[Conceptual validation of a psychiatric syndrome classification].

The authors present the first results of an investigation concerning the conceptual validity of a phenomenological classification of psychiatric patients. Conceptual stereotypes for 8 syndromes, derived from applications of cluster analysis and related numerical taxonomy methods to BPRS symptom rating profiles, were obtained by 33 experienced French psychiatrists. Analysis of the profile data reveal that the names chosen to describe the empirical prototypes elicit 8 clearly differenciated syndromes and that the experts' understanding of the nomenclature is highly consistent. Problems concerning the relationship between two of the empirical prototypes and the corresponding conceptual stereotypes are discussed.

[Methodology of therapeutic trials in psychopharmacology (author's transl)].

The paper discusses the general methodologic aspects of therapeutic trials in psychopharmacology under three main headings: definition of the concepts of disease, treatment, and healing. In each case, the author discusses the main difficulties, and gives concrete examples. In conclusions, the complexity of the methodologic problems is stressed, but the author considers that the cost of using correct experimental designs is more than balanced by the safety of the statistical conclusions.

Depression↗

[Plasma levels of imipramine and desmethylimipramine and antidepressant effect during controlled therapy(author's transl)].

24 depressed impatients received a standardized treatment by imipramine, 6 among them receiving also levomepromazine. Plasma concentration of imipramine and DMI were controlled weekly during the study, and standardized assessment of clinical state were made by a psychiatrist who was unware of biochemical findings. Interaction between the antidepressant and neuroleptic metabolism is shown by a significant increase of DMI levels, though the clinical effect of the drug association could not be asserted. The therapeutic effect seems mainly dependant of etiology of depression. Endogeneous depressions improve more than other types. Among endogenous depressions a significant correlation was found between the degree of clinical improvement after three weeks and DMI level, sum of imipramine + DMI level, but not with concentration of imipramine alone.
