Comparison of ELISA, radioimmunoassay and stool examination for Schistosoma mansoni infection.

A comparison was made of the sensitivity and specificity of four diagnostic tests for Schistosoma mansoni infection in a community of 516 untreated persons in St. Lucia, West Indies. Prevalence of infection as obtained by: (i) the Bell filtration technique was 44.4% (one filter) and 63.2% (three filters); (ii) the Kato thick smear, 60.2%; (iii) by radioimmunoassay (RIA), 73.3%; and (iv) enzyme-immunoassay (ELISA) 70.9%. The age distribution of persons serologically positive but parasitologically negative showed these to be mostly children and persons 40 years old and over. By means of a statistical test due to Cochrane, it was concluded that there was no evidence to indicate a difference between paired serological tests and paired parasitological tests in their diagnostic capability. There was a very significant difference between the Bell technique and the other three tests. The ELISA emerged as a less satisfactory test than the RIA or the Kato thick smear. The levels of sensitivity and specificity of each test were measured by Armitage's "J" index. The reliability of the Bell filtration technique was 64%, of the ELISA 68%, of the RIA 78% and of the Kato 85%.

Ambulatory evaluation of nephrolithiasis. Classification, clinical presentation and diagnostic criteria.

Using the ambulatory protocol previously described, 241 patients with nephrolithiasis were evaluated. They could be categorized into 10 groups from the results obtained. Absorptive hypercalciuria type I (87 per cent male) comprised 24.5 per cent and was characterized by normocalcemia, normal fasting urinary calcium (less than 0.11 mg/100 ml glomerular filtration), an exaggerated urinary calcium following an oral calcium load (greater than 0.20 mg/mg creatinine), normal urinary cyclic adenosine monophosphate (AMP) (less than 5.4 nmol/100 ml glomerular filtration) and serum parathyroid hormone (PTH), and hypercalciuria (greater than 200 mg/day during a calcium- and sodium-restricted diet). Absorptive hypercalciuria type II (50 per cent male) accounted for 29.8 per cent; its biochemical features were the same as those for absorptive hypercalciuria type I, except for normocalciuria during a restricted diet and low urine volume (1.42 +/- 0.55 SD liter/day). Renal hypercalciuria (56 per cent male), disclosed in 8.3 per cent, was represented by normocalcemia and high values for fasting urinary calcium (0.160 +/- 0.054 mg/100 ml glomerular filtration), urinary cyclic AMP (6.80 +/- 2.10 nmol/100 ml glomerular filtration) and serum PTH. Primary hyperparathyroidism (57 per cent female), accounted for 5.8 per cent, typically included hypercalcemia, hypophosphatemia, hypercalciuria and high urinary cyclic AMP. Hyperuricosuric calcium urolithiasis (100 per cent male) comprised 8.7 per cent, and was characterized by hyperuricosuria (776 +/- 164 mg/day) and urinary pH exceeding pK for uric acid (5.91 +/- 0.33). In enteric hyperoxaluria (60 per cent female), encountered in 2.1 per cent of cases, urinary oxalate was increased (6.29 +/- 13.2 mg/day). Noncalcium-containing stones were found in 2.1 per cent of the patients with uric acid lithiasis (100 per cent male) and in another 2.1 per cent of the patients with infection lithiasis (60 per cent female). These conditions were typified by low urinary pH (5.29 +/- 0.12) and high urinary pH (6.69 +/- 1.16), respectively. Renal tubular acidosis was found in one patient (male, 0.4 per cent). In 10.8 per cent of the patients (81 per cent male), no metabolic abnormality could be found, although urine volume was low (1.41 +/- 0.51 liter/day). Hypercalciuria could not be differentiated between absorptive hypercalciuria and renal hypercalciuria in 5.4 per cent of the patients. Thus, this ambulatory protocol disclosed a physiologic disturbance in nearly 90 per cent of the cases and provided a definitive diagnosis in 95 per cent of the patients.

Lack of agreement between subjective ratings of instructors and objective testing of knowledge acquisition in a urological continuing medical education course.

Objective scores from multiple-choice questions before and after a postgraduate course were compared to subjective ratings of the instructors at a 3-day seminar. The objective mean scores after the course were significantly higher than the scores before the course (p less than 0.0001). There was no correlation between test results and subjective ratings of instructors.

The effect of topically applied agents on ultraviolet erythema in guinea pigs.

It has been shown previously that ultraviolet erythema (UV) is partially the result of a local release of prostaglandins. We have studied the effects of different classes of pharmacological agents applied topically to the skin of guinea pigs and have found indomethacin and pirprofen, both prostaglandin synthetase inhibitors, to be highly effective suppressors of UV erythema. Our studies appear to further substantiate that prostaglandin synthesis and release may be the primary mechanistic process in the production of erythema and that the model itself can be predictive of both therapeutic and prophylactic effects of agents against sunburn.

Fracture of the penis with associated laceration of the urethra.

Two patients with rupture of the urethra secondary to sexual trauma are described. Our results with the surgical treatment of such injuries are reported and the potential etiologies are postulated. One patient had an excellent result, while the other had a residual urethral stricture.

[Focal dermal hypoplasia with apocrine nevi and striation of bones (author's transl)].

In a 22 year-old female patient affected with focal dermal hypoplasia, multiple apocrine nevi occurred in the sternal region; these nevi may be part of the syndrome. Roentgenograms of the long bones revealed marked longitudinal striations. This anomaly represents a characteristic and hitherto underestimated feature of the syndrome.

Schistosomiasis japonica on Jishan Island, Jiangxi Province, People's Republic of China: persistence of hepatic fibrosis after reduction of the prevalence of infection with age.

Hepatic fibrosis due to schistosomiasis japonica was examined by ultrasonography in a cross-sectional community study of 825 individuals on Jishan Island, Jiangxi Province, China. The prevalence of active infection was 39.4% with peak infection in the 10-19.9 years age group followed by a significant decline. A similar pattern was observed for intensity of infection. The prevalence of hepatomegaly in the midsternal line > or = 6 cm peaked at 60% in the fourth decade and remained elevated. A progressive increase in the severity of hepatic periportal fibrosis was observed with age, with advanced fibrosis peaking in the fifth decade. The proportion of individuals with advanced fibrosis was significantly greater in males than in females despite equivalent prevalence and intensity of schistosome infection. In addition, a positive association (P < 0.01) was found between periportal fibrosis and both hepatomegaly > or = 6 cm and splenomegaly. This study suggests that the natural history of schistosomiasis japonica in this hyperendemic community in China is marked by persistence of hepatomegaly and schistosome-induced periportal fibrosis in adults despite a decrease in the prevalence of infection.
