[Contribution of cytological punctures to the diagnosis of kidney tumors].

Analysis concerned 47 kidney tumours. Puncture was under ultrasonographic control using 22 G needles. The sample was obtained by suction. The authors then describe the technique of fixing and reading of smears. Four types of indications were adopted; diagnostic uncertainty between benign and malignant nature (22 cases), high risk surgical patients in whom a pre-operative histological diagnosis is very desirable (10 cases), search for a primary neoplasm in the presence of metastatic spread (5 cases), routine puncture of tumours seen by ultrasonography (10 cases). Puncture material was uninterpretable in 6 cases. In 26 cases of puncture-cytology could be compared with those of histological examination (nephrectomy or surgical biopsy). 3 puncture-cytology results were un-interpretable, whilst in 21 cases the correlation was perfect (17 malignant tumours and 4 benign tumours). In 1 case puncture-cytology led to suspicion of the malignant nature of the lesion. Puncture cytology was erroneous in one case (a malignant tumour confused with an angiomyolipoma). In 21 cases no sample was obtained and it was not possible to establish the correlation between cytological and histological results. Amongst these 21 cases 3 were uninterpretable whilst in 18 there was no evidence of error on the basis of the subsequent clinical course. No complications were seen and in particular no abdominal wall dissemination. Sensitivity of the method was of the order 80 to 87%. Interpretation of puncture-cytology is difficult in urothelial carcinomas, renal adenoma and oncocytoma. By contrast puncture-cytology is particularly reliable in angiomyolipoma and metastatic lesions of the kidney.

[Silastic spermatocele].

The study was carried out in two stages. An initial experimental study in the rat showed that it was possible to attach a spermatocele to the rat epididymus and to collect spermatozoa during the months that followed. The clinical trial involved the implantation of 8 spermatoceles in 5 male patients. The technic is simple, as predicted by the animal study but, as previously described by numerous other authors, the communication between epididymus and spermatocele is obliterated within several months. These findings confirm the possibility of use of a spermatocele that is well tolerated and of simple functioning, but which does not allow fertile spermatozoa capable of producing pregnancy to be obtained.

A rapid rabies enzyme immuno-diagnosis (RREID): a useful and simple technique for the routine diagnosis of rabies.

A Rapid Rabies Enzyme Immuno-Diagnosis (RREID) technique has been developed. This technique for the diagnosis of rabies was performed in microplates which had been previously sensitized with IgG to purified antinucleocapsids. Suspensions of homogenized material were incubated in the plate and the specific binding of rabies antigen was revealed by the use of the same IgG conjugated with peroxidase. With the RREID technique it was possible to detect rabies antigens in brain specimens with the same specificity and sensitivity as that of the direct immunofluorescence test or the neuroblastoma cell inoculation technique regardless of the species of animal from which the specimen was derived. Moreover, RREID was performed with fox salivary gland specimens with the same results as were obtained with brain specimens. RREID does not require an UV light microscope and a photometer is not essential. It is a useful and simple technique for the routine laboratory diagnosis of rabies.

The influence of the type of immunosorbent on rabies antibody EIA; advantages of purified glycoprotein over whole virus.

Two types of in vitro assay (enzyme immunoassay and sero-neutralization test) for the titration of rabies antibodies were used to assay sera from mice and humans immunized with cell culture vaccines or neural tissue vaccines. Enzyme immunoassays (EIA) were performed in plates sensitized with whole virus, purified glycoprotein or purified nucleocapsid. Neutralizing antibody titres were determined by the rapid fluorescent focus inhibition test (REFIT) and by an in vitro seroneutralization test including a rapid enzyme immunotitration of intracellular antigens (REITICA). The results obtained with sera of immunized mice and humans showed that (1) cell culture vaccines mainly induced the synthesis of antiglycoprotein neutralizing antibodies; and (2) neural tissue vaccines induced a high synthesis of antinucleocapsid non-neutralizing antibodies and a more or less important synthesis of antiglycoprotein antibodies depending on the origin of the tissue used for their preparation. Consequently, it was emphasized that when using EIA, the antibody titration must be run in glycoprotein-coated plates rather than in whole virus-coated plates to appreciate correctly the immunizing potency of a rabies vaccine, especially neural tissue vaccine.

[Value of the prostatic puncture biopsy in the diagnosis of prostatic cancer. Apropos of 271 cases].

271 patients have been submitted to prostatic needle biopsy between january 1973 and september 1984. Sensitivity and specificity of transperineal prostatic biopsy for prostatic carcinoma are respectively 89% and 98%. Complications are met in 10% of these cases. Rectal examination has been compared to these results. Sensitivity is 75% and specificity 70%. Prostatic biopsy appears to be a good method for diagnosis of prostatic carcinoma but less effective than fine needle aspiration for cytologic examination.

[Endoscopic dilatation of ureteroileal stenosis in Bricker diversions. Apropos of 11 patients].

