[Infantile spasms: clinical course].

The authors report on the factor which interferes on follow-up of children affected by infantile spasm. On the basis of their series and on the data of literature the factor more indicative for the clinical evolution are: type and dosage of treatment, onset early or delayed, and the aetiology. While for the first two factors the opinion of the expert are remarkable discordant, there is a general agreement that the evolution of infantile spasm is mainly based on the causes determining such kind of epilepsy.

[Early relapse of herpes simplex encephalitis. Clinical and therapeutic implications].

A case of Herpes simplex virus encephalitis (HSVE) is reported in a 5 year old boy, who relapsed twice during the acute phase of the disease. The second relapse was completely cured by increasing the daily treatment with acyclovir to a dosage of 60 mg/kg intravenously for two weeks and 30 mg/kg by mouth for a further week. The full disappearance of the symptoms and the regression of cerebral lesions could be correlated to a prompt diagnosis and to the dosage of acyclovir employed, higher than that suggested for diseases caused that was by other members of the Herpes virus family.

Bone marrow hypoplasia associated with acute viral hepatitis in four children.

Four cases of children, aged 2 to 11 years, with acute viral hepatitis and bone marrow hypoplasia are reported. Three patients presented only jaundice and hepatomegaly; one also had liver failure. All the four patients underwent bone marrow aspiration which showed bone marrow aplasia. In two of the four patients a liver biopsy revealed patchy necrosis with inflammatory mononuclear cell infiltrate. Half of the patients received a bone marrow transplantation; the remaining patients were treated by intravenous infusion of immunoglobulins, growth factors and steroids. The main epidemiological and etiological features of such association are also reported.

High resolution computed tomography (HRCT) assessment of beta 2-agonist induced bronchodilation in chronic obstructive pulmonary disease patients.

The evaluation of airway obstruction reversibility in chronic obstructive pulmonary disease (COPD) patients is currently performed by means of an indirect assessment of drug inducing variations in pulmonary function tests. Imaging techniques, especially high resolution computed tomography (HRCT), usually provide a complete evaluation of lung parenchyma (bronchial and vascular structures), but so far they have never been applied to visualize the effects on the bronchopulmonary tree of some pharmacologic stimuli (beta 2 adrenergic agonist), currently used in clinical practice to disclose the presence of airway reversibility. In order to assess the possible role of HRCT in this setting, five COPD patients have been subjected to a double functional radiologic evaluation before and after salbutamol-induced broncho-dilation, with a rigorous assessment of bronchial diameter changes by means of "bronchus-vessel" ratio, currently used for diagnosing bronchiectasis in COPD patients. The results of this experimental study enable us to visualize drug induced broncho-dilation, with a direct assessment of airway reversibility in these patients, and to show a good correlation between functional and HRCT findings, raising the possibility of evaluation COPD patients with abnormal spirometric results or with early signs of lung involvement.

Magnetic resonance imaging using endoanal coil in anal canal tumors after radiochemotherapy or local excision.

The ideal method for evaluation of anal canal tumors after radiochemotherapy and/or local excision remains controversial. Endoanal magnetic resonance imaging (EMRI) is a new, promising technique. The effectiveness of EMRI is reported in a study of 24 patients. Axial SET1-weighted and TSET2-weighted, sagittal and coronal T2-weighted sequences using Fat-suppression were acquired. In 4 cases, the low signal/noise ratio did not allow a diagnosis. In 6 cases, the lesion was not detected. Parietal hypo-intense thickening was detected in 14 patients, but it was not diagnostic for disease recurrence. In this study, EMRI showed 58.3% sensitivity and 41.6% specificity, thus it was not useful in the follow-up of anal tumors.

[Radiologic assessment of femoro-patellar instability. Personal experience and review of the literature].

