[Hearing loss caused by leisure noise].

Although noise in general can induce hearing loss, environmental noise represents an important risk for children, teenagers and young adults. Epidemiological investigations now support the occurrence of an increasing number of irreversible hearing losses in these groups. Major causes of hearing loss are toys (guns), explosives and electroacoustically amplified music delivered by head sets or heard in discotheques and open air concerts. Clinical indications are discussed.

[Comments on the revision of the Königstein instructional pamphlet].

The "Königsteiner Merkblatt (juris prudence)" is an instructional booklet of the general association of juris prudence involves accident insurance ("Hauptverband der gewerblichen Berufsgenossenschaften") in Germany. Medico-legal examinations of occupational noise-induced hearing loss are detailed. In contrast to prior editions, the 4th edition of this booklet, published in January 1996, has been extended by the definitions of legal regulations important for the physician in making medical decisions. Also the number of tests recommended for audiological investigation has been extended. The rules for estimating hearing disability and the calculation of individual handicaps have been changed, and the handicaps for complete deafness in one or both ears have been increased to 20% or 80%, respectively. The most important changes and extensions of the new edition are cited and discussed.

[Hearing loss caused by leisure activity-induced noise].

The number of persons with hearing disability in most countries is more than 10%. In developing countries of the third world, inflammations are the most important cause of hearing dysfunction, but in more civilized countries, more and more individuals are injured by a noisy environment and by noise that is individually produced. In this way, a great number of young people acquire defects of hearing which are not reparable. As a consequence the German government has tried to control the possible danger to the community from noise and has arranged that exposure to unnecessary noises be reduced by standards and regulation.

[Early detection of hearing disorders. 10 years experience with screening more than 8,000 infants].

Prevention and early detection of deficits of the developing human being demand more and more complex solutions. Screening programmes have been developed to detect deficits and diseases before they cause symptoms. A well-organised and accurately performed screening programme can detect preliminary signs of diseases and may prevent irreversible damage. Early detection of deficits only makes sense if it is followed by consequences. Not only early detection of impairment or of children which are threatened by impairment is a very important medical task, but also the interdisciplinary cooperation in therapy. The most serious impact of hearing deficits in newborn is on the lack of speech-acquisition. It is an indisputable fact that the early detection of hearing impairment in newborn markedly improves the prognosis for further language development if early medical and/or rehabilitative therapy takes place. The following description of 10 years of experience with a newborn hearing screening programme that is effected in cooperation with the maternity ward and the ENT-clinic since 1981, can supply further arguments in favour of the need for early detection, the discussion on reflex audiometry, and to emphasise the need for cooperation of all relevant specialists.

[Comparative speech audiometry studies with compact disk].

We compared the results of speech audiometry, performed with recorded tape and CD, respectively, in 21 subjects with bone conduction hearing loss, and in 27 subjects with sensorineural hearing loss. In all cases, speech audiometry was performed by using the German Freiburg speech test material, recorded in 1969 on the basis of the German DIN standard 45626. In subjects with an air-bone gap, the hearing loss for speech is 2.3 dB smaller if tested by use of CD, and the speech recognition is 10.5 points better, respectively. Smaller differences could be found in sensorineural HL: For CD, the hearing loss is 1.2 dB smaller, and the speech recognition score is raised by only 1 point. These differences are statistically not significant, and the calculation of hearing impairment and individual hearing handicap from the tables commonly used in Germany in medicolegal expert opinions must not be corrected, if records on CD are used basing on the recordings from 1969 according to DIN 45626. The handling of compact discs has many advantages in relation to the handling of tapes.

[Expert assessment of hearing loss within the scope of the social compensation law and the law regarding severely handicapped patients].

Special problems are described concerning expert opinions on hearing disability and hearing loss with regard to the German Social Rights for Compensation and the laws for handicapped persons. In some aspects there are similarities to the rights of accident insurance, but disability programs question only the degree of handicap present and not the source of the hearing loss. The expert opinion on the subject's ability to work in his or any other profession must only determine the patient's ability to hear and the tasks needed to fulfill the profession. Special attention is given to the problems arising from tinnitus.

[Long-term results of stapedectomy and stapedotomy].

We have followed up 91 patients for 3 or more years after stapes surgery for otosclerosis, 59 by stapedectomy and 32 by stapedotomy. The surgical technique was chosen randomly. Both surgical techniques achieved significant hearing improvement, but in the higher frequencies (3 kHz) the results of stapedotomy are better and more stable than those after stapedectomy. Surgical revision was necessary in 5 patients--1 after stapedotomy, 4 after stapedectomy.