Eleven patients with stenosis of an uretero-ileal anastomosis following Bricker's operation underwent endoscopic dilatation of these stenoses via an inferior approach with Eder-Puestow's apparatus or Savary's dilators. The preliminary results were favourable in 7 patients with clinical, endoscopic and radiological improvement. An uretero-ileal drainage catheter was inserted in each case and was changed regularly after the operation. In one patient, a stone trapped in the anastomosis was able to be removed via the endoscopic procedure. No complications were observed. The two technical failures required surgical re-operation in one case and dilatation via a superior approach with an inflatable balloon catheter. This technique demonstrates the value of gastrointestinal endoscopy in the treatment of certain complications in patients with a urinary diversion using a segment of the intestinal tract.

Use of a monoclonal antibody for quantitation of rabies vaccine glycoprotein by enzyme immunoassay.

The use of a monoclonal antibody specific for the envelope glycoprotein of rabies virus has been used in a direct enzyme immunoassay to quantify the glycoprotein content of rabies vaccines produced in two kinds of cell culture. The results of this direct enzyme immunoassay are well correlated with the in vivo NIH potency test. This test may be a useful tool to rapidly and accurately control the antigenic value of a vaccine. It could also be used for the "in process' control of vaccine production before deciding whether a vaccine batch may reasonably be subjected to the NIH potency test.

Rabies immunosome (subunit vaccine) structure and immunogenicity. Pre- and post-exposure protection studies.

Rabies immunosomes (glycoprotein anchored on pre-formed liposomes) have been prepared in order to study their structural, biological and immunological properties. The glycoprotein molecules appear to have the same orientation on the immunosome as on the viral particle: (1) electron microscopy analysis shows particles of 40 to 70 nm with spikes protruding outward, (2) one particular epitope shows the same accessibility to a neutralizing monoclonal antibody as on the viral particle. When injected into animals, rabies immunosomes are cleared from the organism by a process different from that for the liposomes used to anchor the glycoprotein: a higher rate of transition through the spleen is observed with immunosomes than with purified glycoprotein or liposomes. Immunosomes induce high levels of neutralizing antibodies and protect animals against challenge with virulent strains. This protective activity is not altered after several months of storage at 4 degrees C. Furthermore, rabies immunosomes were shown to be efficient in post-exposure treatment of laboratory animals that had been experimentally infected with a lethal dose of a rabies wild strain.

Stage III non-seminomatous germ cell tumours of the testis--patients treated at the Centre Leon Berard.

The study of the survival of a series of patients with stage III non-seminomatous germ cell tumours of the testis enables us to confirm two points: the use of cis-platinum has multiplied the survival rate for stage III patients by 3, the concept of stage III includes various metastatic states with different prognoses. The use of a sub-classification, like that proposed by Samuels, allows for a better definition of the natural history of these lesions.

A rabies vaccine produced in a non-tumorigenic rabbit cell line.

From the trypsinized skin of a rabbit embryo, a fibroblastic cell line has been obtained (FEL cells). The primary seed lot has been established at 11th passage. The tests for tumorigenicity on nu/nu mice were negative (10(4) cell/animal) at passage level 11th and 50th. Cell growth and morphology were not altered when 100 passages were performed. FEL cells were tested for their susceptibility to rabies virus. The production of virus on FEL cells was investigated after infection by the P.V. rabies virus strain adapted to several cell types (bovine fetal kidney, Vero and BHK cells). For the evaluation of an experimental rabies vaccine produced with this substrate, FEL cells were compared to Vero cells. The synthesis of glycoprotein (EIA) and the immunogenicity (NIH test) were similar for the vaccines obtained with both cell systems. FEL cells could therefore be an alternative to Vero cells for the production of a cell culture inactivated rabies vaccine.

Structural and immunogenic characteristics of rabies immunosomes.

Rabies immunosomes are prepared by the anchorage of glycoprotein molecules to preformed liposomes. Virosomes are associations of glycoprotein molecules with viral or non viral lipids. Virosomes can be obtained spontaneously after removal of the detergent used for lipids solubilization. Under the electron microscope immunosomes look like homogenous spherical vesicles (50-60 nm) evenly covered with spikes, although glycoprotein molecule orientation in virosomes is not yet clearly established. In addition, the size of virosomes is very heterogeneous. When glycoprotein molecules are in immunosome form, their accessibility to a lectin (WGA) is identical to that of the purified virus. Under the same conditions with virosomes accessibility is weaker. Protection induced in mice is ten to twenty fold higher with rabies immunosomes than with purified glycoprotein or virosomes. The results presented here and previous reports (10) show that immunosomes have structural and immunogen characteristics closer to those of purified and inactivated virus than does any other form of glycoprotein lipids association.

Coding strategy differences between constant and variable segments of immunoglobulin genes.

Vertebrate immunoglobulin (Ig) mRNAs reveal intraspecies variation in codon usage distinct from that seen with yeast or bacterial genes. Comparison of all available Ig gene sequences shows that %(G + C) in codon position III is consistently lower in variable (V) segments than in constant (C) segments. I find an even lower %(G + C) in the hypervariable domains of V segments. This analysis suggests that base substitution in Ig genes correlates positively with local A + T content.