PURPOSE: To propose a simple and reproducible radiological evaluation of patellofemoral instability to enable the orthopaedic surgeon to choose the best therapy. MATERIAL AND METHODS: We have evaluated retrospectively the radiographs and CT examinations of 25 patients, 15 female and 10 male, aged between 18 and 32 years. They underwent surgical treatment for patellar instability in the past 4 years. Lateral retinacular release has been performed in every patient, medial transposition of the anterior tibial tuberosity has been performed in 23 patients and in 5 of them also its distal replacement, proximal Insall's realignement was performed in 20 cases and only 1 patient underwent trochleoplasy. Preoperative conventional radiographs included antero-posterior view, true lateral view (exact superimposition of the posterior trochlear edges) and axial views at 30 degrees of knee flexion. On lateral view, the patellar height was evaluated based on Caton-Deshamps index and trochlear dysplasia assessed using the methods suggested by Dejour. A CT examination of both knees was performed: the images were taken first with knees in extension, with and without quadriceps contraction, then in flexion at 20 degrees. Patellar "bascùle" angles were measured both with quadriceps relaxed and contracted as an index of quadriceps dysplasia; the TAGT in extension was evaluated as an index of the degree of lateral position of the anterior tibial tuberosity. Preoperative conventional and CT findings were compared with those obtained postoperatively at 30.4 months from the surgical intervention. RESULTS: All patients were free from complaints after surgery. In 5 patients a high patella was corrected by distal realignement of anterior tibial tuberosity. A trochlear dysplasia with different degrees of severity was present in all cases and it was not modified by surgical treatment, with the exception of the patient who underwent trochleoplasty. In the 20 patients who underwent proximal Insall's realignment, patellar "bascùle" angles--pathological before surgery--were restored to normal values with the exception of 3 cases. In 23 patients who underwent medial transposition of anterior tibial tuberosity, the values of the TAGT were all normalized: nevertheless, in three patients low values of the TAGT were found after surgery and this condition may predispose to the development of medial patellofemoral by iperpression syndrome. CONCLUSIONS: In patients suffering from patellofemoral instability, a radiological protocol which includes conventional radiographs in two projections and a CT examination both in extension, with and without quadriceps contraction, and in flexion at 20 degrees supplies all the information needed for evaluating patellar height and mobility, trochlear dysplasia, valgism of the knee and the degree of excessive lateral position of the anterior tibial tuberosity. Careful evaluation of such abnormalities is important for selecting the best surgical treatment for each patient. It's a simple, quick, and accurate protocol that may be reproduced, even using different radiographic equipment. The collaboration between the radiologist and the orthopaedic surgeon is crucial for the correct interpretation of radiological findings, which must be evaluated together with clinical findings so that an adequate therapeutical plan could be proposed.

[Neonatal muscular spinal atrophy: a case report].

Spinal Muscular Atrophy (SMA) is an autosomal recessive disease characterized by diffuse proximal and distal weakness due to deletions of the survival motor neuron (SMN) gene localised on chromosome 5q13. Pathological studies show decreased numbers of motorneurons in spinal cord. SMA was initially sub-classified clinically into three types base on age at onset and clinical course. SMA type 1, Werdnig-Hoffmann disease, has an onset within the first 6 months and death within the first two years. In contrast, SMA type II has an onset after six months of life and the children achieve the ability to sit unaided. Children with SMA type III (Kugelberg-Welander) usually have normal milestones in the first year and achieve the ability to walk but then show evidence of mild weakness. The prognosis is good and the clinical course is not (or very slowly) progressive. Recently, Dubowitz described a new form of SMA called type 0 with a neonatal onset, facial weakness, progressive and fatal clinical course. These infants show generalised hypotonia, the lower limbs are more affected than the upper with marked weakness of all axial muscles. We report a case of SMA, uncommon for the early onset and the respiratory difficulties. The diagnosis has been done by genetic analyses showing a SMN mutation.

[Induction of liver metastases in the rat. An experimental model for research on magnetic resonance].

The authors report their experience in the implantation of hepatic metastases in rat, for research in magnetic resonance (MR) imaging. Tumor cells were directly injected with a fine needle into an hepatic lobe, after enzymatic disaggregation of the cell suspension. Cryopreservation of the cell line is possible after the above-mentioned preparation. Tumor implants were observed in all the 20 animals studied, with size varying from 0.5 to 2.7 cm, according to the time elapsed between inoculation and sacrifice of the rat. On MR imaging the tumors presented the same signal intensity as the corresponding human pathology, on both T1- and T2-weighted sequences. The animal model can be used for the evaluation of the efficacy of MR imaging of the liver, and mainly for the assessment of new organo-specific contrast agents.

[Phlebography with MR in the study of deep venous thrombosis].

MR phlebography was recently proposed as a possible alternative to contrast venography in the detection of deep venous thrombosis. We focused our work on the study of the pelvic veins since it is at this level that contrast venography and the other noninvasive techniques, i.e., phlebography, impedance plethysmography and US, exhibit their major limitations. Thirty patients underwent MR phlebography: 13 of them had a diagnosis of DVT of the pelvic venous district and the other 17 had negative results for this condition. All the patients were also examined with color-Doppler US at the pelvis and legs. In all the patients submitted to MR phlebography, thrombosis site and presence were demonstrated, with diagnostic information also on its extent and adhesion to vein wall. To conclude, MR phlebography can provide contrast venography-like images in a noninvasive way, with high accuracy (100% sensitivity and 90% specificity) especially in the pelvic district where the limitations of other techniques are more apparent. Larger series of patients must be studied to assess the actual role of MR phlebography in the patients with DVT or at high risk for this condition.

[Integrated imaging of the breast. Use of a hypermedia program and multimedia archives for teaching purposes].

