[Endobronchial Abrikosov's tumor. Two new cases associated to malignant tumors].

Endobronchial localizations of the granular cell tumours or Abrikossoff's tumour are very rare (6%) but their association with malignant tumours is exceptional and perhaps fortuitous. The clinical manifestations are generally the consequence of bronchial erosion and blocking. Bronchial endoscopy is the essential examination which allows the tumour to be seen and a biopsy to be made. Pathological examination confirms the diagnosis. The typicall cell of granular cell tumour is a large, polygonal one with finely granular eosinophilic cytoplasm and a small dark vesicular nucleus. Such cells are arranged in syncytial masses containing long, spindleshape cells with elongated nuclei. If the optical and elctron microscope description is well known, the histogenesis of the disease is still in dispute; the neurogenic origin seems to be the best one, but at present there is not enough evidence to make this assumption a certainty. Surgical exeresis is the only therapy to bring about a cure, but clinical supervision is necessary because of a possible relapse.

[Value of isotope study with ytterbium-169 citrate in the diagnosis of pulmonary opacities of cancerous nature].

Out of 60 observations, the authors deduct the value of the isotopic study with Ytterbium169 citrate in the diagnosis of cancerous pulmonary opacities. Ytterbium169 is a radiolanthanide with a period of 32 days. Patients receive intravenously 500 muCi of Ytterbium169 citrate. Recordings are made on the 2nd, 7th and 14th day. Valuable information on the data of the fixation rate on the 2nd and 14th day can be obtained from drawing a curve of fixation. As shown by the dosimetric study the radioactive pollution is minimal; nevertheless the dose of irradiation should not be underestimated and not recommended for young patients. Reliability of the examination is great: the percentage of true positive results is satisfactory (87%) and above all the percentage of false positive results (7.7%) much below that of other isotopes (30-40%). Results are excellent for peripheral opacities which escape the usual diagnostic mean. The main failure of the examination is the relatively frequent lack of fixation, not prejudicial in itself as it has no diagnostic value. Indications and limits of this examination are drawn from this study.

Experimental venous thrombosis: what animal model?

A new experimental venous thrombosis model is described and its role relative to other models is defined. Previous models are not satisfactory for all types of investigation. Opposite, present model based on the three classic thrombogenic factors is suitable for venous scintigraphy and evaluation of different forms of therapy for venous thrombosis of the limbs. So this model permits research on diagnosis and therapy of thromboembolic disease.

[Waldenström's disease and periarteritis nodosa: fortuitous association? (author's transl)].

A 48-year-old-woman had simultaneously a peri-arteritis nodosa (PAN) and a Waldenström disease. PAN's symptoms were quickly regressive, neuropathy excepted (with slowness) under treatment by cyclophosphamide and prednisone. Hemopathy is now stabilized. Such a pathological association is exceptional (2 other cases in literature). Is it accidental? No cryoglobulinaemia or circulating immune-complexes or HBS antigen were found. Immunofluorescence study of arteriolar lesions in muscle showed a polyclonal fixation in inflammatory granuloma and monoclonal (IgM lambda) on endothelial side. Passive deposit is not excluded, because research of specific activity of IgM lambda (against fibrin or endothelial cells) was negative. Waldenström's disease has probably facilitated apparition of necrotizing angeitis but mechanism is not proved.

[Dilution hyponatremia and conserved natriuresis in pulmonary tuberculosis (author's transl)].

Association of dilution hyponatremia and normal natriuresis produce an inappropriate secretion of antidiuretic hormone. In this pulmonary tuberculosis case we report manifestations along with the other biological criteria of inappropriate secretion of antidiuretic hormone. All disturbances cease when pulmonary tuberculosis is cured. This syndrome may be due either to disturbance in the regulatory apparatus of antidiuretic hormone secretion or to the antidiuretic principle in tubercular lung tissue.
