[Study of the remote results of five cases of unilateral condylar hyperplasia in the adult].

Exceptionally bilateral, condylar hyperplasia result in graduated facial asymmetry . The principal treatment is the condylectomy. (Condylar head and a portion of the neck of the condyle are removed.) Sometimes we must complete this condylectomy by mandibular osteotomy or resection of a segment of body of the mandible. The occlusal relationships of the teeth are affected by this process. That's why surgery treatment must be associated with orthodontics and prosthetic preparations. We use in post-operative immediate an intermaxillary fixation and traction by elastics. This allow to have a better congruence between the dental arches, and spare functional troubles.

[Current aspects of xeroderma pigmentosum: apropos of two new cases].

Two recent cases of children treated for xeroderma pigmentosum are reported, and knowledge of the disease updated based on numerous reports published over the last few years, particularly by french authors ( Vaillant et al., 1979 and Mouly , Dufourmentel and Papadopoulos , 1980). Points discussed include the similarity of histology and course of the malignant cutaneous lesions with those of adult outdoor workers (co- carcinogenic role of ultraviolet rays ?), and the effect of the introduction of vitamin A acid into treatment on the course of the general and cutaneous lesions (immunocompetence, tumoral growth rate). A further feature discussed is the question raised by the surgical attitude: radical, extensive and mutilating as in the first case, or limited and conservative, with consequent long-term inconveniences, as in the second case. The persistence of an extremely poor prognosis after more than a century of study of the disease (it received its name from Kaposi as early as 1882. . . !!) is a strong argument for imposing the preventive measures which are still applied with difficulty at the present time.

[Experimental study of a new technic of sutureless tubal anastomosis in the rabbit].

The authors have tested an original method of tubal anastomosis in the rabbit, which does not require suture material. The principle of the operation is the intubation of the ovarian tube by a silicone elastomere tube which acts as a guide for healing. From the 11 rabbits operated, the authors obtained 7 pregnancies, i.e. 63,6%. The results of this series do not appear to be any different from the pregnancy rates obtained by other teams with microsurgical techniques in the rabbit. The authors conclude that their method is as reliable, but more simple.

[Various characteristics of industrial noise].

In this paper, broad characteristics of industrial noise are recalled briefly. High levels and multiple sources require special rules for the measurement or risk, such as the location of microphones at the height of the worker's ear. Industrial noise is either continuous-, and in such case one measurement is enough to evaluate the amount of risk for the inner ear-or fluctuates with time. This is the reason why noise level meters which integrate noise levels along time, must be used in many industrial situations and risk expressed in equivalent level. The unit for industrial noise is the dB(A) which is now universally admitted as reflecting satisfactorily the differential sensitivity of the ear. Lastly, the worker's exposure is also determined by the size (among other factors) of the workshop in which the noise source is located. Authors recommend that noise measurement in industry should not be limited to hygienic purposes and protection against industrial deafness but extended to other requirements of workers. In this respect, physical noise level measurement should be considered as one part of a noise abatement program and be completed by a survey on workers feeling of disturbance and uneasiness.

[Non-drug cure of heroin addicts in association with chemical analysis as a factor of evaluation].

Group psychotherapy programme is applied in a heroin addiction treatment center in conjunction with a systematic planning of chemical analyses along the course of the cure. The addicts live under inspected regime. At best 25 out of 70 cases (36%) conclude the programme with occasional and non repetitive failures. The question of their follow-up beyond their treatment period arises in order to better evaluate their retrieval.

[Epidemiologic analysis of occupational accidents using data of the Swiss National Accident Insurance].

The present study is aimed to relate occupational accidents, as recorded by SAIF for 1974, to several risk factors among which the size of industrial enterprises. The results seem to demonstrate that very small enterprises (of less than ten employees) produce the highest rate of occupational accidents and accumulate risk factors. Nevertheless, before going very much further in the comment of results, the authors emphasize the limitations of the available statistical material. Further limitation in planning an ergonomical prevention programme on the basis of those records is to be related to the parameters chosen to explain the genesis of occupational accidents.

[Visual stress in work at visual display units (author's transl)].

The ergonomic study of VDUs demonstrates the importance of visual stress in the total work load. By means of a laboratory experiment and authors showed that the distribution of light in the visual field exerts a major influence on the operator's visual acuity. For this reason, it is of great interest to measure the visual performance in light conditions as close as possible to real working conditions. The results of this experiment can also be of practical value in formulating guidelines for the design of appropriate work places corresponding to the visual requirements of VDU operators.

[Acquiring otoneurological results by computer. Evaluation after 2 years of experience].

Nearly all test performed in audiology and vestibulometry are based on quantitative paradigms and lead to results expressed in numerical form that are well suited for computer processing. Since 1980, the Oto-rhino-laryngology department of the Geneva University Hospital stores the complete patient record with all the results obtained in the otoneurological investigations (audiogram, impedance measurements, binaural tests, auditory-evoked potentials, electro-nystagmogram) on floppy disks using a PDP-11 micro-computer equipped with a low-speed printer and a Tektronix 4010 graphics terminal with an attached hard-copy unit. One of the main advantages of such a computer system is the gain in time for secretarial work such as patient record updating and archival. All test results are manually entered on a keyboard and the computer automatically produces reports with tables and graphs. The storage of patient files on floppy disks is fast, consumes little space and makes possible all types of statistical studies on the recorded clinical data. One should, however, be aware of the fact that the daily clinical use of even such a small computer system requires a solid technical infrastructure and lots of programming time. Nevertheless, the balance of our two years experience with this system is clearly positive and we foresee that the computer will become more and more an appreciated technical tool for several types of clinical work.

[Is Swiss national accident data useful for epidemiological studies if the latter is intended for validating preventive measures?].

In the general frame of an inquiry concerning the development of occupational health in Switzerland and its forthcoming place inside the Swiss health system, data on accidents were communicated to us by the Swiss National Accident Insurance Fund. In this paper, the question is raised whether those data may be effectively submitted to the epidemiological analysis in order to quantify the accident risk and to validate preventive measures. Results will be presented in a subsequent article.

[Preliminary study on the integration of occupational medicine into the Swiss health system].

The aim of our research project is to gather the necessary data in order to introduce a nationwide discussion on the development in Switzerland of occupational health and its place inside our health system. Two types of data will be collected: on one hand the needs, which should be expressed at different levels: employees, employers, undertakings, inspection offices, specialists. On the other hand, preventive actions will be listed. To fulfill the objective of this research project, several means will be used such as foreign and international literature, gathering of epidemiological figures, interviews. As a conclusion, suggestions will be made as regards the actions to be undertaken in order to meet the needs.
