Diagnosis and treatment of post-traumatic hydrocephalus.

From August 1972 to December 1980, 36 patients were operated on at the Neurosurgical Clinic of the University of Pavia for post-traumatic hydrocephalus. The materials and methods for making a correct diagnosis of post-traumatic hydrocephalus and for follow-up assessment are discussed. Of our patients who were surgically treated 45% improved, 22% remained unchanged, 22% died and we have no follow-up data on the remaining 11%. Comparisons are made with data from the literature regarding the immediate and long-term results.

Association of a malignant glioma and an aneurysm of the middle cerebral artery. Report of a case.

A case of association of a malignant glioma of the left frontal lobe and an aneurysm of the middle cerebral artery is reported and the available literature reviewed. The operative mortality of patients with the association of a brain tumor and a cerebral aneurysm is very high (40%). This is the third case reported in the literature in which it was possible to remove the tumor and clip the aneurysm with immediate good surgical results and it is the first case in which the technique of late surgery for malignant glioma was adopted.

The expression of genes of the steroid-thyroid hormone receptor superfamily in central nervous system tumors.

We studied the expression of ten genes (encoding the receptors for glucocorticoid, mineralocorticoid, progesterone, androgen, estrogen, thyroid, retinoid acid, vitamin D) belonging to the steroid-thyroid hormone receptor superfamily (STRS) in 12 neuroepithelial tumors, 12 meningiomas and 2 human glioblastoma cell lines. Our method, based on the polymerase chain reaction, allowed the simultaneous amplification of cDNAs of the STRS genes. On average, 7 STRS genes were simultaneously expressed in each sample. Our study indicates that many STRS gene are commonly co-expressed in human CNS tumors. The importance of our results for the ongoing and proposed hormonal treatment trials is discussed.

The effect of different glucose endovenous administrations on amylin and insulin blood concentration in healthy subjects.

Islet amyloid polypeptide (IAPP) or amylin is a 37 amino-acid peptide involved in carbohydrate metabolism. It is a hormone secreted from the pancreatic cells which is reported to be co-secreted with insulin. Its function and secretion is not well known. A paracrine inhibition of insulin secretion could be the main function of this hormone. Different hypotheses are considered regarding IAPP co-localization with insulin in the secretory granules and its subsequent secretion. In the attempt to clarify these controversial findings we evaluated the plasma concentrations of IAPP and insulin during different intravenous glucose administrations in healthy patients. Twenty normoglycemic patients (10 females, 10 males, age +/- SD 37 +/- 9 years) underwent two different endovenous glucose administrations: a) "long duration" infusion, in which 500 ml of 33% glucose solution was administered in 60 minutes, b) a standard intra-venous glucose tolerance test (IVGTT) characterized by an infusion of 0.33 g/Kg of glucose in 4 minutes. Blood samples were collected at fixed times (from 0 to 180 minutes) to assay IAPP, insulin and glucose concentration. The IAPP secretion seemed to be related more to the glycemic variation than to the insulin secretion. A proportional behavior of IAPP and insulin was observed only in the first 90 minutes of the long duration infusion. The highest increases of IAPP concentrations were found when there was a rapid glucose decrease in the blood. These findings suggest a significant role of IAPP in regulating the rapid decreases of glycemia.
