A versatile and potentially general approach to the targeting of specific cell types by retroviruses: application to the infection of human cells by means of major histocompatibility complex class I and class II antigens by mouse ecotropic murine leukemia virus-derived viruses.

A technique for delivering genes carried by recombinant retroviruses into specific cell types could have numerous applications in oncology, developmental biology, and gene therapy. As a first step toward this remote goal we designed a procedure allowing in vitro cell targeting by retroviruses. Biotinylated antibodies against the viral envelope protein on one side, and against specific cell membrane markers on the other side, were bridged by streptavidin and used to link the virus to the host. The method was successfully used to infect human cells with ecotropic murine retroviruses by means of major histocompatibility complex class I and class II antigens and appears easily adaptable to other cell membrane markers. Moreover, the sequential protocol we designed, although allowing infection of human cells, requires less stringent safety constraints than would handling of amphotropic virus stocks.

[The posterior sacral plexus].

The posterior sacral branches, when connecting, form the posterior sacral plexus and its terminal branch, the posterior gluteal nerve that we suggest be called the inter-gluteal nerve because of its course and territory. Apart from their anatomic interest, their lesions can explain some neuralgias in sacrococcygeal attacks.

[A new case of necrotizing pneumocystosis in AIDS].

The authors report the case of an AIDS patient presenting with both Streptococcus pneumoniae pneumoniae and pneumocystosis. What was remarkable in that case was the appearance, in an otherwise favourable course, of a large excavation in the territory of the pneumonia. The responsibility of Pneumocystis carinii in this excavation is discussed.

[Interosseous dorsal artery of the first intermetatarsal space: anatomic variations and value in functional vascular studies].

The first dorsal metatarsal artery (MDA) is usually described as the last collateral branch of dorsalis pedis artery (DPA, which anastomoses with the lateral plantar branch of the posterior tibial artery (PTA). Doppler assessment on DMA of a flow does not always mean that DPA is free, but may show a functional plantar arch. The aim of this study was to determinate the patency rate of this plantar arch and to appreciate the part of DPA in the vascularisation of the foot. Anatomical descriptions were compared with our own dissections, (25 cadavers and 55 arteriographies in fetus), angiographic analysis and vascular plastic casts. The functional aspect of this study was made by Doppler examination of DMA using alternative or simultaneous compression of the main arteries of the ankle, until doppler curves were canceled. That was done on healthy volunteers and arteriosclerotic patients. The results show that, from the anatomical viewpoint, the plantar arch is almost always present, since it was found in 88% of adult pieces. Nevertheless, on Doppler examination, the plantar arch was found functional only in 64% of healthy people. The difference between the two rates emphasizes the fact that anatomical anastomosis are note always open, according to the subject and probably to the age. Looking after MDA permeability in patients with arteriosclerosis leads to point out the main vascular access to the foot, for limb salvage, and the leading part of distal anastomosis to keep good trophicity of the toes.

[The spinal periosteal layer and the fibrous sheath of the cervical spinal canal].

At the cervical level, in the foramen, the rachidian nerve is surrounded by two mantles: a dural one and a fibrous one continuing the epidural periosteal sheet of which it is a thickening. We can wonder whether this fibrous sheath is at the origin of some cervical radiculalgias in which one cannot detect any radiological lesion of the skeleton or disk.

Sexual abuse of children--a hospital-based study.

Eighty-eight cases of suspected sexual abuse of a child were investigated at Red Cross War Memorial Children's Hospital during 1985. Twenty cases of rape, 15 of incest and 7 of sexual interference were confirmed; but 47% of cases remained unconfirmed. All social strata were affected and victims ranged from 10 months to 13 years in age. An increase in the numbers of victims seen is ascribed to the vigilance of a child abuse team. Implementation of the Child Care Act No. 74 of 1983, which makes notification of suspected abuse compulsory, will inundate social welfare agencies with cases for investigation. Community-based studies of child abuse are needed to plan necessary expansion of staff.

Blood lead concentration in children after gunshot injuries.

Blood lead and erythrocyte zinc protoporphyrin values were examined in 23 children who had suffered gunshot (including multiple shotgun) injuries. No child had any evidence of lead poisoning between 15 and 27 months after the injury despite, in some cases, the retention of multiple pellets. Reference to previous publications suggests that lead poisoning is an unlikely complication of gunshot injury, providing a bullet or pellet is not retained in contact with a synovial cavity.

[Respective frequency and radioclinical features of 150 lung diseases observed in 125 patients with human immunodeficiency virus infection].

Seventy out of 125 patients with HIV infection had diffuse alveolo-interstitial pneumonia usually caused by an opportunistic infection, notably pneumocystosis. Nineteen patients had only localized lung opacities due either to usual or tuberculous bacterial infections or to Kaposi's sarcoma. In 10 patients with pleural effusion or mediastinal adenopathy, the condition was due to Kaposi's sarcoma (n = 4) or to mycobacteriosis (n = 3). An opportunistic or usual infection was demonstrated in 17 of the 51 patients with normal radiography of the chest. Finally, 37 patients free from infectious or tumoral pathology had isolated lymphocytic alveolitis.

Synthesis and biodistribution of labelled p-iodo phentermine (IP), N,N,-dimethyl-p-iodo phentermine (IDMP) and N-isopropyl-p-iodo phentermine (IIP) in rats.

p-Iodo-phentermine (IP) and two of its derivatives, N,N,-dimethyl-p-iodo-phentermine (IDMP) and N-isopropyl-p-iodo-phentermine (IIP) were synthesized and radiolabelled with iodine by isotopic exchange. They were evaluated as potential brain imaging agents and compared to IAMP biodistribution in rats did not show any superiority to IAMP.

[Significance of the presence of Candida sp. in bronchopulmonary secretions. Contribution of pulmonary biopsy].

Candida SP is often found in broncho-pulmonary secretions (expectorations, bronchial aspirations, bronchiolo-alveolar lavage...); however, its presence even in massive amounts, is difficult to interpret. Is it a sign of broncho-pulmonary candidosis or mouth-pharynx contamination? The different clinical and biological elements, in particular immunology in immunodepressed patients, bring little information. In this article, we present the retrospective study of 18 patients hospitalized in Pneumology Department of Tenon Hospital for severe extensive pneumopathy of unexplained origin. In these patients, for the most part immunodepressed, Candida sp was found in large amounts in at least a sample (expectorations [5] and/or bronchial aspirations [4] and/or bronchiolo-alveolar lavage [12]. No lesion suggestive of candidiasis was found during bronchial endoscopy carried out for these different samples. In pulmonary biopsy with diagnostic in view, these yeasts were found only in one patient. In this patient, as C. tropicalis was associated with Pneumocystis carinii and Cryptococcus neoformans, it was difficult to know the respective part of each of these agents in pulmonary symptomatology. In the other 17 cases, the direct examination and cultures were negative. The presence of Candida sp in the different broncho pulmonary secretions is not synonymous of bronchopulmonary candidiasis; likely it is, in most cases, a sign of contamination of mouth-pharynx origin.

[Cutaneous territory of the posterior branches of spinal nerves. A review of Dejerine's scheme].

This report concerns not only anatomical study of posterior branches of the spinal nerves, but has also a clinical aim. The cutaneous territories and the sites of pain irradiation are precised. In the cervico-thoracic region emphasis is placed on the importance of the fourth cervical posterior branch and of the second dorsal posterior branch. At the lumbar and sacral levels the authors distinguish three pain pathways: from up to down and from outside to inside: the first corresponding to posterior branches of 11th and 12th dorsal nerves, the second to those of first, second and third lumbar nerves and the third to sacral nerves.