This paper deals with the results obtained with a computerized senology system developed at the Institute of Radiology of "La Sapienza" University in Rome. The system combines a hypermedia program with a multimedia didactic archive integrated with the radiologic information system. These programs have been developed on Macintosh computers: the hypermedia one on a Macintosh IIfx with 160-Mb hard disk and 8-Mb RAM and a Supercard software, the multimedia archive on Macintosh IIvx, IIvi and Quadra 650 units, connected with an Ethernet network to a server Quadra 950 (RAM: 20 Megabytes; optical disk: 1 Giga) and using the 4th Dimension as software. The basics of breast anatomy, radiologic semiology and breast diseases are illustrated with the hypermedia program: such a system has many advantages to teach the basics requiring just a process of learning by heart. The multimedia archive allows to classify a large number of difficult and uncommon clinical cases, according to the ACR code. Thus, it is useful also to teachers to study particular subjects, including anatomical variants and uncommon conditions. In conclusion, we believe these systems to be valuable tools in the formation and update of the physicians devoted to the study of breast diseases.

[Magnetic resonance in gallbladder carcinoma. Its diagnostic reliability].

The use of magnetic resonance imaging (MRI) to study the liver is currently on the increase. Our study was aimed at evaluating MR diagnostic reliability in the detection and staging of gallbladder carcinomas. Thirty-one patients with gallbladder carcinoma were studied. MRI was performed with a 0.5 T superconductive magnet, using spin-echo (SE) T1- and T2-weighted sequences and gradient-echo (GE) refocusing sequences to evaluate portal system patency. Sixteen patients underwent surgery, while 24 were submitted to angio-CT. MR findings correlated well with angio-CT results. In 4 cases only MRI could not define tumor extent due to the presence of breathing artifacts. In 6 cases the tumor was identified at intracholecystic level, while in 21 cases it involved gallbladder fossa and infiltrated liver parenchyma. In 10 of these patients, the lesion involved also the hepatic hilum, while 7 patients exhibited liver metastases. Twenty patients presented lymphadenopathy at both the hilum and the portocaval space. In 8 patients GE refocusing sequences allowed the evaluation of neoplastic thrombosis of the portal vein. Of 16 patients who underwent surgery, MRI underestimated 2 cases--namely, the presence of micrometastases in one case and tumor spread greater than demonstrated by MRI in the other case. To conclude, in our experience MRI can be considered a valuable diagnostic technique to detect and stage gallbladder carcinomas.

[The information management of a radiology department: the development of a new type of software for the archiving of alphanumeric data and images].

The authors report the main characteristics and the goals of the development of the RIS of the II Chair of the Institute of Radiology, La Sapienza University, Rome. The system was developed with a commercial software (4th Dimension), for use with an Ethernet network and Macintosh Apple computers. One of the main problems was to obtain a user-friendly system. The main options of our system are: booking, registration, exam execution, reporting, archives and statistics and system administration. The main characteristics of our RIS is that it allows important images to be archived in limited number and at low resolution. The aim is to use images for consultation and teaching purposes, not for diagnosis which is made on the original images. Low resolution images permit to use limited storage space. Image quality is very similar to that of the original images for the equipment connected on line with the RIS--i.e., US and MR units in our institute. Conventional radiographic and CT images are digitalized by two scanners with maximum resolution of 4k x 4k x 11 bits. To date, good results have been obtained. Our RIS has been used by the medical and non-medical staffs, without any particular instruction and has allowed us to organize and make faster department management and reporting.

Non-invasive evaluation of the biliary tree with magnetic resonance cholangiopancreatography: initial clinical experience.

Magnetic resonance cholangiopancreatography is a new, non-invasive imaging technique for visualization of the biliary ducts. Magnetic resonance cholangiopancreatography was performed on 136 patients (20-87 years old) with a superconductive magnet at 0.5T (Philips Gyroscan T5). Volumetric images on coronal planes were acquired; a T2 weighted turbo spin echo sequence (TR = 3000; TE = 700; number of excitations = 8; echo train length = 128; Acquisition time = 5'48") with respiratory compensation was performed. Images were reconstructed on coronal planes rotated at different angles using the MIP algorithm. When neoplastic disease was detected additional images on axial planes (SE Tlw: TR/TE 300/10 and turbo spin echo T2w: TR/TE 3000/120) were acquired. Magnetic resonance cholangiopancreatography allowed images of diagnostic value to be obtained in all cases. In choledocholithiasis, the technique had a sensitivity of 91.6%, specificity of 100% and overall diagnostic accuracy of 96.8%. Of the 48 patients with stenotic lesions, 16 cases were correctly characterized as benign and 30 as malignant. Two cases of focal chronic pancreatitis were misdiagnosed as pancreatic head carcinoma. In patients submitted to bilioenteric anastomosis, the technique was able to detect dilation of intrahepatic ducts, stenosis and associated stones in the 8 positive cases. In all 11 patients with chronic pancreatitis, dilated Wirsung duct and the stenotic tracts were revealed. In conclusion, magnetic resonance cholangiopancreatography can be considered as a technique able to completely replace diagnostic endoscopic retrograde cholangiopancreatography. However, further studies are necessary for a better evaluation of potential advantage and disadvantages.
