Berbers or Tamazights) was established. Portuguese people have also maintained a certain degree of cultural and ethnic-specific characteristics since ancient times. The results of the present HLA study in Portuguese populations show that they have features in common with Basques and Spaniards from Madrid: a high frequency of the HLA-haplotypes A29-B44-DR7 (ancient western Europeans), A2-B7-DR15 (ancient Europeans and paleo-North Africans), and A1-B8-DR3 (Europeans) are found as common characteristics. Portuguese and Basques do not show the Mediterranean A33-B14-DR1 haplotype, suggesting a lower admixture with Mediterraneans; Spaniards and Algerians do have this haplotype in a relatively high frequency, indicating a more extensive Mediterranean genetic influence. The paleo-North African haplotype A30-B18-DR3 present in Basques, Algerians, and Spaniards is not found in Portuguese either. The Portuguese have a characteristic unique among world populations: a high frequency of HLA-A25-B18-DR15 and A26-B38-DR13, which may reflect a still detectable founder effect coming from ancient Portuguese, i.e., oestrimnios and conios; Basques and Algerians also show specific haplotypes, A11-B27-DR1 and A2-B35-DR11, respectively, probably showing a relatively lower degree of admixture. A neighbor-joining dendrogram place Basques, Portuguese, Spaniards, and Algerians closer to each other and more separated from other populations. Genetic, cultural, geological, and linguistic evidence also supports the hypothesis that people coming from a fertile Saharan area emigrated towards the north (southern Europe, Mesopotamia, the Mediterranean Islands, and the North African coast) when the climate changed drastically to hotter and drier ca 10 000 years B.C.

Serum antibody reactivity to recombinant mig and whole cell antigens in Mycobacterium avium infection.

Mycobacterium avium is a significant opportunistic pathogen in immunocompromised patients. Moreover, the prevalence of infections in patients without known predisposing conditions has also been increasing in recent years. Patients would greatly benefit from early diagnosis of disseminated infection. Serodiagnostic tests have already been promising in tuberculosis and immunocompetent patients but studies in HIV-infected patients and humoral response to M. avium antigens resulted in conflicting data. We have evaluated the use of the phagocytosis-induced MIG protein of M. avium as a diagnostic antigen. Serum antibody levels of M. avium-infected, HIV-negative patients were significantly elevated for the recombinant MIG (p < 0.001) and also for M. avium whole-cell antigens (p < 0.025) as compared to controls. In contrast, HIV-infected patients with disseminated M. avium infection demonstrated also elevated levels of antibody for the whole-cell antigen (p < 0.00001) but a decreased reactivity for the MIG antigen (p < 0.007). The recombinant antigen proved to have no cross-reactivity with M. tuberculosis antigens as antibody levels were decreased in tuberculosis patients (p < 0.001). Therefore, a simultaneous serological test using recombinant MIG and the whole cell antigens might be helpful in the sometimes problematic diagnosis of M. avium infections in patients without predisposing conditions.

Neoadjuvant chemotherapy with cisplatin and 5-fluorouracil, both in continuous 96-hour infusion, in the treatment of locally advanced head and neck cancer.

In this study, 79 patients with locally advanced head and neck cancer were treated with induction chemotherapy. Cisplatin, 25 mg/m2, and 5-fluorouracil (5-FU), 1000 mg/m2, were employed, both of them in 24-hour continuous infusion over 96 hours, four cycles. The patients later underwent surgery and/or radiation therapy. The response to chemotherapy was 49%: of the complete responses (CR), 56% were histological; 29% were partial responses (PR). With the administration of the fourth cycle, CR increased from 30% to 49%. Once the complete treatment had been finished, 75% of CR and 5% PR were achieved. With a maximum follow-up period of 44 months, overall survival stands at 50%.

[Melanoma and pregnancy].

In spite of being a rare form of neoplasia, melanoma is the ones which occur most frequently in pregnancy. Pregnancy does not seem to aggravate the prognosis. However, the authors present the case of a woman suffering from melanoma, in which the intercurrence of pregnancy led to the development of the disease. After 32 weeks of pregnancy a living foetus was born without any symptoms of the disease, thus proving the low incidence of foetal metastatic lesions.
